Arthroscopy techniques, 2017Co-Authors: Justin S Chang, James P Higgins, Jonathan D Kosy, John TheodoropoulosAbstract:Abstract Pigmented Villonodular Synovitis (PVNS) is a benign aggressive disease that presents in either a localized (LPVNS) or diffuse (DPVNS) form. Arthroscopic synovectomy is the standard operative treatment for LPVNS, and when used to treat DPVNS, it is usually combined with an open posterior procedure. The purpose of this Technical Note is to report the technique that we have refined to allow for arthroscopic synovectomy as the sole treatment for DPVNS. We describe our technique with the factors we have found to be important to ensure adequate arthroscopic synovectomy, while minimizing risks and complications. The combination of additional portals, the use of multiple different shavers and arthroscopes, and the use of a leg holder all maximize our ability to clear disease.

The abnormal synovial proliferation was noted at arthroscopy, and histological analysis of the resected tissue confirmed the diagnosis.

Journal of Arthroplasty, 2010Co-Authors: Jeong Joon Yoo, Young Sam Kwon, Kyunghoi Koo, Kang Sup Yoon, Byung Woo Min, Hee Joong KimAbstract:The authors retrospectively reviewed all patients with pigmented Villonodular Synovitis (PVNS) of the hip treated by cementless total hip arthroplasty (THA) combined with synovectomy. Eight patients were reviewed and observed for an average of 8.9 years (range, 4.3-13.5 years). Mean preoperative Harris hip scores improved from 49.3 to 96.6 points, and all patients were able to regularly perform moderate daily living activities. None of the patients had clinical or radiographic evidence of recurrent PVNS. Osteolysis occurred in 4 hips, and 2 revision surgeries were performed during follow-up. Cementless THA combined with synovectomy is an adequate therapeutic choice for patients with PVNS demonstrating end-stage joint destruction and appears to be effective at improving clinical results and preventing disease recurrence.

Clinical Orthopaedics and Related Research, 2000Co-Authors: Sungjae Kim, Sangjin Shin, Namhong Choi, Euitak ChooAbstract:This study investigated 11 patients with localized pigmented Villonodular Synovitis of the knee that was diagnosed and treated by arthroscopic technique. There were six male and five female patients between the ages of 15 and 59 years (mean, 34.6 years). Seven patients reported extension limitation without joint line tenderness. Four of the 11 patients had a history of trauma before the onset of knee symptoms. All patients were treated by arthroscopic resection with partial synovectomy. The most common involved site was the anteromedial synovium near the anterior horn of the medial meniscus (five patients). The remaining cases were located in the anterior fat pad (two patients), suprapatellar pouch, posteromedial compartment, medial gutter, and the anterior horn of the lateral meniscus. Nine patients had one mass, and the remaining patients each had two or three masses. There was no evidence of recurrence at followup for an average of 29.9 months (range, 24-48 months). Arthroscopy is effective in the diagnosis of localized pigmented Villonodular Synovitis with minimal morbidity, and complete arthroscopic excision can be considered the definitive treatment for localized pigmented Villonodular Synovitis.
