Transcatheter Cerebral Revascularization in the Treatment of Atherosclerotic Lesions of the Brain

MaksimovichAbstract:Background: The research investigates cerebral blood supply recovery be means of transcatheter laser revascularization in patients with advanced Brain Atherosclerosis. Materials and Methods: 974 patients aged 29-81 (mean age 74) with various types of cerebral Atherosclerosis. Transcatheter treatment was performed in 594 (60.99%) cases-Test Group. Conservative treatment was performed in 380 (39.01%) cases-Control Group. Examination plan: CDR, MMSE, IB evaluation, cerebral SG, REG, CT, MRI, MRA, MUGA. High-energy lasers were used for main intracranial arteries revascularization; low-energy lasers-for intracranial distal branches revascularization. Results: Treatment results were studied in the early period (2-6 months) and in 2-10 years. Test Group: 478 (80.47%) patients showed good clinical outcome, 234 (48.95%) of them were re-examined: results persisted in 217 (92.73%), satisfactory outcome was observed in 17 (7.27%). Satisfactory clinical outcome-96 (16.16%) patients, 55 (57.29%) reexamined: results persisted in 50 (90.91%), relatively satisfactory outcome-in 5 (9.09%). Relatively satisfactory clinical outcome-20 (3.37%), re-examined 10 (50.00%): results persisted in 8 (80.00%), relatively positive outcome-2 (20.00%). Control Group: good clinical outcome was not obtained. Satisfactory clinical outcome was obtained in 65 (17.11%), 37 (56.92%) were re-examined: results persisted in 14 (37.84%), relatively satisfactory outcome-in 23 (62.15%). Relatively satisfactory clinical outcome was obtained in 122 (32.11%), re-examined 75 (61.48%): results persisted in 34 (45.33%), relatively positive outcome-in 41 (54.67%). Relatively positive outcome was obtained in 193 (50.78%), re-examined 86 (44.56%): results persisted in 40 (46.51%), worsened condition was detected in 46 (53.49%). Conclusions: The method of transcatheter laser revascularization of cerebral vessels is physiological, effective and of small traumatism; it stimulates natural angiogenesis, collateral, capillary revascularization and mental and motor disorders regression. The effect persists up to 10 years, which makes the method significantly different from conservative treatment.

Transcatheter Technologies in Treating Brain Atherosclerosis Complicated by Chronic Cerebrovascular Insufficiency

MaksimovichAbstract:The research investigates the possibility of restoring the blood supply in patients with Atherosclerosis of the Brain, as well as the treatment of chronic cerebrovascular insufficiency. The research involves 946 patients aged 29 - 81 (average age 74) suffering from various types of cerebral Atherosclerosis. 568 (60.04%) patients underwent transcatheter treatment—Test Group. 378 (39.96%) patients underwent conservative treatment—Control Group. The examination plan included laboratory diagnostics, assessment of dementia severity (CDR), assessment of cognitive impairment (MMSE), evaluation of the activities of daily living (IB), cerebral computed tomography (CT), cerebral magnetic resonance imaging (MRI), cerebral scintigraphy (SG), rheoencephalography (REG), cerebral multi-gated angiography (MUGA). To restore the blood supply, the method of transcatheter laser revascularization was applied; high-energy pulsed lasers were used for major intracranial arteries treatment, and low energy CW lasers—for distal intracranial branches treatment. Test Group: 459 (80.81%) patients had good clinical outcome, 91 (16.02%)—satisfactory clinical outcome, 18 (3.17%)—relatively satisfactory clinical outcome; relatively positive clinical outcome was not obtained in any case. Control Group: good clinical outcome was not obtained in any case; 65 (17.20%) patients had satisfactory clinical outcome, 121 (23.26%)—relatively satisfactory clinical outcome, 192 (50.79%)—relatively positive clinical outcome. The method of transcatheter laser revascularization of cerebral vessels is a physiological, effective and low-invasive treatment for patients suffering from Atherosclerosis of the Brain. The obtained results last up to 10 years and more; it causes regression of mental and motor disorders, promotes regression of dementia and largely improves the patients’ quality of life; it has virtually no alternative—which makes the proposed method significantly different from conservative treatment methods.

