    One of the main challenges faced by today's developers is keeping up with the staggering amount of source code that needs to be read and understood. In order to help developers with this problem and reduce the costs associated with it, one solution is to use simple textual descriptions of source code entities that developers can grasp easily, while capturing the code semantics precisely. We propose an approach to automatically determine such descriptions, based on automated text summarization technology.

    Focuses on investigating the combined use of semantic and structural information of Programs to support the comprehension tasks involved in the maintenance and reengineering of software systems. "Semantic information" refers to the domain-specific issues (both the problem and the development domains) of a software system. The other dimension, structural information, refers to issues such as the actual syntactic structure of the Program, along with the control and data flow that it represents. An advanced information retrieval method, latent semantic indexing, is used to define a semantic similarity measure between software components. Components within a software system are then clustered together using this similarity measure. Simple structural information (i.e. the file organization) of the software system is then used to assess the semantic cohesion of the clusters and files with respect to each other. The measures are formally defined for general application. A set of experiments is presented which demonstrates how these measures can assist in the understanding of a nontrivial software system, namely a version of NCSA Mosaic.

    Twenty-two male patients, addicted to heroin 9.5 years (median), were stabilized using oral methadone hydrochloride and then observed for approximately 1 to 15 months (median, 3 months). The medication had 2 main effects: (1) relief of narcotic hunger (craving); and (2) induction of sufficient tolerance to block the average illegal dose of heroin.A combination of the methadone treatment and a comprehensive Program of rehabilitation was associated with marked improvement in patient problems such as jobs, returning to school, and family reconciliation. No adverse effect other than constipation was found.The authors note that “careful medical supervision and many social services” were necessary and stressed that “both the medication and Supporting Program were essential.” The small size of the group studied and short duration of the follow-up would best describe this as a promising and exciting but preliminary report.See PDF for full text of the original JAMA article.

    Purpose: The purpose of this study was to describe new technology assisted by 3-dimensional (3D) image fusion of F-18-fluorodeoxyglucose (FDG)-positron emission tomography (PET)/computed tomography (CT) and contrast-enhanced CT (CECT) for computer planning of a maxillectomy of recurrent maxillary squamous cell carcinoma and defect reconstruction. Materials and Methods: Treatment of recurrent maxillary squamous cell carcinoma usually includes tumor resection and free flap reconstruction. FDG-PET/CT provided images of regions of abnormal glucose uptake and thus showed metabolic tumor volume to guide tumor resection. CECT data were used to create 3D reconstructed images of vessels to show the vascular diameters and locations, so that the most suitable vein and artery could be selected during anastomosis of the free flap. The data from pre-operative maxillofacial CECT scans and FDG-PET/CT imaging were imported into the navigation system (iPlan 3.0; Brainlab, Feldkirchen, Germany). Three-dimensional image fusion between FDG-PET/CT and CECT was accomplished using Brainlab software according to the position of the 2 skulls simulated in the CECT image and PET/CT image, respectively. After verification of the image fusion accuracy, the 3D reconstruction images of the metabolic tumor, vessels, and other critical structures could be visualized within the same coordinate system. These sagittal, coronal, axial, and 3D reconstruction images were used to determine the virtual osteotomy sites and reconstruction plan, which was provided to the surgeon and used for surgical navigation. Results: The average shift of the 3D image fusion between FDG-PET/CT and CECT was less than 1 mm. This technique, by clearly showing the metabolic tumor volume and the most suitable vessels for anastomosis, facilitated resection and reconstruction of recurrent maxillary squamous cell carcinoma. Conclusions: We used 3D image fusion of FDG-PET/CT and CECT to successfully accomplish resection and reconstruction of recurrent maxillary squamous cell carcinoma. This method has the potential to improve the clinical outcomes of these challenging procedures.   • Circulating adiponectin levels in various malignancies: an updated meta-analysis of 107 studies

    Abstract:

