    Importance: Electronic prescribing promises to improve the safety and clarity of prescriptions. However, it also can introduce miscommunication between prescribers and pharmacists. There are situations where information that is meant to be sent to pharmacists is not sent to them, which has the potential for dangerous errors. Objective: To examine how frequently prescribers or administrative personnel put information intended for pharmacists in a Field not sent to pharmacists, classify the type of information included, and assess the potential harm associated with these missed messages. Design, Setting, Participants: Medication record data from our legacy electronic health record were requested for ambulatory care patients seen at an academic medical center from January 1, 2000, to May 31, 2015 (20 123 881 records). From this database, 6 060 272 medication orders met our inclusion criteria. We analyzed a random sample of 10 000 medication orders with internal comments. Main Outcomes and Measures: Reviewers classified internal comments for intent. Comments intended for pharmacists were also sorted into descriptive categories and analyzed for the potential for patient harm. Results: We found that 11.7% of the prescriptions in our sample contained comments that were intended to be sent to pharmacists. Many comments contained information about the dose, route, or duration of the prescription (38.0%). Approximately a third of the comments intended for pharmacists contained information that had the potential for significant or severe harm if not communicated. Conclusion: We found undelivered comments that were clearly intended for pharmacists and contained important information for either pharmacists or patients. This poses a legitimate safety concern, as a portion of comments contained information that could have prevented severe or significant harm.

    Among others, consumer products can be purchased in the Internet and in traditional stores. Each of the two has dedicated advantages. An online survey conducted within the frames of this work investigates these advantages. It motivates the transition of the advantages of online shopping, such as access to recommendations of other consumers, to the sales floor. Recent trends in mobile phone technology, for example the emergence of the mobile Internet, enable exactly this transition, potentially enriching the shopping experience in the real world. A key challenge though is a fast and convenient identification of products. This work compares five product identification modalities for mobile phones in a comparative study. The dependent variables evaluated are `task completion time' and `perceived ease of use'. Our study is the first that quantifies the advantage of automatic identification. The results indicate that automatically identifying a product scanning a tag can be up to eight times faster than entering a product name in a Text Field. Surprisingly, barcode recognition using a camera phone can be conducted almost as fast and convenient as scanning an RFID tag. Our work provides a benchmark for developers having to choose appropriate identification technology for their mobile application.

    Injury surveillance data provides the evidence base to inform injury prevention programme and policy development. The Queensland Injury Surveillance Unit (QISU) collects data on emergency department (ED) injury presentations at Queensland hospitals. When a patient presents for treatment of an injury, the triage nurse activates an injury surveillance screen and assigns codes for intent, place, part of place, activity, subtype of activity, major injury factor, mechanism and external cause of injury. In busy ED situations this can be time consuming, 3–5 min/record. QISU investigated the possibility of reducing the number of coded Fields and having QISU coders assign the remaining codes based on the triage presenting problem Text Field. 500 records were randomly sampled from the database. Two experienced QISU coders were blinded to the codes assigned at triage and recoded the records using only the presenting problem Text descriptor. The quality of the information contained within the Text descriptor was assessed by recording what variables were able to be coded from the Text descriptor. Concordance between data coded by triage nurses and QISU coders was compared. There was variability in the concordance of codes and the quality of the Text descriptor between facilities, injury type and patient groups. This information will be used to streamline data collection by reducing the number of variables coded by the triage nurse and demonstrates the utility of Textual data for injury surveillance purposes.

    Abstract Objective To summarise the extent to which narrative Text Fields in administrative health data are used to gather information about the event resulting in presentation to a health care provider for treatment of an injury, and to highlight best practise approaches to conducting narrative Text interrogation for injury surveillance purposes. Design Systematic review. Data sources Electronic databases searched included CINAHL, Google Scholar, Medline, Proquest, PubMed and PubMed Central. Snowballing strategies were employed by searching the bibliographies of retrieved references to identify relevant associated articles. Selection criteria Papers were selected if the study used a health-related database and if the study objectives were to a) use Text Field to identify injury cases or use Text Fields to extract additional information on injury circumstances not available from coded data or b) use Text Fields to assess accuracy of coded data Fields for injury-related cases or c) describe methods/approaches for extracting injury information from Text Fields. Methods The papers identified through the search were independently screened by two authors for inclusion, resulting in 41 papers selected for review. Due to heterogeneity between studies meta-analysis was not performed. Results The majority of papers reviewed focused on describing injury epidemiology trends using coded data and Text Fields to supplement coded data (28 papers), with these studies demonstrating the value of Text data for providing more specific information beyond what had been coded to enable case selection or provide circumstantial information. Caveats were expressed in terms of the consistency and completeness of recording of Text information resulting in underestimates when using these data. Four coding validation papers were reviewed with these studies showing the utility of Text data for validating and checking the accuracy of coded data. Seven studies (9 papers) described methods for interrogating injury Text Fields for systematic extraction of information, with a combination of manual and semi-automated methods used to refine and develop algorithms for extraction and classification of coded data from Text. Quality assurance approaches to assessing the robustness of the methods for extracting Text data was only discussed in 8 of the epidemiology papers, and 1 of the coding validation papers. All of the Text interrogation methodology papers described systematic approaches to ensuring the quality of the approach. Conclusions Manual review and coding approaches, Text search methods, and statistical tools have been utilised to extract data from narrative Text and translate it into useable, detailed injury event information. These techniques can and have been applied to administrative datasets to identify specific injury types and add value to previously coded injury datasets. Only a few studies thoroughly described the methods which were used for Text mining and less than half of the studies which were reviewed used/described quality assurance methods for ensuring the robustness of the approach. New techniques utilising semi-automated computerised approaches and Bayesian/clustering statistical methods offer the potential to further develop and standardise the analysis of narrative Text for injury surveillance.
