Reasons for Prolonged Bottle-Feeding and Iron Deficiency Among Mexican-American Toddlers: An Ethnographic Study

Mexican-American toddlers are the racial/ethnic group at greatest risk for prolonged Bottle-Feeding and iron deficiency, yet no studies have examined reasons for prolonged Bottle-Feeding in Mexican-American toddlers. The objective of this study was to assess infant Feeding beliefs, knowledge, and behaviors among Mexican-American parents. Methods Ethnographic interviews were conducted of parents of Mexican-American toddlers 15 to 48 months old at 3 community sites. A 31-question moderator's guide addressed 4 domains: knowledge and cultural beliefs; sources of nutritional information; anticipatory guidance; and suggestions for ways to change infant Feeding practices. Interviews were audiotaped, transcribed, and analyzed using grounded theory. Results Thirty-nine parents were interviewed; the mean parental age was 29 years, and mean child age, 2.2 years. Parents cited convenience as a reason for prolonged Bottle-Feeding, and believed that they should give toddlers as much milk as they want. Many parents lacked essential knowledge regarding infant Feeding practices and iron deficiency, including when to stop Bottle-Feeding, health problems caused by prolonged Bottle-Feeding, the quantity of milk to give a child >1 year old, and iron deficiency as a complication of prolonged Bottle-Feeding. Parents reported not receiving enough education from physicians, and they supported educational interventions on healthy infant Feeding practices, including refrigerator magnet charts, videos, brochures, and teaching by physicians. Conclusions Parents of Mexican-American toddlers often are unaware of the adverse consequences of prolonged Bottle-Feeding and developmental problems associated with iron deficiency. Parents supported educational interventions, including videos, brochures, and refrigerator magnet charts on healthy infant Feeding practices.

The Neonatal Eating Assessment Tool (NeoEAT) is a parent-report assessment that currently has two versions: NeoEAT – BreastFeeding and NeoEAT – Bottle-Feeding for use in breast and Bottle-fed infants, respectively. There are currently no valid and reliable parent-report measures to assess Feeding through a combination of both breast and Bottle delivery. The purpose of this study was to conduct a factor analysis and test the psychometric properties of a new measure, the NeoEAT – Mixed BreastFeeding and Bottle-Feeding (NeoEAT – Mixed Feeding), including internal consistency reliability, test-retest reliability, construct validity and known-groups validity. Methods Parents of infants younger than 7 months who had fed by both Bottle and breast in the previous 7 days were invited to participate. Internal consistency reliability was tested using Cronbach’s α. Test-retest reliability was tested between scores on the NeoEAT – Mixed Feeding completed 2 weeks apart. Construct validity was tested using correlations between the NeoEAT – Mixed-Feeding, the Infant Gastroesophageal Reflux Questionnaire - Revised (I-GERQ-R), and the Infant Gastrointestinal Symptoms Questionnaire (IGSQ). Known-groups validation was tested between healthy infants and infants with Feeding problems. Results A total of 608 parents participated. Nursing Research, 2004Co-Authors: Suzanne M Thoyre, Roger L BrownAbstract:Background: Preterm infants have difficulty maintaining engagement throughout early oral Feedings, which can lead to less efficient Feeding and prolonged Feeding skill development. Objective: To examine contributions of the infant, mother, and Feeding context to infant engagement during Bottle-Feeding. Methods: Bottle-Feedings of very-low-birthweight infants (n = 22) by their mothers were observed. Infant and maternal behaviors were coded and synchronized with physiologic measures. At completion of the Feeding, the mothers were interviewed, and their working model of Feeding coregulation was scored. Feedings were subdivided into Feeding episodes (n = 114). Using multilevel linear regression analyses, four dyadic characteristics (working model of the caregiver's role as coregulator, birthweight, postconceptional age, baseline oxygen saturation) and five episode characteristics (readiness at episode onset, episode baseline oxygen saturation, mean oxygen saturation during the episode, maternal Feeding behavior, and phase of Feeding) were examined as potential predictors of Feeding episode engagement. Results: Conditions observed during the Feeding observation explained most of the variation in engagement. Engagement was more likely to occur during the early phase of Feeding (p <.05), during Feeding episodes that began with infant readiness (p <.05), and during Feeding episodes with higher mean oxygen saturation during the episode (p <,05). Feeding episodes with less jiggling of the nipple had a significantly greater amount of engagement (p <.05). Conclusions: The ability of the preterm infant to maintain engagement during Bottle-Feeding cannot be explained by characteristics of the infant or by the preFeeding condition of the infant alone. Rather, engagement is coregulated by the caregiver and the infant throughout the Feeding. Strategies to assist infants in maintaining physiologic stability during Bottle-Feeding and further study of effective and contingent caregiver Feeding behaviors are needed.
