primary Papillary Carcinoma of a thyroglossal duct cyst report of a case and literature review

They are twice as frequent as branchial cleft abnormalities and, in children, are second only to enlarged cervical lymph nodes as the cause of neck mass. Generally, duct cysts are benign, but 1 per cent of cases may be malignant. From the world literature, 114 cases of malignant thyroglossal cysts were available for review. With the addition of our own case, we discuss 115 instances of duct cysts. The different types of neoplasia described included Thyroid Papillary Carcinoma in 81·7 per cent, mixed Papillary-follicular Carcinoma in 6·9 per cent, squamous cell Carcinoma in 5·2 per cent, follicular and adenoCarcinoma in 1·7 per cent each, and malignant struma, epidermoid Carcinoma and anaplastic Carcinoma in 0·9 per cent each. Of the 115 cases surveyed, 35 Thyroid glands were examined microscopically; of these, four (11·4 per cent) contained malignant foci. Whether these are primary malignancies of the thyroglossal duct cysts or metastases is discussed.

We immunohistochemically examined 90 Thyroid Papillary Carcinomas (85 invasive type and five encapsulated type), using a rat IgG2a monoclonal antibody against the carboxyl terminal region of human MK in archival paraffin sections. The Thyroid tumors exhibited an intense reaction in the cytoplasm. Most of the Papillary Carcinomas (77/90), had tumor cells that expressed MK. These were classified into the following two types: invasive type (76/85) and encapsulated type (1/5). Notably, the intensity of MK was stronger at the invading border area of the tumors than in the center. In tissues adjacent to the cancer tissues, normal follicular epithelial cells expressed MK very faintly or not at all. The in situ hybridization analysis revealed that the signals of MK transcripts were found in the cytoplasm of the cancer cells. In the noncancerous follicular epithelial cells adjacent to neoplasm the signals of MK transcripts were detected very weakly or not at all. The distribution and localization of the MK-transcript signals determined by in situ hybridization analysis were similar to those obtained by immunohistochemical analysis. We conclude that Thyroid Papillary Carcinoma strongly expresses MK protein and messenger RNA, and that this overexpression may relate to the development and invasion of these Carcinomas.

Integrating Five Feature Types Extracted From Ultrasonograms to Improve the Prediction of Thyroid Papillary Carcinoma

And, the Thyroid Papillary Carcinoma (TPC) was usually diagnosed during the regular examination of the Thyroid gland. The current diagnosis guideline heavily replies on the experienced clinical endoscopists. This paper comprehensively evaluated four classification algorithms and five image feature extraction algorithms for the TPC diagnosis problem. Our data demonstrated that the Hessian features extracted from the transverse ultrasonograms performed better than those from the longitudinal view. The best model (Acc = 0.9949) was achieved by the seven-layer shallow neural network with the LBP and Hessian features extracted from both the longitudinal and transverse views of the ultrasonograms.

The Transverse Ultrasonogram of Thyroid Papillary Carcinoma Has a Better Prediction Accuracy Than the Longitudinal One

Thyroid cancer has no symptoms and may be detected by ultrasound imaging incidentally for other medical conditions. An accurate computational detection model may help the precise diagnose of Thyroid Papillary cancer (TPC). A standard protocol captures at least the transverse and longitudinal ultrasonograms of the Thyroid. This study investigated the detection problem of Thyroid cancer using the ultrasound images. Our data suggested that the original local binary pattern (LBP) features extracted from the ultrasonograms were very sparse and the compressed LBP features outperformed the original version. And the best model (Acc = 0.9829) was achieved by the fivefold cross validation of the classifier support vector machine (SVM). Other sources of biomedical data may be integrated to further improve the TPC detection model in future studies.
