    Abstract:

    Objective To compare the changes of the electroneurophysiological on comprehensive therapy in mild occupational hand-arm Vibration Disease,and to observe the therapeutic effect.Methods The result of electroneurophysiological of 32 patients with mild occupational hand-arm Vibration Disease in 3-month comprehensive therapy was analyzed.Results After the therapy,the electroneurophysiological of 19 cases recovered to normal.The incidence rate of distal latency in median nerve and ulnar nerve was decreased form 90.63% to 23.44% and 35.94% to 1.56% respectively(P0.01).The distal sensor nerve conduction velocity(SCV) was decreased form 29.69% to 3.13% and 23.44% to 4.69% respectively(P0.01).Conclusion The early symptom of electroneurophysiological was the distal latency prolonged and the SCV slowed of the median nerve and ulnar nerve.The comprehensive therapy can obviously improve all indexes.It's suggested that the therapy should have a remarkable effect for patient with hand-arm Vibration Disease at the early stage.

    Abstract:

    To study the relationship between neurochemical changes in specialized tissue structures and the state of the conducting pathways of the central and peripheral nervous system, we analyzed the levels of neuronal antibodies (ABs), somatosensory evoked potentials, and electroneuromyography in patients with Vibration Disease (VD) induced by local Vibration. We found correlations between the concentrations of ABs against NF-200, MBP, S-100, GFAP, B2GP, Ser-P, DNA, voltage-dependent Ca channel, Glu-P and changes in the central conductive structures; as well as between ABs against MBP, GFAP, DNA, Ser-P, S-100 GABA-P, DA-P, Glu-P, AX-P and the rate of impulses along axons of various structures of the nervous system. The revealed interrelationships confirm the functional integration of the nervous and immune systems and the important role of autoimmune reactions in the development and course of VD. Possible mechanisms of neurotransmitter metabolism disorders and demyelination are discussed. These results may serve as a basis for the development of new informative laboratory indicators and differential diagnostics of subjects working under the influence of local Vibration.

    Abstract:

    Pathological changes of the state of peripheral nerves according to data of electroneuromyography examination were observed both in patients with Vibration Disease as a result of local Vibration impact and in patients with occupational sensorineural deafness. This suggests general mechanism of impact of physical factors on the human organism. In the first case observed changes were of more expressed character.

    Abstract:

    Electroneuromyography in workers exposed to local Vibration revealed disordered functional state of nerves in lower and upper limbs, more significant in arms. Individuals exposed to local and general Vibration combined with occupational hazards demonstrated more frequent axonal type of ulnar nerve disorder, with less length of service in comparison with group 1 members.

    The article covers the particular features of the neuro-muscular system in Vibration Disease of various types and severity by electroneuromyographical tests at the Department of Occupational Disorders, Regional Center for Occupational Disorders, Samara Medical and Sanitary Unit № 5 of the Kirov Region (State Budgetary Healthcare Institution of the Samara Region). Changes in excitability and functional mobility of the sensorimotor system, development of vegetative-sensory polyneuropathy depending on the Vibration Disease type, severity, and the length of exposure to Vibration at work are found in Vibration Disease patients.

    Abstract:

    The article presents data on the impact of Vibration Disease on the quality of life of patients. The main changes in the quality of life parameters of patients based on the results of the study using the SF-36 questionnaire are described. The quality of life of patients with various types of Vibration Disease is compared. It is established that industrial Vibration (both General and local) in addition to the classical changes characteristic of Vibration Disease from exposure to local Vibration (WBL), Vibration Disease from exposure to Vibration (VBOW) first and second degrees (vascular, neurological) substantially violates the quality of life of patients, with the most pronounced changes are characteristic for WBOW, which confirms the systemic effects of Vibration and its effects on the body working. The quality of life of sick WIDOWS is significantly reduced, although significant differences are observed only on the scales of «physical functioning» and «physical component of health».

    The article describes the peculiarities of Central hemodynamics and myocardial contractility in patients with Vibration Disease, various forms and degrees of severity with the help of echocardiography of the Department of pathology of the regional center of occupational pathology of SBME "Samara mediko-a sanitary part of № 5 of Kirov district". It was found that patients with Vibration Disease are characterized by the formation of a phase syndrome of myocardial hyperdynamics and a hyperkinetic variant of Central hemodynamics, while hemodynamic changes depend on the form of Vibration Disease, the severity of the Disease, and the length of work in contact with Vibration.

    Vibration Disease (WB) is one of the leading Diseases among workers at engineering enterprises, in the metallurgical, mining and construction industries, and in various branches of agriculture. The purpose of the study was to study the state of the hormonal profile and the severity of erectile dysfunction in men with Vibrational Disease. Violations of interaction in the system of the hypothalamic-pituitary-gonadal system in men with EF from the effects of general Vibration, which is a harmful production factor in drivers of heavy vehicles, machine operators of agricultural enterprises, were revealed. The decrease in testosterone does not reach statistically significant changes compared with the control group and does not cause the naturally expected increase in the secretion of follicle-stimulating hormone and luteinizing hormone according to the law of feedback.

    Clinical and functional studies helped to assess influence of vegetative nervous system on course of Vibration Disease and chronic lumbosacral radiculopathy in miners. Vegetative dysfunction syndrome varying in intensity was associated with simultaneous overstrain in segmentary and supersegmentary units of vegetative nervous system. The authors defined dependence between vegetative dysfunction intensity and neuromuscular disorders severity. With this, increased threshold of N-reflex with lower amplitude accompanied decrease in M-response amplitude and shorter latent period, in accordance with peripheral nerve impulse speed. The dependence between vegetative nervous system strain and neuromuscular disorders intensity underlies pain syndrome degree in Vibration Disease and chronic lumbosacral radiculopathy in miners.

    Abstract:

    Clinical immunologic and electroneuromyographic examination covered patients with Vibration Disease. Registered- demyelination disorders on lower and upper limbs in Vibration Disease patients were associated with neurochemical changes in special neural structures (OBM, S-100 protein, DNA, AH-R, DA-R, Ser-R, GAMK-R). Direct proof of demyelination disorders in Vibration Disease patients are reliable correlation between levels of antibodies to OBM and conduction velocity in distal part of median nerve, in ulnar nerve from elbow bend and lower third of arm, and ulnar residual latency. Diagnosed relations between levels of antibodies to regulatory proteins in nervous tissue (antibodies to OBM and to S-100 protein), neurotransmitters (antibodies to AH-R, DA-R, Ser-R, GAMK-R) and functional parameters of nervous system prove important role of autoimmune reactions in Vibration Disease pathogenesis.
