Staphylococcus Lugdunensis native tricuspid valve endocarditis a case report and review of literature

Repeated isolation of coagulase negative staphylococci in blood cultures should warrant a species identification to recognize unusually virulent organisms that demand aggressive treatment, such as Staphylococcus Lugdunensis. Staphylococcus Lugdunensis is known to cause a wide variety of infections, including a predominant left-sided endocarditis. We report a rare case of native tricuspid valve Staphylococcus Lugdunensis endocarditis in a non-intravenous drug user and include a brief literature review.

Antimicrobial Agents and Chemotherapy, 2016Co-Authors: P L Ho, Kinhung Chow, Waiu Lo, Wai Kei LoAbstract:ABSTRACT Of 137 Staphylococcus Lugdunensis isolates collected from two nephrology centers in Hong Kong, 10 (7.3%) and 3 (2.2%) isolates had high-level and low-level mupirocin resistance, respectively. Isolates with high-level resistance contained the plasmid-mediated ileS2 gene, while isolates with low-level resistance contained the mutation V588F within the chromosomal ileS gene. All but one of the ileS2 -positive isolates belong to the predominating clone HKU1. Plasmids carrying the ileS2 gene were mosaic and also cocarry multiple other resistance determinants.

Lugdunensis in 990 clinical specimens (from tissue, pus, or wound swabs). Significantly more S. Lugdunensis isolates were detected on SSL medium (34/990) than on routine medium (7/990) (P = 0.001, McNemar's test).

The clinical course of S Lugdunensis endocarditis is aggressive and the mortality rate is high in contrast to S epidermidis endocarditis. Most reported cases of S Lugdunensis endocarditis have involved mitral or aortic valves. Herein, we present a case with isolated tricuspid endocarditis due to S Lugdunensis .
