Gastrointestinal Reconstruction by Continuous Jejunal Interposition after Total Gastrectomy
Bulletin of Chinese Cancer, 2004Co-Authors: Qian Zhi-xiangAbstract:To study the value of digestive tract reconstruction by continuous jejunal interposition after total gastrectomy.Twenty-six cases with Stomach cancer undergoing digestive tract reconstruction by continuous jejunal interposition after total gastrectomy between Jan.2000 to Mar.2002 were analyzed.No operative death and Stomach Fistula was found in all cases.The follow-up period ranged from 12 to 25 months (average 17.4 months).Eleven cases in the series were examined by barium meal.The volume of artificial Stomach was more than 300ml and the emptying time was 30 to 120 minutes.Reflux esophagitis was found in 6 cases at 6 months after operation and 2 cases at 12 months after operation. The quality of life improved postoperation.[Conclusions]Continuous jejunal interposition is a perfect method for digestive tract reconstruction after total gastrectomy.

China Oncology, 2011Co-Authors: Zhan Li-yongAbstract:Background and purpose:Symptoms would always occurred in patients underwent total gastrectomy,like low weight,reflux esophagitis and serious dumping syndrome and so on,which seriously affect the quality of life of the patients.It is the common purpose to explore an ideal way of digestive tract reconstruction to relieve the digestion disorders symptom of the patients after the surgery and to improve their quality of life.This research aimed to discuss the application values of digestive tract reconstruction by continuous jejunal interposition after total gastrectomy through inspecting the weight,blood index and reconstructed Stomach capacity.Methods:Forty-two cases with Stomach cancer underwent digeslive tract reconstruction by continuous jejunal interposition after total gastrectomy between Jan.2000 to Mar.2010,and all the characteristics of the patients were analyzed.Results:No operative death and Stomach Fistula was found in all cases.The follow-up period ranged from 12 to 24 months(average 17.4 months).The number of hemoglobin and lymphocyte of the patients has increased 6 and 12 months after postoperative.Eleven cases in the series were examined by barium meal.The volume of artificial Stomach was more than 300 mL and the emptying time was 30 to l20 minutes.Reflux esophagitis was found in 11 cases at 6 months and 4 cases at 12 months after operation.The quality of life was improved postoperation.Conclusion:Digestive tract reconstruction by continuous jejunal interposition after total gastrectomy is an effective method to improve the quality of life,to reduce complications after gastrectomy and to maintain the canal of the chyme through the duodenal tract.

Diagnosis and treatment of esophageal carcinoma in old patients postoperative respiratory dysfunction

Methods: Retrospecitive analysis was performed on clinical data of 102 old patients with respiratory dysfunction after esophageal carcinoma resection . Results: Among 102 old patients with postoperative respiratory dysfunction, 34cases(33.3%) developed respiratory failure and 12 cases dead . whten respiratory dystunction took place,as ranged from when the tracheal tube could not be taken away smoothly postanesthesisa to 10 days after operation. Respiratory failure took place 6 to 12 hours after respiratory dysfunction mostly and the period of ventilator using ranged from 2 days to 27 days.Conclusion: The main factors causing postoperative respiratory dysfunction in old patients with esophageal carcinoma are as follows: poor preoperative cardiac and lung function, the primary diseases of lungs, electrolyte solution overperfusion or much more transfusion, operative trauma, postoperative pain of incisions, lung infection, inflation of thoracic Stomach, Fistula of anastomotic stoma and Fistula of cardia of Stomach etc.

The effect of intravenous administrations of a plasma substitute solution made from whey on gastric secretory function

Secretion of acid, a decrease in pH of the juice, an increase in secretion of protein and intensification of the digestive ability of the juice were observed during infusion of the preparation and in the nearest period after it. Infusion of the milk serum preparation with succinate was not followed by activation of the Stomach secretion.

A study on formation of N-(nitrosomethyl) urea in experimental pig Stomach gavaged with fish sauce
Zhonghua yu fang yi xue za zhi [Chinese journal of preventive medicine], 1999Co-Authors: H Xin, Dajun Deng, Ru-ming WangAbstract:OBJECTIVE To study formation of N-(nitrosomethyl) urea (NMU), which is categorized as N-nitrosamides -- a sort of strong chemical carcinogen, by perfusion with fish sauce via pig Stomach Fistula, which is used daily as cooking flavor by local residents in Changle County, Fujian Province, a highly-prevalent area for gastric cancer. METHODS Fistulization was performed in fasting experimental pigs' Stomach, and their gastric juice was suctioned 30 minutes after perfusion with fish sauce and sodium nitrite via Fistula. Gastric juice specimen was purified and concentrated and determined for NMU with high performance liquid chromatography-photolysis pyrolysis-thermal energy analyzer. RESULTS NMU was detected in the pig Stomach at pH 1 - 2, where NMU formed. Formation of NMU depended on the amount of nitrite added in a dose-dependent relationship. Level of NMU in gastric juice reached 25.4 and 7.97 micromol/L, respectively, when 3.48 and 0.87 mmol of sodium nitrite were fed into the Stomach. No NMU could be detected, as 0.22 mmol of sodium nitrite fed. CONCLUSION Under the condition of pH 1 - 2 and presence of sodium nitrite, NMU was synthesized in the Stomachs of experimental pigs fed with fish sauce via gastric Fistula. It is postulated that N-nitrosamides, such as NMU could be formed endogenously in the Stomachs of local residents who consume fish sauce often and have a higher exposure to nitrite in their Stomachs.
