Early and adequate atropine administration in organophosphorus (OP) or Carbamate insecticide poisoning improves outcome. However, some authors advise that oxygen must be given before atropine due to the risk of inducing ventricular dysrhythmias in hypoxic patients. Because oxygen is frequently unavailable in district hospitals of rural Asia, where the majority of patients with insecticide poisoning present, this guidance has significant implications for patient care. The published evidence for this advice is weak. We therefore performed a patient cohort analysis to look for early cardiac deaths in patients poisoned by anticholinesterase Pesticides. Methods. We analysed a prospective Sri Lankan cohort of OP or Carbamate-poisoned patients treated with early atropine without the benefit of oxygen for evidence of early deaths. Unfortunately, little clinical research has been performed and little evidence exists with which to determine best therapy. A cohort study of acute Pesticide poisoned patients was established in Sri Lanka during 2002; so far, more than 2000 Pesticide poisoned patients have been treated. A protocol for the early management of severely ill, unconscious organophosphorus/Carbamate-poisoned patients was developed for use by newly qualified doctors. It concentrates on the early stabilisation of patients and the individualised administration of atropine. We present it here as a guide for junior doctors in rural parts of the developing world who see the majority of such patients and as a working model around which to base research to improve patient outcome. Improved management of Pesticide poisoning will result in a reduced number of suicides globally.

IEEE Journal of Solid-state Circuits, 2016Co-Authors: Zhihong Shi, Qingru Huai, Hongyi ZhangAbstract:Graphene-based pipette tip solid-phase extraction was combined with ultra high performance liquid chromatography-tandem mass spectrometry for the determination of Carbamate Pesticide residues in fruit juice samples. 4 mg of graphene was used as sorbent material to pack a 1000 μL pipette tip for the extraction of pirimicarb, propoxur, isoprocarb, fenobucarb and diethofencarb from 3 mL of fruit juice sample. The whole extraction process was finished in 12 min, and the volume of eluent used was only 1.5 mL. Under the optimized conditions, good linear relationship (R>0.999) and lower limits of detection (0.0022–0.033 ng⋅mL−1) were achieved. The recoveries at three spiked levels ranged from 80.90 to 124.60% with relative standard deviations less than 4.88%. Compared with commercially available sorbents including propylsulfonic acid silica, graphitized carbon black, and C18, graphene was superior in extraction efficiency. quantification and confirmation of fifteen Carbamate Pesticide residues by multiple reaction monitoring and enhanced product ion scan modes via lc ms ms qtrap system
Molecules, 2018Co-Authors: Ying Zhou, Jian Guan, Weiwei GaoAbstract:In this research, fifteen Carbamate Pesticide residues were systematically analyzed by ultra-high performance liquid chromatography–quadrupole-linear ion trap mass spectrometry on a QTRAP 5500 system in both multiple reaction monitoring (MRM) and enhanced product ion (EPI) scan modes. The Carbamate Pesticide residues were extracted from a variety of samples by QuEChERS method and separated by a popular reverse phase column (Waters BEH C18). Except for the current conformation criteria including selected ion pairs, retention time and relative intensities from MRM scan mode, the presence of Carbamate Pesticide residues in diverse samples, especially some doubtful cases, could also be confirmed by the matching of Carbamate Pesticide spectra via EPI scan mode. Moreover, the fragmentation routes of fifteen Carbamates were firstly explained based on the mass spectra obtained by a QTRAP system; the characteristic fragment ion from a neutral loss of CH3NCO (−57 Da) could be observed. The limits of detection and quantification for fifteen Carbamates were 0.2–2.0 μg kg−1 and 0.5–5.0 μg kg−1, respectively. For the intra- (n = 3) and inter-day (n = 15) precisions, the recoveries of fifteen Carbamates from spiked samples ranged from 88.1% to 118.4%, and the coefficients of variation (CVs) were all below 10%. The method was applied to Pesticide residues detection in fruit, vegetable and green tea samples taken from local markets, in which Carbamates were extensively detected but all below the standard of maximum residue limit.
