    Cultural consensus analysis is a statistical method that can be used to assess Participant responses to Survey questions. The technique concurrently estimates the knowledge of each Survey Participant and estimates the culturally correct answer to each question asked, based on the existence of consensus among Survey Participants. The main objectives of this paper are to present the cultural consensus methodology and apply it to a set of median design and safety Survey data that were collected using the Delphi method. A total of 21 Delphi Survey Participants were asked to answer research questions related to cross-median crashes. It was found that the Delphi panel had agreeable opinions with respect to the association of average daily traffic (ADT) and heavy vehicle percentage combination on the risk of cross-median crashes; relative importance of additional factors, other than ADT, median width, and crash history that may contribute to cross-median crashes; and, the relative importance of geometric factors that may be associated with the likelihood of cross-median crashes. Therefore, the findings from the cultural consensus analysis indicate that the expert panel selected to participate in the Delphi Survey shared a common knowledge pool relative to the association between median design and safety. There were, however, diverse opinions regarding median barrier type and its preferred placement location. The panel showed a higher level of knowledge on the relative importance regarding the association of geometric factors on cross-median crashes likelihood than on other issues considered. The results of the cultural consensus analysis of the present median design and safety Survey data could be used to design a focused field study of median safety.

    ABSTRACTBackground: Despite a routine vaccination for preventing HAV and HBV in young age groups during the past decade, adults at high risk of hepatitis are not reached effectively by vaccination programs. This study sought to estimate the prevalence of adults at higher risk of vaccinepreventable hepatitis (hepatitis A virus (HAV) and hepatitis B virus (HBV)) in the US and their vaccination rates. Also, the association between vaccination rates and selected demographic, socioeconomic characteristics and comorbidities was investigated.Methods: Four years, 1999–2002, of publicly available National Health and Nutrition Examination Survey (NHANES) data were utilized. Survey Participants aged 20–59 years were selected.   • Identifying a High-Risk Cohort in a Complex and Dynamic Risk Environment: Out-of-bounds Skiing—An Example from Avalanche Safety

    The development of effective prevention initiatives requires a detailed understanding of the characteristics and needs of the target audience. To properly identify at-risk individuals, it is crucial to clearly delineate risky from acceptable behavior. Whereas health behavior campaigns commonly use single conditions (e.g., lack of condom use) to identify high-risk cohorts, many risk behaviors are more complex and context dependent, rendering a single condition approach inadequate. Out-of-bounds skiing, an activity associated with voluntary exposure to avalanche hazard, is an example of such a multifaceted risk-taking activity. Using a dataset from an extensive online Survey on out-of-bounds skiing, we present an innovative approach for identifying at-risk individuals in complex risk environments. Based on a risk management framework, we first examine risk-taking preferences of out-of-bounds skiers with respect to exposure and preparedness—the two main dimensions of risk management—separately. Our approach builds on existing person-centered research and uses Latent Class Analysis to assign Survey Participants to mutually exclusive behavioral classes on these two dimensions. Discrete Choice Experiments are introduced as a useful method for examining exposure preferences in the context of variable external conditions. The two class designations are then combined using a risk matrix to assign overall risk levels to each Survey Participant. The present approach complements existing person-centered prevention research on the antecedents of risk-taking by offering a process-oriented method for examining behavioral patterns with respect to the activity itself. Together, the two approaches can offer a much richer perspective for informing the design of effective prevention initiatives.
