Profil Penggunaan Antibiotik dan Peta Kuman Pada Pasien Gangren Diabetes Melitus di sebuah RSUD di Kabupaten Gresik

Penggunaan antibiotik yang tidak tepat dapat menimbulkan berbagai masalah, diantaranya pengobatan akan lebih mahal dan juga risiko terjadinya resistensi bakteri terhadap antibiotik. Penelitian ini bertujuan untuk mengetahui profil penggunaan antibiotik dan profil peta kuman pada pasien gangren diabetes melitus di sebuah RSUD di Kabupaten Gresik serta untuk mengetahui kesesuaian penggunaan antibiotik dengan mengacu pada Permenkes Republik Indonesia No. 2406/Menkes/PER/XII/2011. Data penggunaan antibiotik diperoleh dari catatan Rekam Medis pada periode Januari – November 2017. Data penggunaan antibiotik dihitung dengan menggunakan rumus DDD/100 pasien-hari rawat. Hasil perhitungan DDD/100 pasien-hari rawat menunjukkan hasil sebesar 470,11 DDD/100 pasien-hari rawat. Peta kuman pada pasien gangren, melaporkan adanya bakteri Enterobacter cloacae sebesar 24%, Escherichia coli sebesar 18%, Staphylococcus aureus sebesar 15%, Acinetobacter baumannii sebesar 9%, Pseudomonas aeruginosa sebesar 6%, Citrobacter youngae sebesar 6%, Enterobacter aerogenes sebesar 6%, Proteus vulgaris sebesar 6%, Staphylococcus Schleiferi sebesar 6%, Klebsiella pneumoniae sebesar 3%, dan Proteus mirabilis sebesar 3%. Penggunaan antibiotik seftriakson dan metronidazol pada pasien gangren diabetes melitus di sebuah RSUD di Kabupaten Gresik pada periode Januari – November 2017 telah sesuai dengan pedoman penggunaan antibiotik berdasarkan Permenkes Republik Indonesia No. 2406/Menkes/PER/XII/2011, yaitu antibiotik golongan sefalosphorin generasi III yang lebih aktif terhadap Enterobacteriaceae dan antibiotik golongan nitroimidazol yang dapat mengobati infeksi bakteri basil anerob Gram-Negatif

genomic variability of Staphylococcus aureus and the other coagulase positive Staphylococcus species estimated by macrorestriction analysis using pulsed field gel electrophoresis
International Journal of Systematic and Evolutionary Microbiology, 1996Co-Authors: Roman Pantůček, Jiří Doškař, Friedrich Gotz, Stanislav RosypalAbstract:The genomic DNAs of 95 culture collection and hospital Staphylococcus aureus subsp. aureus strains of various origins, as well as the genomic DNAs of other coagulase-positive Staphylococcus species, were cleaved with restriction endonuclease Sma I and subjected to pulsed-field gel electrophoresis. The levels of similarity of the Sma I restriction patterns of the S. aureus subsp. aureus strains varied from 30 to 100%, which is considered characteristic of this species; thus, these organisms belonged to the same species restriction group. Within this range of similarity values 13 S. aureus intraspecies restriction groups were identified, and each group consisted of strains whose levels of similarity ranged from 65 to 100%. S. aureus subsp. aureus CCM 885T (T = type strain) belonged to the major intraspecies restriction group that comprised 39% of the S. aureus strains which we studied. The strains of the other coagulase-positive staphylococci, including Staphylococcus aureus subsp. anaerobius, Staphylococcus hyicus, Staphylococcus intermedius, Staphylococcus delphini, and Staphylococcus Schleiferi subsp. coagulans, clustered with their type strains in separate restriction groups. S. aureus subsp. aureus exhibited almost no similarity to these species. We found 44-kb Sma I fragments in all of the S. aureus subsp. aureus and S. aureus subsp. anaerobius strains studied, and these fragments are considered characteristic of the species S. aureus. The high level of homology of these fragments was confirmed by the results of DNA hybridization experiments in which we used representatives of individual intraspecies restriction groups. Of the other staphylococci studied, only Staphylococcus epidermidis and one strain of S. hyicus contained these fragments. However, the levels of homology between these fragments and the fragments of S. aureus were found to be very low.

RankinAbstract:Objective—To define clinical differences between coagulase-positive and coagulase-negative Staphylococcus Schleiferi infections in dogs and to identify risk factors for the isolation of oxacillin-resistant S Schleiferi. Design—Retrospective case series. Animals—225 dogs (yielding 225 S Schleiferi isolates). Procedures—Information obtained from affected dogs' medical records included isolate body site source, antimicrobial treatments, and primary disease. For each dog, the S Schleiferi isolate was characterized and antimicrobial susceptibility data were recorded. Risk factors for infection based on coagulase status and for S Schleiferi oxacillin resistance were investigated. Results—Allergic dermatitis was the most common underlying disease (111/225 dogs). Ears (102 [45%]) and skin (95 [42%]) were sources of most of the 225 isolates. Isolate coagulase status was not significantly associated with any patient-level factors. screening for skin carriage of methicillin resistant coagulase positive staphylococci and Staphylococcus Schleiferi in dogs with healthy and inflamed skin

