    Summary Reasons for performing study Sesamoiditis is a common radiological finding in yearling Thoroughbreds. The condition is believed to be associated with suspensory ligament branch injury (SLBI), which is known to affect racing performance. The presence of subclinical suspensory ligament branch change (SSLBC) in untrained yearlings has not been investigated. Associations between Sesamoiditis, SSLBC and subsequent SLBI would allow more accurate prognoses to be made regarding the development of SLBI. They could also provide opportunity for intervention and prevention of SLBI. Objectives To test our hypotheses that untrained yearling Thoroughbreds with Sesamoiditis would be more likely to have ultrasonographic findings of SSLBC and those horses with concurrent Sesamoiditis and SSLBC would be more likely to develop clinical suspensory ligament branch injury with training. Study design Prospective, cohort, observational study. Methods Yearling Thoroughbreds located at a single training centre were evaluated at the onset of their training careers and for the next 9 months. Radiographic and ultrasonographic examination of bilateral forelimb proximal sesamoid bones and suspensory ligament branches was performed. Results Fifty horses were eligible, resulting in 200 forelimb sesamoid/suspensory ligament branch pairs. A significant relationship existed between possibly significant (PS) Sesamoiditis and PS SSLBC (P<0.001). The odds ratio of sesamoids with PS Sesamoiditis also demonstrating concurrent PS SSLBC was 5.1 (95% confidence interval 2.68–9.70, P<0.001). A significant relationship also existed between the concurrent presence of PS Sesamoiditis and PS SSLBC and the subsequent development of clinical signs of SLBI (P<0.001, odds ratio 11.7, 95% confidence interval 4.1–33.4). Conclusions The associations identified in this study highlight the importance of ultrasonographic examination of suspensory ligament branches in horses with PS Sesamoiditis. This information should allow more accurate prognostic advice regarding potential SLBI development and also provide opportunities for intervention and prevention of clinical SLBI. Since Watanabe’s description in 1972 [1], although scarce, treatment of various pathologies such as pigmented villonodular synovitis [2–4], hallux rigidus [2, 5], hallux valgus [6–8], osteochondral lesions [2, 9, 10], Sesamoiditis [11, 12], and gouty tophi [4, 13–15] has been reported in the past. The understanding of surgical approaches and available portals will allow surgeons to strategically address specific lesions while minimizing postoperative pain and complications.

While most injuries can be treated nonsurgically, there are a number of specific injuries that require a high index of suspicion, careful management, and in some cases, surgical intervention.     Acute great toe (Hallux) pain is a common complaint encountered by the primary care physician. Pathological conditions can vary from acute trauma to acute exacerbation of underlying chronic conditions. Delay in treatment or misdiagnosis can lead to debilitating loss of function and long-lasting pain. This review endeavors to discuss the pertinent history, physical exam findings, radiographical evidence, conservative treatment options, and surgical management for the musculoskeletal causes of acute and acute on chronic great toe pain in the adult population. The acute pathologies discussed in this review are hallux fractures and dislocations, turf toe, sand toe, and sesamoid disorders. The chronic pathologies discussed include hallux rigidus, hallux valgus, and chronic Sesamoiditis.

    Background Pathologic conditions of the sesamoids can be a source of disabling pain for patients, particularly during toe-off. Some underlying causes include osteonecrosis, inflammation, arthritis, and fracture. Nonoperative treatment is the initial standard of care, and has demonstrated satisfactory outcomes overall; however, operative management may be indicated in cases of pain refractory to conservative management. Sesamoidectomy is an uncommon procedure with risk of potential complications, but may be warranted in select cases of failed nonoperative treatment. Methods A retrospective chart review was conducted at one institution from 2009 to 2018. Twelve patients diagnosed with fibular Sesamoiditis were treated with sesamoidectomy. Baseline patient demographics as well as postoperative outcomes were recorded. Results All 12 patients underwent fibular sesamoidectomy using the plantar approach following which their symptom (pain) resolved. Average follow-up for this cohort was 35 months. Of the sample, two patients experienced transient neuritis, one patient developed a superficial infection, and one had painful postoperative scarring. Hallux varus deformity was not observed in any patients. Conclusion Fibular sesamoidectomy may be a safe, viable procedure for patients with Sesamoiditis who fail conservative measures.

    Abstract:

    Background: Medial sesamoidectomy has been reported to be a safe and effective means of treating a range of pathologies including chronic Sesamoiditis and fracture nonunion. The purpose of this case series is to determine common clinical indications for medial sesamoidectomy and to evaluate the postoperative clinical course and outcomes. Methods: A retrospective chart review was conducted on patients who had isolated medial sesamoidectomy from 2009 to 2018. Demographics, comorbidities, physical exam variables, operative findings, radiographic measurements including hallux valgus angle (HVA) and intermetatarsal angle (IMA), and clinical outcomes including visual analog score (VAS), Foot Function Index (FFI), and complications were gathered. Results: Twenty-six patients (13 males and 13 females) were identified to have undergone tibial sesamoidectomy with a mean age of 49.8±18.5 years. VAS improved significantly (5.27±2.41 to 0.91±1.14, P Conclusions: Isolated tibial sesamoidectomy, when utilized judiciously and with the right indications can improve pain and ulcer healing.