    Early detection of cancers is challenging for lack of specific biomarkers. Adiponectin is an adipokine predominantly derived from adipocytes and hypoadiponectinemia has been reported to associate with risk of many types of cancers. However, available evidence is controversial. Some studies show that increased adiponectin levels correlate with cancer risk. Therefore, we performed a meta-analysis of the association between circulating adiponectin levels and cancer development. A systematic search of PubMed, EMBASE, Wiley Online Library and Cochrane Library was conducted for eligible studies involving circulating adiponectin and malignancies from inception to August 8, 2015. Standard mean differences (SMDs) with 95% confidence intervals (95% CIs) were calculated by use of a random-effect model. Funnel plot and Egger's linear regression test were conducted to examine the risk of publication bias. 107 studies were included with 19,319 cases and 25,675 controls. The pooled analysis indicated that circulating adiponectin levels were lower in patients with various cancers than in controls, with a pooled SMD of -0.334 mu g/ml (95% CI, -0.465 to -0.203, P = 0.000). No evidence of publication bias was observed. Circulating high molecular weight adiponectin levels were also lower in cancer patients than in controls, with a pooled SMD of -0.502 mu g/ml (95% CI, -0.957 to -0.047, P = 0.000). This meta-analysis provides further evidence that decreased adiponectin levels is associated with risk of various cancers.   • Comparison of Local Recurrence Rate of Three Treatment Modalities for Kimura Disease

    Eosinophilic hyperplastic lymphogranuloma, also known as Kimura disease, is a benign and chronic inflammatory condition, predominantly involving the head and neck region. Surgical excision, radiotherapy (RA), surgical resection combined with low-dose postoperative radiotherapy and oral corticosteroids are 4 treatment modalities reported to control this disease effectively. Local recurrence, however, is common and the optimum treatment for Kimura disease is controversial. Thus, the present meta-analysis was performed to identify the treatment modality associated with the lowest local recurrence. Electronic databases (Cochrane Library, Wiley Online Library, PubMed, Chinese National Knowledge Infrastructure, and Wanfang Data) were searched. Data were also obtained from other sources such as related references and communication with the relevant authors. Two reviewers screened the literature according to preselected criteria. All studies involving different treatments for Kimura disease were collected. After data extraction and research quality assessment, the meta-analysis of 22 studies involving 570 patients was conducted using STATA 12.1 software. Meta-analysis revealed that administration of RA or surgical excision alone were inferior in controlling local recurrence compared with surgical resection combined with postoperative RA (risk ratio (RR)=2.72; 95% confidence interval (CI), 1.47-5.04 and RR=4.72; 95% CI, 2.53-8.82). Surgical excision alone did not show significant advantage in controlling local recurrence compared with RA alone (RR=2.13; 95% CI, 0.88-5.17). Surgical resection combined with postoperative RA is superior to either surgery or RA alone in treating Kimura disease.   • A Revised Approach for Mandibular Reconstruction With the Vascularized Iliac Crest Flap Using Virtual Surgical Planning and Surgical Navigation

    Purpose: The purpose of this study was to describe a revised approach for mandibular reconstruction with vascularized iliac crest flap using virtual surgical planning and surgical navigation. Patients and Methods: Preoperative maxillofacial and iliac non-contrast-enhanced computed tomography (CT) scans were acquired, and CT data were imported into ProPlan CMF software (Materialise, Leuven, Belgium). We performed virtual mandibulectomy and superimposed the 3-dimensional iliac image on the mandibular defect. The surgeon shaped the iliac flap according to virtual parameters and the stereomodel. Surgical navigation was used to check and correct the shaped segments. The position of the osteotomy lines and relevant parameters regarding the shape of the iliac flap also were provided to the surgeon. After computer simulation, a reconstructed mandibular stereomodel was manufactured. A reconstruction plate was prebent and fixed on this model using titanium screws. The model was scanned, data were imported into ProPlan CMF, the mandible was segmented, and data were imported into the intraoperative navigation system. Then, the model was registered with the original CT data, and the reconstruction plate was eliminated. Navigation data were exported into a universal serial bus drive, which was connected to the terminal working station during surgery. Intraoperative navigation was used to implement the virtual plan for patients. The sagittal, coronal, axial, and 3-dimensional reconstruction images displayed by the navigation system were used to accurately determine the osteotomy sites and osteotomy trajectory during surgery. Surgical probe guidance was used to mark the osteotomy line and transfer the virtual procedure to real-time surgery. Results: Using our method, we precisely recovered the original configuration of the mandible. The shift in the reconstructed mandible and plate was less than 5 mm. Conclusions: We provided a new method for mandibular reconstruction with vascularized iliac crest flap and an individual reconstruction plate using computer-assisted techniques involving surgical navigation, which have the potential to improve the clinical outcomes of this procedure.
