Disappearance of biased visual attention in young infants: remediation of Tonic Neck Reflex or maturation of visual asymmetry?
2018Co-Authors: Chris Lange-küttnerAbstract:Typically, infants younger than four months fail to attend to the left side of their spatial field, most likely because of an innate asymmetrical Tonic Neck Reflex (ATNR). In a critical transition, by four months of age, infants begin to reach and develop depth perception; and, by five months of age, they tend to monitor the entire spatial field. However, this developmental transition can be delayed. Moreover, there always is a residual right sided spatial bias under cognitive load, a phenomenon that may occur as well among adult stroke patients. While causative factors of biased visual attention in both infants and brain-injured adults may vary, mechanisms of remediation may be similar. This literature review addresses whether the infant’s emergence of attention toward a full visual spatial field and the associated shift from monocular to binocular vision occurs because of (a) increased left side reaching loosens the rarely mentioned high muscle tension ATNR, or (b) maturational resolution of a visual asymmetry in motion perception. More research is needed to investigate the origins of the infants’ visual control system and factors involved in its development, especially because Alzheimer and dementia patients may also show primitive two-dimensional vision and deficits in perceiving objects-in-motion that seem to mirror earlier infant visual perception.

The effects of asymmetric Tonic Neck Reflex during reaching movement following stroke: Preliminary results

DewaldAbstract:Previous studies and clinical observations reveal that stroke survivors show the resurgence of the asymmetric Tonic Neck Reflex (ATNR) both in static and dynamic conditions during maximal efforts. This observation may imply more reliance on the brainstem pathways following stroke. However, the effect of ATNR during a dynamic condition that represents more natural movement, such as reaching, has not been studied before. During reaching movements, the application of a robot controlled haptic environment is important to quantify the effect of ATNR following stroke. Therefore, this paper reports the use of a novel setup using the ACT3D robotic device to investigate and quantify this Reflexive behavior. Our preliminary results demonstrate that the effect of ATNR is significant in the stroke population when abducting the shoulder at 25% of maximum ability. These results show that the ATNR affects reaching distance especially when shoulder loading in abduction is required. In conclusion, these preliminary results provide evidence that the effect of ATNR in stroke subjects during reaching task can be quantified by using a novel 3-D robotic setup.

Pelvic Symmetry Is Influenced by Asymmetrical Tonic Neck Reflex during Young Children's Gait.

One test increasingly used by physiotherapists and pediatricians is asymmetrical Tonic Neck Reflex (ATNR). Physiologically, it is observed from in utero up to six postnatal months. This reaction is inhibited with the growing maturation of the central nervous system (CNS). In some children, when the natural process of development is incorrect, ATNR manifests later in life, when it is observed as an automatic response of muscle tension to head rotation. Analysis of pelvis symmetry in the gait of children with active ATNR is important for better understanding their specific movements. In the gait of children with persistent ATNR, some variations are observed. The aim of the study was to investigate the gait symmetry of preschool children and the influence of persistent ATNR. Fifty preschool children with a trace form of ATNR were examined. The distribution of the gait parameters was determined using a BTS G-SENSOR measurement instrument. ATNR negatively influences pelvic obliquity and pelvic rotation (p < 0.01). Younger children have a statistically higher symmetry index of pelvis obliquity in the examined group (p = 0.015). Boys obtain a higher result of symmetry in pelvic tilt than girls in the group (p = 0.027). ATNR affects walking symmetry in preschool children, thus evaluation of the Reflex activity and then proper therapy is required to support proper development.

Trunk rotation due to persistence of primitive Reflexes in early school-age children.

Choińska, Małgorzata Paprocka-borowiczAbstract:Background The angle of trunk rotation (ATR) is a measurement that allows an objective assessment of a growing child's spine. Early detection of trunk rotation prevents the progression of scoliosis. One of the factors that predispose children to the formation of faulty posture may be primitive Reflexes, which should be integrated in the central nervous system (CNS) by the age of one year. If retained, primitive Reflexes affect children's physical development as well as their development at school. Objectives The aim of the study was to determine the prevalence of trunk asymmetry and the persistence of primitive Reflexes and their inter-relationships in early school-age children. Material and methods In this population-based survey, 61 children, average age 6.3 years (SD ±1.43), were examined. The ATR was examined using a scoliometer. The degree of integration of Reflexes was assessed using tests developed by S. Goddard to assess the asymmetrical Tonic Neck Reflex (ATNR), symmetrical Tonic Neck Reflex (STNR), and spinal Galant Reflex (SGR) on a 0-4 scale. Spearman's rank correlation coefficient and the χ2 test were used in the statistical analysis. Results In almost half of the children body rotation was observed, in most cases toward the right (p = 0.012). This asymmetry was positively correlated with non-integrated Galant Reflex on the same side (r = 0.335, p = 0.050). The presence of trunk rotation is associated with sex: There was higher frequency of asymmetry among the girls than among the boys. Conclusions In the evaluation of scoliosis, it could be useful to examine primitive Reflexes as a possible reason for trunk rotation. In the treatment of scoliosis, primitive Reflex integration methods should be used in some cases.

Infants with vitamin B12 deficiency-related neurological dysfunction and the effect of maternal nutrition
'ScopeMed Publishing', 2019Co-Authors: El Çiğdem, Çelikkaya, Mehmet EminAbstract:Aim: Studies linked vitamin B12 deficiency in infancy are most commonly in patients with hematological and dermatological complaints rather than infant with neurodevelopmental retardation. In our study we aimed to evaluate the data of infants who presented with various neurological complaints and determined neurodevelopmental retardation due to vitamin B12 deficiency and also effects of maternal diet on the infants. Material and Methods: Infants who were admitted with various neurological complaints due to vitamin B12 deficiency between the dates of September 2016 and March 2018 were included in this study. Patients were retrospectively analyzed according to age, growth and development, clinical and laboratory findings (neurological, hematologic), continuation of breastfeeding, age at onset of complemantary food. During the pregnancy and lactation, the amount of feeding with animal foods, intake of vitamin supplements, duration of breastfeeding and vitamin B12 levels were examined. Results: The most common complaints were fatigue, pallor, no eye contact, apathy, vomiting, constipation, diarrhea, refusal to feed, retardation to growth and development, undeveloped or lost of ability (ability to Tonic Neck Reflex, ability to sit supported and unsupported ability to sit). In the neurological examination of infants; iIn 13 infants, there was hypotonia, undeveloped ability to Tonic Neck Reflex and ability to sit (supported and unsupported). Fifteen, in 15 infants just undeveloped ability to sit (supported and unsupported) and also in 6 infants had swallowing disorders. Conclusion: Vitamin B12 routinely to all pregnant women like folic acid and iron supplementation. In addition, developmental stages of infants should be evaluated and vitamin supplements should be given to infants when necessary
