    Problem In a previous clinical study, the incidence of Chronic Otitis Media in cases of otosclerosis was reported to be less than that observed in patients without otosclerosis. Histopathologically, we can detect minimal changes such as histological otosclerosis or silent Otitis Media which are not detected clinically. The purpose of this study is to reveal the association of otosclerosis and Chronic Otitis Media by evaluating human temporal bones, histopathologically. Methods 1235 human temporal bones were reviewed for this study. In order to match patients with otosclerosis, patients with Chronic Otitis Media were limited to 16 to 92 years of age. The incidence of otosclerosis (clinical otosclerosis, histological otosclerosis) and Chronic Otitis Media, either clinical (tympanic membrane perforation) or silent (without perforation) were analyzed. Results There was no statistically difference between the incidence of Chronic Otitis Media in temporal bones with and without otosclerosis. Conclusion The association of Chronic Otitis Media and otosclerosis appears to be a coincidental. Significance The incidence of Chronic Otitis Media in cases of otosclerosis is not less than that observed in cases of Chronic Otitis Media in cases without otosclerosis.     Abstract:

    Objectives: Otitis Media with effusion and obesity are both common in childhood and might share some immunological alterations. This study aimed to investigate the relationship between Chronic Otitis Media with effusion and childhood overweight or obesity, including the potential effects of adenoid or tonsillar hypertrophy on that relationship. Methods: This study included 60 children with Chronic Otitis Media with effusion and 86 healthy children aged from 2 to 10 years. Measures of height and weight were used to calculate the body mass index, weight for height and weight z score. Results: The prevalence of overweight or obesity was higher in children with Chronic Otitis Media with effusion, according to the weight for height percentiles ( p = 0.012). However, neither the presence of adenoid or tonsillar hypertrophy nor the degree of adenoid hypertrophy was associated with overweight or obesity. Conclusion: Overweight and obesity might be risk factors for developing Chronic Otitis Media with effusion, or vice versa.

    The information on incidence of the facial nerve canal dehiscence in Chronic Otitis Media is important for surgeons. The purpose of this study is to disclose the histopathologic findings of facial nerve canal dehiscence in human temporal bones with Chronic Otitis Media. We divided the human temporal bones into two groups (age 4 years, and under 4 years of age). We evaluated the incidence and the area of the facial nerve canal dehiscence in Chronic Otitis Media under light microscopy. Age-matched normal control temporal bones were also examined. In the age group of 4 years, 68.9 % of temporal bones with Chronic Otitis Media and 71.9 % of controls had the facial nerve canal dehiscence. There was no significant difference between them (P = 0.61). The area of the dehiscence in temporal bones with Chronic Otitis Media was not statistically different from controls (P = 0.53). In the age group under 4 years, 88.2 % of temporal bones with Chronic Otitis Media and 76.5 % of controls had the dehiscence. No significant difference was found between them (P = 0.66). The area of the dehiscence in temporal bones with Chronic Otitis Media was not statistically different from controls in the age group under 4 years (P = 0.43). In Chronic Otitis Media, the incidence of facial nerve canal dehiscence was high and was not statistically different from controls. These results suggest that there is no association between Chronic Otitis Media and the presence of facial nerve canal dehiscence.

    Objectives/Hypothesis: The objective was to describe the morphological changes in the cochlea in Chronic Otitis Media. Study Design: Retrospective human temporal bone analysis. Methods: Fifteen temporal bones with unilateral Chronic Otitis Media were selected and compared with contralateral normal temporal bones. Standard cytocochleograms and spiral ganglion cell reconstructions were performed on all temporal bones. Spiral ligament was divided into four segments according to the locations of different types of fibrocytes. The average loss of fibrocytes in each segment was estimated. Morphometric measurements of areas of stria vascularis and spiral ligament were made in all turns of the cochlea on mid modiolar sections. Results: Loss of outer and inner hair cells was common in the basal turn of the cochlea in temporal bones with Chronic Otitis Media compared with control ears. There was no difference in the number of spiral ganglion cells in the Chronic Otitis Media and contralateral ears. The areas of stria vascularis and spiral ligament in the basal turn decreased significantly in the ears with Chronic Otitis Media compared with control ears. There were no significant differences between the ears with Chronic Otitis Media and the contralateral ears for any of the regions characterized by the presence of types I-IV fibrocytes. Conclusion: The results of the study are consistent with the hypothesis that Chronic Otitis Media causes cochlear disease.

