    Abstract:

    Fabry's disease is a rare inherited metabolic disorder caused by a deficiency in the enzyme α-galactosidase A. It can affect almost every organ, including the lungs. Confirmation of lung involvement has depended on invasive bronchial biopsy specimens or brushings to confirm the presence of typical lamellar inclusion bodies within bronchial epithelial cells. We report a patient with known Fabry's disease in whom these inclusion bodies were identified by Examination of induced Sputum.

    Abstract:

    The application of Sputum induction and refined methods of Sputum Examination has provided the opportunity to examine cell and molecular markers of airway inflammation in asthma, COPD, and other airway diseases. The measurements are relatively noninvasive and can be applied safely, with care, even in more severe exacerbations of asthma and severe COPD. Induced Sputum Examination can be applied at random and repeatedly and gives results that are reproducible, valid, and responsive to changes in treatment. An eosinophilic bronchitis, defined as Sputum eosinophilia, is typical of asthma but can also occur in patients with a chronic cough without asthma, and in some patients with COPD in whom the classic inflammatory response is neutrophilic without eosinophilia. When eosinophilia occurs in COPD, it has been considered to be the result of cigarette smoking but it may be due to other causes. The clinical importance of eosinophilic bronchitis is that it responds to treatment with corticosteroid. In contrast, there is increasing evidence that an absence of Sputum eosinophilia is associated with steroid resistance. Hargreave FE, Leigh R.     Abstract:

    In the case reported, serial evaluation of Sputum inflammatory cell counts made it possible to identify an unusual series of events in a man with eosinophilic bronchitis. The patient initially presented with a productive cough, which did not respond to treatment with antibiotics or high-dose inhaled corticosteroids. A diagnosis of eosinophilic bronchitis was made after demonstration of intense Sputum eosinophilia. When inhaled corticosteroids were stopped, symptoms and Sputum eosinophilia became worse and airway hyperresponsiveness developed. Both abnormalities were reversed by a course of prednisone. When the prednisone was stopped the productive cough recurred but on this occasion Sputum Examination suggested a different disease process and the symptoms resolved after a course of co-trimoxazole. The patient has subsequently remained well on no treatment with little or no Sputum eosinophilia.

    Nous avons investigue le delai entre l'examen des expectorations et la mise en route du traitement chez les patients enregistres comme atteints de tuberculose pulmonaire (PTB) a bacilloscopie negative dans quatre hopitaux du Malawi. Les informations ont ete obtenues chez 701 des 887 patients qui avaient ete enregistres pendant une periode de 12 mois entre 1997 et 1998. Chez 86% des patients, le traitement a demarre dans les 3 semaines qui ont suivi l'examen de crachats. Le debut du traitement se situe apres 3 semaines chez 14% et apres 6 semaines chez 6%. De tels delais sont inacceptables pour diverses raisons. On donnera des recommandations au staff du programme en vue de repeter les examens d'expectoration chez les patients suspects de PTB, porteurs d'un cliche thoracique anormal et dont les frottis d'expectoration negatifs ont ete examines plus de 3 semaines auparavant.

    Abstract Rationale Sputum Examination is a valuable research tool to study airway allergic diseases, but the requirement to process samples within 2 hours of their collection limits its utility. A new protocol assesses the value of freezing the Sputum sample at the time of collection, and Examination at a later time. Methods Upon collection, Sputum to which was added dimethyl sulfoxide (DMSO) was frozen. The reliability of cell counts in frozen samples and in fresh portions from the same Sputum specimens was assessed using Sputum from 41 asthmatics (18 males) with varying levels of disease severity: 19 of the samples were spontaneously produced while 22 samples required induction with hypertonic saline. Results Significant correlations (p Conclusions Examination of frozen Sputum + DMSO yields cell counts consistent with those of fresh Sputum, especially when employing induced Sputum rather than spontaneously expectorated Sputum for analysis.

    Abstract:

    : In recent studies, Sputum smear cell counts were found to be reproducible and usefully applied to research in asthma and other airway conditions. However, cell definition on the smears is poor, and the procedure is tedious and has limited utility. The objective of this study is to improve the methods of Sputum Examination. The subjects used in this study were people with bronchitis or asthma from whom Sputum could be obtained. By inverted microscopy, portions of fresh Sputum were selected to exclude salivary contamination. These portions were exposed to different volumes of dithiothreitol for varied time intervals. We used the resulting cell suspensions to perform total cell counts and prepare cytospins for differential cell counts and immunohistochemical stains for GM-CSF, EG2, TNF alpha and IL-8. Cytospins were compared with smears for differential cell counts on the same Sputum specimens. Excellent cell dispersion and definition in cytospins could be observed. The time required for differential cell counting on cytospins was reduced and cytospin counts were more reproducible than smears. Greater duration of treatment of Sputum with dithiothreitol tended to increase total cell counts and significantly decreased EG2 staining but had no effect on differential cell counts or the cytokine cell components. Therefore the proposed method of Sputum Examination involving cell dispersion and use of cytospins overcomes a number of the disadvantages of the Examination of smears.

    Objective:Explore the thin layer of liquid based cytology technology(LCT)in lung cancer patients with Sputum exfoliated cells inspection application value.Methods:Collected from January 2011to December 2012were Sputum for lung cancer,a total of 480cases.Simultaneously thin layer liquid based cytology(LCT)production and conventional smear(CS),by the same group of senior doctors in lung cancer patients with positive smear was summarized parting.Results:Positive LCT in the diagnosis of lung cancer cases to 102.The positive rate of 21.25%.Positive CS in the diagnosis of lung cancer cases to 72.The positive rate of 15.00%.LCT cytological smear with conventional smear positive rate difference was statistically significant.Conclusion:LCT technology fall off in the Sputum cytology in lung cancer is easy to implement a new technology of quality control,has higher sensitivity in the diagnosis of lung cancer, specific inspection method.Is worth promoting.
