Severe Trachea Compression caused by Riedel's thyroiditis: A case report and review of the literature

Case presentation: We present a case of RT in a 73-year-old female with dyspnoea caused by severe Trachea Compression. She had reported dyspnoea during physical stress, and had noticed a large mass on the front of the neck. Despite the combination of various imaging modalities, the thyroid mass was not differentiated from thyroid malignancy and other thyroid disorder. Total thyroidectomy and tracheotomy were performed. During surgery, the thyroid had severe adhesion to surrounding tissue and the pathology revealed RT. Conclusions: Clinicians should be aware of RT. It is important to differentiate this condition from other thyroid disorders, especially malignant lesions. Thyroidectomy is indicated for patients with compressive symptoms, suspicious malignancy and failure of conservative management

The cartilaginous rings of the Trachea may be weakened or destroyed by long-standing Compression, causing loss of structural support. Tracheomalacia after resectioning of benign goiter compressing Trachea was an important issue. It has extended the time of mechanical ventilation, increased the risk of failure extubating and mortality. Materials and Methods: Descriptive and prospective study. From November 2014 to January 2017, we have collected 102 patients who had benign goiter compressing Trachea and had thyroidectomy. Tracheomalacia was diagnosed and managed intra and postoperatively. Results: Recognition of women accounted for the majority of 81.4%. The average age was 54.5 years old and the average duration of goiter was 5 years. The patients were hospitalized with the main symptom of dysphagia accounting for 21.6% and difficulty breathing when lying down was 16.7%. On CT scan, the average Goiter was 270g, the average narrow airway diameter was 8.4 mm, of which 12.7% of patients had narrow Tracheal diameter Conclusion: Tracheomalacia should be noted in patients older than 65 years old, goiter exist longer than 5 years and goiter is over 200g.

Hyperthyroid goitre treated surgically.
Roczniki Akademii Medycznej w Bialymstoku (1995), 1997Co-Authors: Jacek Dadan, Ladny, Zbigniew PuchalskiAbstract:: The aim of the study was the comparative analysis of the degree of intensity of male and female hyperthyroidism treated surgically in the years 1990 to 1996. In this period 295 females and 42 males underwent operation for hyperthyroid goitre. Female predominance was noted in hyperthyroidism (ratio 7:1), in Graves' disease (7.4:1) and in toxic nodular goitre (6.3:1). The clinical findings in pre- and postoperative patients, including laboratory, visual diagnosis, and intra- and postoperative complications were evaluated. In the preoperative period, the incidence of the thyreocardiac syndrome was greater in the male. Male hyperthyroidic goitres were more frequently located retrosternally and caused Trachea Compression. No significant sexual differences were found in routine laboratory tests. Operations for hyperthyroidic male goitres usually caused more intraoperative problems and were connected with greater blood loss. Estimation of cardio-vascular parameters in the early postoperative period showed higher intensification of hyperkinetic circulation and higher mean body temperature in men. Signs of psychosis developed postoperatively in two men. The analysis of patients with hyperthyroidic goitre treated surgically revealed more severe course of male thyreotoxicosis in the perioperative period.

Chinese Journal of Ophthalmology and Otorhinolaryngology, 2007Co-Authors: Qian MinfeiAbstract:Purpose To explore the method of treatment of huge thyroid tumor inducing Trachea Compression and shift.Methods The data of 50 cases with huge thyroid tumor inducing Trachea Compression and shift which were comfirmed hy CT and B ultrasound examination were analyzed retraspectively.Results All of the thyroid tumors were resected completely. Fourty six cases were diagnosed as benign thyroid tumors and 4 cases,as papillary carcinoma.Among the 50 cases,1 case was made tracheotomy because of dyspnea induced from tracheomalacia.In 1 case recurrent laryngeal nerve palsy emerged.Conclusion Active preoperative measures should he taken in the treatment of huge thyrood tumor in order to achieve good therapeutic effect.
