    Abstract A reliable test that facilitates the accurate diagnosis of Parkinson's and disorders will help with both, clinical management and Therapeutic Research. In this context, neurofilament light chain (NFL) is candidate for a biomarker in cerebrospinal fluid (CSF). A comprehensive literature search yielded 4 eligible studies. We expressed between-group difference of NFL concentration in CSF as the standardized mean difference. Four studies involved 166 Parkinson's disease (PD), 116 multiple system atrophy (MSA) and 73 progressive supranuclear palsy (PSP) patients. Patients with MSA showed higher concentration of NFL concentration in CSF than those with PD (standardized mean difference=1.60, P P =0.17). NFL in CSF in PSP was significantly elevated relative to PD with homogeneous studies (standardized mean difference=2.04, P P =0.99). The present meta-analysis suggested that NFL concentration in CSF in MSA and PSP was significantly increased relative to PD, and that this could help us to separate PD from atypical parkinsonian syndromes.

    Abstract:

    In medical historiography, clinical-Therapeutic Research is usually presented as an Anglo-American success story that only began after the Second World War. However, Paul Martini, who headed clinics in Berlin and Bonn, had already outlined the methods and virtues of an experimentally controlled Research of clinical efficacy in 1932. This article seeks to fathom the resonances and implementation initiatives that Martini’s methodological reform program experienced during the Nazi era. The article focuses on a Research project called Comparative Therapy ( Vergleichende Therapie – V. T.), conceived by Martini and developed at the beginning of the war in cooperation with the Breslau (Wroclaw) internist Kurt Gutzeit. This large-scale project, which was carried out between 1940 and 1944 within the framework of military medical institutions, was intended to identify effective and efficient therapies. Under wartime conditions, the project increasingly encountered difficulties that could not be overcome. Moreover, the ethical implications of the conflict between Research and treatment became apparent. In the end, V. T. achieved the opposite of what had been intended: in the post-war period, the failed project added to the scepticism about schematic clinical Research controlled by statistical methods, which could only be established in Germany with delay.

    Because of repeated failures of clinical trials, the concept of Alzheimer's disease (AD) has been changing rapidly in recent years. As suggested by the National Institute on Aging and the Alzheimer's Association Research Framework, the diagnosis and classification of AD is now based on biomarkers rather than on symptoms, allowing more accurate identification of proper candidates for clinical trials by pathogenesis and disease stage. Recent development in neuroimaging has provided a way to reveal the complex dynamics of amyloid and tau in the brain in vivo, and studies of blood biomarkers are taking another leap forward in diagnosis and treatment of AD. In the field of basic and translational Research, the development of animal models and a deeper understanding of the role of neuroinflammation are taking a step closer to clarifying the pathogenesis of AD. Development of big data and the Internet of Things is also incorporating dementia care and Research into other aspects. Large-scale genetic Research has identified genetic abnormalities that can provide a foundation for precision medicine along with the aforementioned digital technologies. Through the first international conference of the Korean Dementia Association, experts from all over the world gathered to exchange opinions with association members on these topics. The Academic Committee of the Korean Dementia Association briefly summarizes the contents of the lectures to convey the depth of the conference and discussions. This will be an important milestone in understanding the latest trends in AD's pathogenesis, diagnostic and Therapeutic Research and in establishing a future direction.

    Abstract:

    Although zinc is an essential trace element involved in many physiological functions, toxicological data concerning acute exposure are scarce. The aim of our study was to determine the maximal iterative dose of zinc that can be administrated in rats without any adverse effect. Saline (control group) or zinc gluconate at 1, 2 or 4 mg/kg were intraperitoneally injected in animals daily during 7 days. The tolerance of zinc treatments was evaluated by the observation of clinical symptoms, haematological parameters and biochemistry, in relation to the zinc and copper levels in blood, liver, pancreas and faeces. We found no serious adverse effect within 1 week in rats injected intraperitoneally with 1 or 2 mg/kg/day of zinc gluconate, which tends to indicate that those doses could be useful in future Therapeutic Research. In contrast, the Therapeutic treatment of adult rats with repeated intraperitoneal injections of a 4 mg/kg/day zinc dose should be cancelled, due to the occurrence of clinical adverse effects within a few days, as intraperitoneal local intolerance or major growth underdevelopment.
