    Abstract:

    The Sonograph er examining a patient with suspected portal hypertension must have thorough knowledge of both the principles of Doppler Sonography and the pathophysiology of portal hypertension. In this chapter, we will describe the various findings of color Doppler Sonography in patients with portal hypertension. When a patient is referred for Doppler Sonography, the diagnosis of portal hypertension may already be known, or it may be completely unsuspected and discovered during an examination done for other purposes. We will illustrate the various manifestations of portal hypertension, with particular emphasis on porto-systemic collateral vessels and methods of tracing them from their origin towards their recipient vein.

    Abstract:

    The accuracy of Doppler Sonography in the evaluation of testicular torsion was tested in 65 boys (aged 0-18 years) with acute scrotal pain or swelling. All patients underwent pulsed Doppler Sonography of both testes, followed by scintigraphy (n = 16) and/or surgery (n = 34) and a close clinical follow-up for 4-8 months (n = 31). Color Doppler Sonography was performed in 29 patients. The testicular artery was deemed patent if Doppler shifts from branches within the parenchyma could be found. Surgery revealed 19 cases of testicular torsion, 17 of which were diagnosed with Doppler Sonography. There were six technical failures, in which no signals could be found on either side. In four boys, no color signals were obtained in either testis but subsequent examination with a mechanical sector scanner and pulsed Doppler Sonography yielded arterial signals.     AbstractPurpose: We determined the shortcomings of color Doppler Sonography for the diagnosis of testicular torsion.Materials and Methods: We reviewed 5 cases of testicular torsion in which findings on color Doppler Sonography were inconsistent with surgical findings.Results: Color Doppler Sonography showed blood flow in 2 patients who had necrotic testes at surgery. In 3 other patients blood flow was also normal by color Doppler Sonography of whom 2 were treated for epididymitis with subsequent loss of the testes and 1 underwent salvage by immediate exploration.Conclusions: Testicular torsion is a clinical diagnosis requiring consideration of many variables. Color Doppler Sonography is helpful but should not be regarded as the definitive discriminator.

    Purpose: Although color coded duplex Sonography is useful for visualizing testicular perfusion in patients with clinical suspicion of testicular torsion, fear of overlooking the condition itself remains. Thus, routine scrotal exploration in every patient with acute scrotal pain is common at many centers.
