    Abstract:

    Third molar development and eruption are two related areas of major interest in dental research into the etiology of "wisdom Tooth" Impaction. Third molars are not only an excellent model for studying dental development but also of fundamental clinical importance because they are very frequently impacted. Because the third molar is located in the distal-most region of the oral cavity, clinical access is relatively challenging. With the increasingly widespread use of cone-beam computed tomography (CBCT) in dentistry, studies and measurements of the third molar and its eruption area have become considerably easier to do. Here we present a novel CBCT-based measurement methodology we developed for our recent investigations that we hope will also be useful for the broader dental research community.

    Abstract:

    Recent studies in transgenic mice have demonstrated that PTH-related protein (PTHrP), signaling through the type 1 PTH/PTHrP receptor (PTHR1), regulates endochondral bone development and epithelial-mesenchymal interactions during the formation of the mammary glands and teeth. Recently, it has been shown that loss-of-function mutations in the PTHR1 gene result in a rare, lethal form of dwarfism known as Blomstrand chondrodysplasia. These patients suffer from severe defects in endochondral bone formation, but abnormalities in breast and Tooth development have not been reported. To ascertain whether PTHrP signaling was important to human breast and Tooth development, we studied two fetuses with Blomstrand chondrodysplasia. These fetuses lack nipples and breasts. Developing teeth were present, but they were severely impacted within the surrounding alveolar bone, leading to distortions in their architecture and orientation. Compatible with the involvement of PTHR1 and PTHrP in human breast and Tooth morphogenesis, both were expressed within the developing breasts and teeth of normal human fetuses. Therefore, impairment of the PTHrP/PTHR1 signaling pathway in humans is associated with severe abnormalities in Tooth and breast development. In addition to regulating human bone formation, this signaling pathway is also necessary for the normal development of the human breast and Tooth.

    Tooth Impaction is defined as any Tooth that fails to erupt into a normal functional position and remains unerupted in the jaw beyond the time of its normal eruption. Primary Impaction of primary teeth is rare. Furthermore, a combination of eruption failure of primary second molars and horizontally impacted permanent first molars has not yet been reported. The purpose of this study was to present the treatment of two cases of primary Impaction of primary second molars combined with horizontally impacted permanent first molars. The impacted primary second molars were extracted, and orthodontic traction was performed to induce normal eruption of the permanent first molars. We then regained space for the eruption of the second premolar and applied a space maintainer to secure the space and induce successful eruption of succedaneous teeth and normal function of permanent first molars in their positions.

    Abstract:

    Impacted teeth are teeth with a delayed eruption time or that are expected to erupt incompletely. Those teeth can cause a series of potential problems such as root displacement and resorption, periodontal problems in adjacent teeth, referred pain and the formation of cysts and odontogenic tumors. The purpose of this study was to investigate characteristics and treatment of child and adolescent patients younger than 15 years of age that were diagnosed with an impacted Tooth who visited the Chosun University Dental Hospital. The impacted Tooth, its etiology, treatment and traction period were surveyed through electric medical records, radiographs in 335 patients. We excluded the impacted third molar, supernumerary and deciduous teeth from this study. The most frequently impacted teeth are upper canine, followed by the upper incisor. The most common etiologies of Impaction were an abnormal eruption pathway and localized pathologic lesions. The treatment of an impacted Tooth was mostly orthodontic traction. The traction period was relatively short in cases with distinct obstacles, with an impacted upper incisor and if patients were younger. An orthodontic traction is considered to be more unfavorable if the patient gets older. Therefore, an early diagnosis and a precise treatment plan through a regular check-ups are mandatory.
