  • Application of Minimally Invasive Surgery in Treating Posterior Fossa Hemorrhage

    Abstract:

    Objective To summarize the minimally invasive surgeries for posterior fossa Hemorrhage,including Cerebellum Hemorrhage and epidural Hemorrhage/hematoma.Methods Thirty patients with posterior fossa Hemorrhage,in which 19 cases were with cerebellar Hemorrhage,11 cases with occipital epidural/hematoma,received the minimally invasive by using YL-1 puncture needle to drill a hole in skull and indwelling silicon catheter to evacuate hematoma.The effects of the minimally invasive surgeries were compared with the traditional craniotomy.Results The good results were obtained in the experimental group.One case was with postoperative bleeding and brain hernia,and underwent line decompressive craniotomy to evacuate the hematoma.Conclusion The minimally invasive surgeries can reduce invasive,cause less complications,reduce hospitalization costs,improve the cure rate and quality of life of patients with posterior fossa Hemorrhage.

  • Microsurgical treatment of cerebellar cavernous angioma

    Abstract:

    Objective To investigate the clinical presentation and surgical results of cerebellar cavernous angiomas.Methods A total of 12 cases with cerebellar cavernous angiomas confirmed by pathology and operation were analyzed retrospectively.Results Eleven patients had acute onset with the symptom of Cerebellum Hemorrhage and 1 case had chronic onset presenting with the progressive ataxia.T1WI showed hyper-intense signal in 7 cases;T2WI showed non-homogeneous hyper-and mixed-intense signal with a surrounding hypo-intense rim.Susceptibility-weighted imaging(SWI) increased the diagnostic specificity and sensitivity of cerebellar cavernous angioma.Conclusion Spontaneous Hemorrhage is the main symptom of cerebellar cavernous angioma,which usually is accompanied by the signs of Cerebellum damage such as ataxia.As a method of diagnosis,MRI is highly specific and sensitive and microsurgical treatment is a safe and effective option for cerebellar cavernous angioma.

  • Treatment of 27 patients with hypertensive cerebellar hematoma with fourth ventricle hematoma

    Abstract:

    Objective To investigate the time,method and therapeutic effect of surgery on Cerebellum Hemorrhage with fourth ventricle hematoma.Methods 27 patients with hypertensive cerebellar hematoma and fourth ventricle hematoma were retrospectively analyzed.Results There were 17 male and 10 female patients,with age between 41 and 79 years.There were 9 patients with cerebellar hemisphere Hemorrhage and 18 with vermis Cerebellum.Obstructive hydrocephalus was found in 15 patients.Followed-up from 3 months to 3 years,9 patients was good recovery,11 moderate disability,2 severe disability,and 5 death.Conclusion Prompt evacuation of cerebellar and intraventricular hematoma can relief brain stem compression and prevent function deteriorating,which might improve patients'outcome.

    Abstract:

    Objective To study the reason,clinical manifestations,treatment methods and clinical effects of Hemorrhage in Cerebellum.Methods Clinical information and treatment methods of 42 cases of Cerebellum Hemorrhage treated by us in recent 5 years were retrospectively analyzed.Results The main reasons of Cerebellum Hemorrhage were hypertension and arteriosclerosis.Clinical features were complex and diverse.When bleeding amounts less than 15 mL,they were given conservative treatment and when bleeding amounts higher than 15 mL,they were treated by lateral ventricle drainage and minimally invasive surgery.Bleeding amounts less than 5 mL had a good prognosis;when higher than 25 mL prognosis was very poor.Conclusion Bleeding amounts,compressing brain stem or not and whether Hemorrhage disrupting into the fourth ventricle are key factors of treatment method selection and prognosis judgment.