RankinAbstract:Methicillin resistance rates of 41% for Staphylococcus aureus, 16% for S. intermedius, and 40% for S. Schleiferi have recently been reported in canine patients. These were deemed to be reflective of referral and clinician-selection biases, which would imply significantly lower methicillin-resistant staphylococcal carriage rates in less-biased canine populations. In this study, swabs for bacterial culture were collected from five cutaneous sites on each of 50 healthy dogs and 59 dogs with inflammatory skin disease to determine prevalence of carriage and relative frequency of methicillin resistance in coagulase-positive staphylococci and S. Schleiferi ssp. Schleiferi. These were identified morphologically and by Gram's staining, catalase and coagulase testing, and biochemical speciation. Coagulase-positive staphylococci and S. Schleiferi ssp. Schleiferi were isolated from 88% (52 of 59) of affected dogs. Species identified in the culture-positive dogs were: S. aureus in 12%, S. intermedius (92%), S. Schleiferi ssp. Schleiferi (10%), and S. Schleiferi ssp. coagulans (10%) with methicillin resistance rates of 17%, 8%, 20% and 20%, respectively. Coagulase-positive staphylococci were isolated from 74% (37 of 50) of healthy dogs: S. aureus (16%), S. intermedius (92%) and S. Schleiferi ssp. coagulans (5%). Methicillin resistance rates were 0%, 3% and 50%, respectively. Of total methicillin-resistant isolates, 11 of 13 were positive for PBP2a via latex agglutination. Methicillin-resistant S. intermedius and S. Schleiferi ssp. Schleiferi isolates were all positive for the mecA gene via PCR. There was no significant difference in staphylococcal isolation or methicillin resistance between study groups. While present, methicillin-resistant coagulase-positive staphylococci are significantly less common in these less-biased populations than in the clinical isolates previously reported from this institution which provided the impetus for this study.

RankinAbstract:In this study, bacterial cultures were collected from five sites on each of 50 healthy cats and 48 cats with inflammatory skin disease (ISD), to determine prevalence of carriage and relative frequency of methicillin resistance in coagulase-positive staphylococci and Staphylococcus Schleiferi ssp. Schleiferi. Latex agglutination testing for penicillin-binding protein 2a (PBP2a) and pulsed field gel electrophoresis (PFGE) were performed on all methicillin-resistant (MR) isolates. Polymerase chain reaction (PCR) for the mecA gene was performed on MR S. intermedius and S. Schleiferi isolates. Staphylococcal chromosomal cassette (SCCmec) typing was performed on all MR S. aureus isolates. Coagulase-positive staphylococci and S. Schleiferi ssp. Schleiferi were isolated from 24 of 48 cats with ISD: Staphylococcus aureus (14 of 24, 58%), Staphylococcus intermedius (11 of 24, 46%), Staphylococcus Schleiferi ssp. Schleiferi (1 of 24, 4%), and Staphylococcus hyicus (1 of 24, 4%). Prevalence of MR was 7% for S. aureus, 0% for S. intermedius, 100% for S. Schleiferi ssp. Schleiferi, and 0% for S. hyicus. Coagulase-positive staphylococci were isolated from 17 of 50 healthy cats: S. aureus (10 of 17, 59%), S. intermedius (11 of 17, 65%), and S. Schleiferi ssp. coagulans (1 of 17, 6%). Prevalence of MR was 20% for S. aureus, 18% for S. intermedius, and 0% for S. Schleiferi ssp. coagulans. All MR isolates were positive for PBP2a via latex agglutination. Methicillin-resistant S. intermedius and S. Schleiferi ssp. Schleiferi isolates were also positive for the mecA gene via PCR. Methicillin-resistant S. aureus isolates were identified as SCCmec type II. Results of PFGE indicated heterogeneity among isolates. There was no significant difference in staphylococcal isolation or methicillin resistance between study groups. While present, MR coagulase-positive staphylococci are significantly less common in these study populations.

RankinAbstract:Companion animal staphylococcal isolate antibiograms were screened retrospectively to determine the frequency of methicillin-resistant (MR) infection by Staphylococcus aureus, Staphylococcus intermedius, and Staphylococcus Schleiferi. Rates of MR were: S. aureus 35%, S. intermedius 17%, and S. Schleiferi 40%. Frequency of isolation of methicillin-resistant S. aureus (MRSA) from dogs and cats was similar, whereas methicillin-resistant S. intermedius (MRSI) and methicillin-resistant S. Schleiferi (MRSS) were significantly more common in dogs. MRSS was more commonly associated with superficial (skin and ear canal) infections, whereas MRSA was more commonly associated with deep infections. The MR strain resistance pattern to other classes of antibiotics was also investigated. MRSA was resistant to the most classes of antibiotics, followed by MRSI, while MRSS maintained the most favourable susceptibility profile. MR staphylococci may pose a significant risk to animal and public health. Therefore, to avoid selecting for resistant strains in cases of suspected staphylococcal infection, clinicians should consider culture and susceptibility testing early in the course of treatment.
