    Abstract:

    Twenty patients with Sleep Apnea syndrome were examined polysomnographically with and without an Esmarch Sleep Apnea Appliance. The relationship between the therapeutic effect of the device and dentofacial morphologic characteristics of the patients were analyzed with the use of diagnostic casts and cephalometric radiographs. The Apnea index or number of apneic episodes per hour decreased significantly (p < 0.0001) after insertion of the Appliance. The diagnostic casts and cephalometric analyses revealed that the device was most effective for patients who exhibit a tendency toward micrognathia (p < 0.03) and a short soft palate (p < 0.01). The Esmarch Appliance is indicated for the treatment or diagnosis of Sleep Apnea syndrome.

    Obstructive Sleep Apnea (OSA) and snoring are common related conditions with major health and social implications. OSA is a progressive disease with symptoms of daytime Sleepiness and chronic cardiovascular morbidity A mandibular advancement oral Appliance is the only nonsurgical management modality available if continuous positive airway pressure (CPAP) cannot be tolerated. A patient who had been diagnosed with OSA was successfully treated but developed a posterior open bite and symptomatic temporomandibular joints (TMJ). An integrated approach to managing his OSA and TMJ conditions enabled him to have a comfortable and stable bite and to continue using his obstructive Sleep Apnea Appliance. Management of OSA with an oral Appliance should be handled by a dentist who is trained and experienced in the overall care of oral health, temporomandibular joints, dental occlusion, and associated structures. A team approach starting with the diagnosis of OSA by a physician and management by a dentist is described.

    Suomessa on arviolta 300 000 uniApneaa sairastavaa. Määrän on katsottu olevan jo kansantaudin luokkaa. UniApnealla tarkoitetaan hengityshäiriötä, jossa hengitystiet voivat unen aikana ahtautua joko osittain tai kokonaan. Lievässä uniApneassa ja kuorsauksen hoidossa hoitomuotona käytetään uniApneakiskoa. Hampaistoon tukeutuvalla suukojeella tuodaan alaleukaa yhdessä kielen lihaksiston kanssa eteenpäin, jolloin hengitystiet avartuvat ja nielun lihakset aktivoituvat. Kiskohoidon on todettu tutkimuksissa olevan tehokas hoito uniApneaan. Opinnäytetyö uniApneakiskoista uniApnean ja kuorsauksen hoidossa toteutettiin toiminnallisena opinnäyteyönä Turun Ammattikorkeakoulun Hammastekniikan koulutusohjemalle. Opinnäytetyön tarkoituksena oli koota tiivistetty yleiskatsaus uniApneasta sekä Suomessa käytettävistä eri uniApneakiskomalleista. Tarkoituksena oli myös kuvata uniApneakojeen valmistusvaiheet. Toiminnallisessa osiossa valmistettiin Herbst-tyyppinen uniApneakisko ja dokumentoitiin Narvall CC 3D-tulostetun uniApneakiskon valmistusvaiheet. Tavoitteena oli koostaa opas uniApneakiskon valmistukseen hammastekniikan opiskelijoille sekä hammaslaboratorioissa työskenteleville. Oppaan tueksi ja pohjatiedoksi yhdessä teoreettisen viiitekehyksen kanssa koostettiin selvitys eri uniApneakiskomalleista. Suomessa käytössä olevia hammaslaboratorio-valmisteisia uniApneakiskoja ovat yksiosainen Monoblock, teleskooppiaisoilla kiinnittyvät Herbst- ja IST-kiskot, edestä metallilukolla kiinnittyvä T-Tap -kisko, 3D-tulostettu Narvall CC -kisko, siivekkeillä yhteen ohjautuva SomnoDent ja kokonaan muovista valmistettava Silensor-sl -uniApneakisko. UniApneakisko valmistetaan akryylistä prässäämällä, lämpömuovautuvasta vetolevy-muovista tai yhdistelemällä näitä. UniApneakiskoja valmistetaan myös lasersintraamal-la. Kiskon tulee olla miellyttävä käyttää ja sallia leuan liikkeet leukaniveloireiden välttämiseksi. Tehokas kisko pitää alaleuan oikeassa asemassa ja on säädettävissä. Toimiva kisko on yhdistelmä käyttömukavuutta, tehokkuutta ja kestävyyttä. UniApnean esiintyvyys on lisääntynyt voimakkaasti, samoin kun erilaisten uniApnea-kojeiden valmistuskin. Päivitetyt tiedot käytössä olevista uniApneakiskomalleista sekä opas uniApneakiskon valmistukseen hyödyntävät hammastekniikan alaa. UniApneakiskojen opetusta tulisi sisällyttää myös hammastekniikan koulutusohjelmiin.There are approximately 300 000 people who have Sleep Apnea in Finland. It can already be classified as a national disease. Sleep Apnea is a respiratory disorder, where airways can narrow completely or partially during Sleep. A Sleep Apnea device is used to treat mild Sleep Apnea and snoring. With an oral Appliance the lower jaw and tongue muscles are brought forward. This causes respiratory airways to expand and pharyngeal muscles to activate. A Sleep Apnea device is proven in several studies to be an efficient treatment for Sleep Apnea. This thesis was executed as a functional thesis for Turku University of Applied Science for Dental Technician programme. The purpose was to present an overview of Sleep Apnea and the different types of Sleep Apnea Appliances used in Finland and to describe the process used in fabrication of Sleep Apnea Appliances. In the functional part of thesis a Herbst-type Sleep Apnea Appliance was made and a manufacture process of Narvall CC 3D-printed Sleep Apnea Appliance was documented. The aim of this thesis was to collect a guide for fabrication of Sleep Apnea Appliance to the students in dental technology and employees in dental laboratories. A research of different kinds of Sleep Apnea Appliance designs and theoretic references were gathered to support the guide. Appliances manufactured in dental laboratories in Finland are unionparted Monoblock, telescopically guided Herbst- and IST-Appliances, titan lock attaching T-tap-Appliance, 3D-printed Narvall CC-Appliance, patented wings guided SomnoDent and an all plastic Silensor-sl-Appliance. Sleep Apnea Appliances are made by pressing from akrylic, from thermoforming vacuum pressed plastic foils or combination of these materials. It can also be made from lasersintred polyamid. Sleep Apnea Appliances should be pleasant to use and allow jaw movements to prevent temporomandibular joint discomfort. An efficient Appliance keeps lower jaw in right position and is adjustable. It is a combination of convenience, effectiveness and endurance. Sleep Apnea appears to increase rapidly, as well as treatment with oral Appliances. Updated information on different types of Sleep Apnea Appliance as well as fabrication guide to Sleep Apnea Appliances are beneficial to the field in dental technology. Sleep Apnea Appliances should be included in dental technician programme as well
