The ecology and evolution of Tooth Wear in red deer and moose

StensethAbstract:Differences in body size and diet type (browser-grazer continuum) have formed functional traits of ruminants, including Tooth design. Grazers and mixed-feeders eat a more fibrous diet than browsers, which arguably increase Tooth Wear. Tooth Wear has also been suggested to increase with body size. Moreover, for species with large distribution ranges, different populations may be exposed to very different ecological factors affecting diet and thus Tooth Wear rates. Therefore, evolutionary history and contemporary ecological conditions, both operating through diet, may be important for patterns of Tooth Wear. Here, we compare inter- and intraspecific rates of Tooth Wear in multiple populations of one large browser (moose Alces alces) and one mixed-feeder (red deer Cervus elaphus) covering the main distribution range of each species in Norway. We found that the mixedfeeding red deer wore teeth faster than the larger and browsing moose, suggesting that feeding-type was more important than body size for patterns of Wear. There was substantial spatial variation in Tooth Wear rates, but the inter-specific difference in Wear was consistent. Molar Wear rates, but not incisors Wear rates, in the browser were less variable between populations than in the mixed-feeder. There was no close link between incisor and molar Wear rates at the population level. Our findings are consistent with the view that both evolution related to diet type and current ecological conditions (being proxies for within-species variation in diet quality) are important for patterns of Tooth Wear.

We tested these predictions on data on Tooth Wear (estimated as height of first molar) of 1,311 male and 1,348 female red deer (Cervus elaphus) aged 3–25 years and harvested along the west coast of Norway. The rate of Tooth Wear decreased with age. Males Wear teeth at a higher rate (from 0.61 mm/year in 4-year olds to 0.45 mm/year in 11-year olds) than females (from 0.52 mm/year in 4-year olds to 0.39 mm/year in 11-year olds). Molar height correlated positively with body weight in both sexes, but not after adjusting for body size. Molar height was strongly dependent on body size in 3-year-old individuals (when Tooth Wear is minimal). Earlier reports in the literature of a positive correlation between Tooth height and body weight may therefore be due to initial size differences rather than differences in condition due to Tooth Wear.

Risk Assessment: Tooth Wear
Risk Assessment in Oral Health, 2020Co-Authors: David Bartlett, Saoirse O’tooleAbstract:Tooth Wear is the multifactorial process where acids and mechanical challenges either act alone or together remove enamel and then dentine. The interaction of acids and mechanical processes, attrition and abrasion, has led to increasing adoption of the term “erosive Tooth Wear”. This term acknowledges that most Tooth Wear is multifactorial, often with acids having some role in the progression. Risk assessment of Tooth Wear should focus on the frequency, duration and interplay of the causes. This chapter provides an overview of risk factors and discusses their individual and synergistic role in Tooth Wear progression. Risk characteristics are categorised by severity and existing clinical presentation. The basic erosive Wear examination (BEWE) is advocated as a basic tool to screen for Tooth Wear. Patient health and behaviours can change which may necessitate repeating risk assessments on a regular basis when uncontrolled Wear is diagnosed. Patients need to be informed not only of the risk of progression but also on the risks of restorative treatment. There is evidence that progression can be slowed to physiological levels with elimination of risk factors. Practitioners are reminded that Tooth Wear is a slow process and preventive approaches are encouraged.

Tooth Wear and aging

It is increasing in prevalence in the younger population and a greater number of patients are retaining their teeth into old age. Methods This paper is a narrative review of the clinical presentation, the epidemiology and the restorative intervention for erosive Tooth Wear. The dilemmas in managing this common condition with the aging dentition in mind are described. The paper discusses evidence-based prevention methods and highlights how preventive intervention may be preferable over extensive restorative care and high maintenance needs. Patient wishes, expectations and commitment to treatment and maintenance require consideration during clinical decision making. Conclusion Successful management of erosive Tooth Wear in an ageing population depends on effective diagnosis, preventive intervention and holistic advice regarding restorative intervention.

Prevalence, severity and explanatory factors of Tooth Wear in Arab populations.

Al Habashneh, David BartlettAbstract:Abstract Objectives The aim of this study was to assess the prevalence and severity of Tooth Wear and related risk indicators in six Arab countries. Methods This multicenter, multinational cross-sectional study was conducted among 2924 participants between the ages of 18–35 years old from six Arab countries. Calibrated dentists assessed Tooth Wear using the Basic Erosive Wear Examination Index (BEWE). Participants were also asked to complete a questionnaire that assessed their dietary and oral health habits. Each participant was identified by the sextant with the highest BEWE score in the upper and lower jaws. Results: Odds ratios were estimated for correlates of Tooth Wear anterior and posterior regions. Oman had the highest prevalence of BEWE score 3 (N = 255 (60.2%)). Overall, the highest BEWE score 3 was observed on the incisal edge of the upper anterior teeth (N = 602 (20.6%)) and in the lower left posterior region (sextant 6) on the occlusal surface (N = 466 (15.9%)). Correlates of severe Tooth Wear in both regions were eating or drinking more than six times per day, age and education. Drinking soft drinks “once a day” was significantly associated with severe Tooth Wear in the posterior region (OR: 1.3, 95% CI:1.05, 1.6). Brushing teeth more than twice a day was inversely associated with Tooth Wear in the anterior region (OR:0.75, 95% CI: 0.57,0.97). Conclusion The prevalence of Tooth Wear in Arab populations is relatively high, specific preventive and therapeutic measures should be developed to target people at higher risk of this condition. Clinical Significance Tooth Wear is now regarded as a significant oral health problems, dentists should advise their patients about dietary and oral health habits that can lead to severe Tooth Wear.

Tooth Wear risk assessment and care planning in general dental practice
British Dental Journal, 2018Co-Authors: Saoirse Otoole, David Bartlett, M Khan, A Patel, N J Patel, N Shah, S MovahediAbstract:Objective To assess charting, risk assessment and treatment-planning of Tooth Wear between recently qualified and experienced dentists in general dental practice.Design Service evaluation.Setting Multi-setting evaluation of three mixed NHS/Private general dental practices in North-East London.Methods The clinical notes of new patient examinations on dentate adults presenting from the 1 October 2016 to 31 December 2016 were audited collecting data on Tooth Wear charting, risk assessment and treatment planning. Data were analysed using descriptives, chi square and logistic regressions in SPSS. Significance was inferred at p <0.05.Results Foundation dentists and experienced dentists performed 85 and 200 new patient examinations, respectively, during the evaluation period. Tooth Wear was charted for 48% of those attending foundation dentists and 5% of those attending experienced dentists. Diet was assessed in 50.6% of patients examined by foundation dentists and 1.0% of patients examined by experienced dentists. Foundation dentists were more likely to chart Tooth Wear, risk assess and preventively manage Tooth Wear compared to experienced dentists (p <0.001).Conclusion This service evaluation highlights that improvements are required in recording, risk assessing and preventive treatment planning of erosive Tooth Wear. Experienced dentists were less likely to risk assess Tooth Wear and less likely to provide preventive treatment. Experienced GDPs may benefit from re-training in this area.
