    Abstract:

    Code inspections have been used on the 5ESS switch project since 1983. Beginning with a training program for all the developers involved in the project, code inspections have improved with each new 5ESS switch generic. Variations on a standard SQC (Statistical Quality Control) technique, the Control chart, have been used to track the metrics indicative of the effectiveness of code inspections. Parameters used in the computation of these metrics include the preparation effort, inspection time, number of inspectors, the size of the inspected unit of code, and the number of errors found at the inspection. It is noted that the exact form that these Control charts have taken has evolved and improved with experience.

    OBJECTIVES: To establish an objective, scientific, evidence-based process for planning Statistical Quality Control (SQC) procedures based on Quality required for a test, precision and bias observed for a measurement procedure, probabilities of error detection and false rejection for different Control rules and numbers of Control measurements, and frequency of QC events (or run size) to minimize patient risk. METHODS: A Sigma-Metric Run Size Nomogram and Power Function Graphs have been used to guide the selection of Control rules, numbers of Control measurements, and frequency of QC events (or patient run size). RESULTS: A tabular summary is provided by a Sigma-Metric Run Size Matrix, with a graphical summary of Westgard Sigma Rules with Run Sizes. CONCLUSION: Medical laboratories can plan evidence-based SQC practices using simple tools that relate the Sigma-Metric of a testing process to the Control rules, number of Control measurements, and run size (or frequency of QC events).

    : Six sigma concepts provide a Quality management system (QMS) with many useful tools for managing Quality in medical laboratories. This Six Sigma QMS is driven by the Quality required for the intended use of a test. The most useful form for this Quality requirement is the allowable total error. Calculation of a sigma-metric provides the best predictor of risk for an analytical examination process, as well as a design parameter for selecting the Statistical Quality Control (SQC) procedure necessary to detect medically important errors. Simple point estimates of sigma at medical decision concentrations are sufficient for laboratory applications.

    Abstract Multivariate Statistical Quality Control principles, including Control charting of calibration parameters and Control samples, are applied to multilevel ion chromatography (IC) calibrations to determine instrument response stability and minimum calibration frequency. For phosphate species quantitated by suppressed-conductivity IC with NaOH gradient elution, ortho- and pyrophosphate exhibit stable responses over the 10-week course of the study, while tripolyphosphate shows a response that decreases with increasing eluent age. Analyses of Control samples are used to compare the effect of different calibration protocols on long-term method precision and accuracy. Cases are found where using a single, averaged calibration curve for extended periods can give either better or worse precision than a method which employs daily instrument calibration. Optimum calibration frequencies determined for phosphate analyses were weekly for tripolyphosphate, and ≥10 weeks for ortho- and pyrophosphate. Methods for detecting when a calibration has shown a Statistically significant change are discussed. Statistical calculations are presented in a simple algebraic form amenable to the use of spreadsheets for data analysis.

    A research on the application of Statistical Quality Control technique was conducted in a furniture manufacturing company. This company produces a variety of wooden furniture, such as tables and chairs, to cater local and overseas customers. Based on initial observation, it was found that the product with the highest percentage of defects is the folding chairs. Statistical Quality Control technique was applied to ensure that the products meet the standard specified. This research identified five major defects based on the pareto rules. Those defects are pin holes (26.03%), heart wood (22.5%), breakage (17.57%), cracks (17.57%) and discoloration (8.7%). The DPMO for the folding chair production is 5,135.77 and the daily unreworkable cost for one of the components of the chair is IDR 1,961,630.
