Spontaneous twin gestation after vaginal dilation in a woman with Uterus Didelphys and bladder exstrophy.
Obstetrics and gynecology, 2002Co-Authors: Eliran Mor, Peyman Saadat, Rebecca Z Sokol, Richard J PaulsonAbstract:Bladder exstrophy is an uncommon anomaly rarely associated with Uterus Didelphys. Fertility is limited by associated vaginal malformations, which have traditionally required surgical reconstruction. A woman with a history of bladder exstrophy and hypoplastic vagina presented complaining of painful intercourse. The patient underwent vaginal dilator treatment after corrective surgery for the bladder defect. Without any further fertility therapy she subsequently conceived twins, with one implantation in each horn of a didelphic Uterus. Bladder exstrophy, Uterus Didelphys, and vaginal hypoplasia share a common embryological devel-opment, and the finding of one anomaly can alert to the presence of another. Vaginal dilators may be used in place of surgical reconstruction to allow conception.

Spontaneous Triplet Pregnancy Complicated by Uterus Didelphys: A Case Report
Fetal diagnosis and therapy, 2010Co-Authors: Cesar Diaz-garcia, Jessica P. Subirá Nadal, Amaya Urgal Ayala, Eduardo Cazorla Amorós, Dolores Borrás Suñer, Alfredo Perales MarínAbstract:Uterine malformations and spontaneous triplet pregnancy are rare situations separately, and both are related to worse perinatal outcomes. In the 3 cases of combined triplet pregnancy and Uterus Didelphys previously published, at least 2 of the 3 fetuses died. Morbidity rates are also high. We report the case of a 24-year-old Caucasian woman with a Uterus Didelphys and two previous caesarean sections who carried a spontaneous triplet pregnancy. To our knowledge, this is the first case of a spontaneous triplet pregnancy in a Uterus Didelphys ever reported and the only one with uneventful perinatal outcome for all fetuses. Expectant management may be an alternative to embryo reduction to manage triplet pregnancies in a Didelphys Uterus, especially when this option is not considered. Nevertheless, there is not enough evidence to confirm which is the optimal management strategy, and more studies are needed.

Uterus Didelphys with unilateral obstructed hemivagina with hematometrocolpos and hematosalpinx with ipsilateral renal agenesis
Journal of Human Reproductive Sciences, 2009Co-Authors: Gaurav Jindal, Satish Kachhawa, G L Meena, Gopal DhakarAbstract:Uterus Didelphys with blind hemivagina and ipsilateral renal agenesis (Herlyn Werner-Wunderlich Syndrome) is a rare congenital anomaly. It mostly presents with severe dysmenorrhea and a palpable mass due to unilateral hematocolpos. A patient with dysmenorrhea from a double Uterus and an obstructed hemivagina is a diagnostic dilemma because the menses are regular. We report a case of a 14-year-old girl with this condition who was diagnosed as Uterus Didelphys with unilateral hematocolpos and hydrosalpinx with ipsilateral renal agenesis on the basis of sonography and confirmed by laparoscopic examination.