Morphology study on the affections of Brain Atherosclerosis in optic nerve blood supply
Chinese Journal of Practical Ophthalmology, 2004Co-Authors: Liu XueAbstract:ObjectiveIn order to provide morphological evidences for visual field defect caused by optic nerve ischemia.Methods The origins,numbers,distributions and pathological changes of optic nerve arteries in 102 sides of adult Brain specimens were observed with microsteroscope and operation microscope,among which the Pathological slide from nourishing arteries of optic nerve in 60 sides aged from 50 to 70 were did.Results The blood supply of optic nerve came mainly from internal carotid,cerebral anterior artery and anterior communicating artery.5% of the blood supply of optic nerve in 60 sides were mono-origin and 58 3% of them were three origins.88 3% of them had Atherosclerosis,in which some eye arteries (3 eyes) and superior hypophyseal arteries (11 eyes) were blocked completely and 11 sides were blocked partly.There were obvious impressions on the optic nerves from the compressions by the sclerosed internal carotid arteries.Pathological changes in optic nerve,for example,the neual fiber atrophy,were observed.Conclusions Excluding other disease,the declining of eyesight and visual field (especially the surrunding and the central part) in patients over 50 years old were caused possibly by Atherosclerosis of the arteries at the Brain.

Morphological study on blood supply of optic chiasma affected by Brain Atherosclerosis and its clinical significance

Methods: The origins, numbers, distributions and pathological changes of optic chiasma arteries in 102 sides of adult Brain specimens were observed with microstereoscope and operation microscope, among which the nourishing arteries of optic chiasma in 60 sides aged from 50 to 70 were studied by pathological section method. Results: The blood supply of optic chiasma comes mainly from internal carotid artery, posterior communicating artery and the branches of anterior cerebral artery. 48.0% of these arteries in 60 sides were mono-origin; 29.3% were two origins; 7.9% three origins. 88.3% of them existed Atherosclerosis and the optic chiasma arteries of 3 sides were blocked completely and 8 sides partly. Pathological changes in optic chiasma, such as neural fiber atrophy, were observed. Conclusions: Excluding other diseases, the eyesight decline, visual field defect, above all upper nasal visual field defect in patients over 50 years old were caused possibly by Atherosclerosis of the arteries at the base of the Brain.

The value of ophthalmoscopy in the diagnosis of arterial hypertension in patients with ischemic Brain disease.

Significant number of patients is not aware of their hypertension, and a paradoxical blood pressure decrease can occur at the onset of IBD, due to the failure of autoregulation mechanisms. Likewise, valid anamnestic data can not frequently be obtained due to difficulties in communication with patients. Regarding these facts, our hypothesis was that ophthalmoscopy in patients with IBD had the greatest sensitivity in the diagnosis of hypertensive disease, its duration and severity. For that reason, the purpose of this study was to determine the significance of ocular fundus examination in those patients with IBD who were not aware of their hypertension, or the high blood pressure was not registered at the admission. Study comprised 140 IBD patients selected upon the following criteria: ophthalmoscopy was performed by the same ophthalmologist, and IBD was diagnosed according to clinical criteria and by Brain computerized tomography. Results of the study demonstrated that 26 (18.6%) patients, although not aware of having hypertensive disease, had grade I hypertonic fundus, 14 (10%) had grade II, and 8 (5.5%) had grade III hypertonic fundus, which indicated the high sensitivity of ophthalmoscopy in the diagnosis of hypertensive disease, as well as its duration and severity. This is particularly important in patients with negative history of hypertension, and also suggests the significance of routine ophthalmoscopy in normotensive patients.

Association of risk factors for cerebral Atherosclerosis in patients with acute ischemic Brain disease

JovicAbstract:Atherosclerosis is the primary disease in the majority of patients with ischemic Brain disease (IBD). Etiopathogenesis of Atherosclerosis has not been more precisely defined, but risk factors significant for its generation and development, so as for IBD development, have been identified. They were designated as risk factors for Brain Atherosclerosis (RFBA). The aim of this study was to establish the frequency and significance of RFBA association in the patients with acute IBD in relation to IBD severity. The investigation included 60 patients with AIBD. RFBA were: arterial hypertension, dyslipoproteinemia, diabetes and smoking. For all the patients included in the study detailed history was noted, daily glycemia profile was done, blood pressure was measured 3 times a day and parameters of lipid status with counting of LDL/HDL indices and Apo A1/Apo B were determined. IBD was confirmed by computerized tomography of Brain. The degree of IBD severity was determined in all the patients by Grotta's scale. After the statistical processing, the results of the research showed that in the patients with IBD the most frequent were arterial hypertension, then smoking, dyslipoproteinemia and diabetes. In significant number of patients existed association of several factors, which was in significant positive correlation with the degree of IBD severity. It was concluded that risk factors for Atherosclerosis were simultaneously risk factors for IBD. IBD is the most severe, final stage in evolution of Brain Atherosclerosis.
