    Abstract:

    Objective To review clinical implications, reproductive outcome, and long-term consequences in women with complete septate uterus and longitudinal Vaginal Septum. Design A retrospective study. Setting A university hospital. Patient(s) Sixty-seven patients who had a complete septate uterus including the cervix and a longitudinal Vaginal Septum. Intervention(s) The longitudinal Vaginal Septum was incised in 36 cases and metroplasty for uterine Septum was undertaken in 4 patients. Main Outcome Measure(s) Fertility, outcome of pregnancies, possible late consequences, and presence of other anomalies. Result(s) Eight (15.7%) of 51 women attempting pregnancy had primary infertility of nonuterine causes. Forty-nine women not undergoing metroplasty produced 115 pregnancies, abortion rate 27%, preterm delivery 12%, and live birth rate 72%. Only five women with no metroplasty had only miscarriages and 44 women at least one delivery. One of four women undergoing metroplasty delivered preoperatively and three after metroplasty. During the follow-up period endometriosis was observed in two (3%) of 61 cases and two patients had borderline ovarian tumors. Kidney abnormalities were found in 11 (20%) of 55 patients studied, double ureter being the most common. Conclusion(s) Complete septate uterus with longitudinal Vaginal Septum is not associated with primary infertility, and pregnancy may progress successfully without surgical treatment. The results do not support elective hysteroscopic incision of the Septum in asymptomatic patients or before first pregnancy.

    Abstract:

    The isolated, complete, transverse Vaginal Septum is one of the most infrequent anomalies of the female genital tract, and when it coexists with a septate uterus, it is even rarer. This report describes a case of transverse Vaginal Septum with septate uterus. A 12-year-old girl sought medical assessment because of severe cyclic lower abdominal cramping and pelvic pain. Local examination revealed a blind Vaginal pouch of 2 cm and on rectal examination a tender pelvic mass was noted. Radiological examination showed transverse Vaginal Septum in the lower vagina with bicornuate uterus. Surgical resection of the Vaginal Septum was done under laparoscopic guidance. Hysteroscopy revealed presence of uterine Septum which was resected by a resectoscope. Post-operative dilatation of vagina was done to prevent restenosis. Laparoscopic guided abdominoperineal approach is better in such a case as multiple mullerian anomalies may coexist with each other.

    Abstract:

    Background: A genital outflow tract obstruction is an uncommon cause of primary amenorrhea. If ovulation occurs, menstrual bleeding is prevented. Patients typically present with abdominal/pelvic pain due to hematocolpos. Absence of significant hematocolpos could indicate a secondary source of primary amenorrhea and be challenging to the clinical diagnosis. Case: 17 year-old patient with primary amenorrhea, appropriate Tanner staging secondary sex characteristics, and transverse Vaginal Septum presents with virtual absence of hematocolpos. After Vaginal Septum resection, the patient began menstruating, although only evidenced by two cycles of Vaginal spotting. Conclusion: Significant hematocolpos is an expected sequella of distal outlet obstruction when collated with secondary sexual characteristics. Absence of such along with suboptimal return of menstruation reflects pathophysiology which may be attributed to a coexistent disorder of the hypothalamus or higher central nervous system function.

    Abstract:

    Carcinoma of the recto-Vaginal Septum is a quite rare location and related to peritoneal and primary ovarian carcinomas. There are only few reports in the literature with a very poor prognosis. Here we report the case of a 63 years old woman with past medical history of left oophorectomy presenting with a pelvic pain. The magnetic resonance imaging (MRI) demonstrated a 10 cm mass located in the recto-Vaginal Septum. A block resection was performed allowing the retrieval of a 10 cm solid tumor of the recto-Vaginal Septum. Peritoneal biopsies and the right ovary were normal the final diagnosis was cystadenocarcinoma of the recto-Vaginal Septum. The patient received adjuvant chemotherapy and displays no sign of recurrence 36 months after diagnosis. The management of recto-Vaginal Septum carcinoma with en bloc resection should be performed to avoid peritoneal spread and improve prognosis.