    ABSTRACT Inflammation of the middle ear, known clinically as Chronic Otitis Media, presents in different forms, such as Chronic Otitis Media with effusion (COME; glue ear) and Chronic suppurative Otitis Media (CSOM). These are highly prevalent diseases, especially in childhood, and lead to significant morbidity worldwide. However, much remains unclear about this disease, including its aetiology, initiation and perpetuation, and the relative roles of mucosal and leukocyte biology, pathogens, and Eustachian tube function. Chronic Otitis Media is commonly modelled in mice but most existing models only partially mimic human disease and many are syndromic. Nevertheless, these models have provided insights into potential disease mechanisms, and have implicated altered immune signalling, mucociliary function and Eustachian tube function as potential predisposing mechanisms. Clinical studies of Chronic Otitis Media have yet to implicate a particular molecular pathway or mechanism, and current human genetic studies are underpowered. We also do not fully understand how existing interventions, such as tympanic membrane repair, work, nor how Chronic Otitis Media spontaneously resolves. This Clinical Puzzle article describes our current knowledge of Chronic Otitis Media and the existing research models for this condition. It also identifies unanswered questions about its pathogenesis and treatment, with the goal of advancing our understanding of this disease to aid the development of novel therapeutic interventions.

    Chronic Otitis Media is a Chronic inflammatory disease of the middle ear cleft which is manifested as deafness and ear discharge. It is a common condition affecting 0.5 - 30% of any community worldwide, and most common in developing countries. The prevalence mainly depends on age, low socio-economic status, overcrowding and limited medical facilities. To compare the clinical presentation between Chronic Otitis Media mucosal with squamous types. A prospective, longitudinal and analytical study performed among 200 patients who have features of Chronic Otitis Media and were attended to in the ear, nose and throat outpatient department of Dhulikhel Hospital - Kathmandu University Hospital, Dhulikhel, Nepal from January 2010 to January 2011. Patients who had already undergone surgery and came back to the hospital, with inadequate information were excluded from the study. The results showed that, out of 200 cases, 120 (60%) were Chronic Otitis Media- mucosal and 80 (40%) were Chronic Otitis Media - squamous. Patients less than 30 years were affected in both groups. The male to female ratio in Chronic Otitis Media mucosal was 1.14:1, whereas in Chronic Otitis Media squamous, it was 1.96:1. Similarly in etiological factors, oil and water exposure (76%) was the most common cause in Chronic Otitis Media mucosal, whereas in Chronic Otitis Media squamous, oil and water exposure (62.5%) and recurrent upper respiratory tract infection (62.5%) were the most common causes. Of all the clinical features, ear discharge was the leading feature in both Chronic Otitis Media mucosal (98%) and squamous (100%) cases. The complications were mainly seen in the Chronic Otitis Media squamous and the most common complication was mastoid abscess (5%). The frequency of Chronic Otitis Media is higher in the younger age group and those lacking in health education. As a result, it is important to disseminate the health education regarding the ear disease which will help in decreasing the frequency of the disease in developing countries like ours.

    Abstract Objectives: Chronic Otitis Media is currently associated with an increase in air conduction thresholds. However, a few groups reported a decrease in sensorineural function in these patients. This study evaluates the occurrence of sensorineural hearing loss in patients with Chronic Otitis Media. Methods: The records of patients with unilateral Chronic Otitis Media were reviewed. Eighty-four patients met the inclusion criteria that were normal otoscopy and normal hearing in the contra lateral ear. Bone conduction threshold averages were calculated over 500 Hz, 1000 Hz, 2000 Hz, 3000 Hz and 4000 Hz frequencies. We compared the bone conductions threshold averages between the normal ear and the ear with Chronic Otitis Media. Thresholds were examined separately for each frequency. Results: The bone conduction threshold averages in the normal side were smaller than those of the Chronic Otitis Media ear. The threshold shift was statistically significant in each frequency ( P t Student test) but in 500 Hz ( P = 0.16). There were no differences between the groups when we analyzed the role of age or in the role of the cholesteatoma presence in the sensorineural hearing loss. Conclusions: This study shows that Chronic Otitis Media is associated with decrease in cochlear function. Neither the presence of cholesteatoma in the middle ear nor the patients’ age showed any influence in the sensorineural hearing loss.

    Chronic Otitis Media is generally associated with some degree of hearing loss, which is often the patient’s chief complaint. This hearing loss is usually conductive, resulting from tympanic membrane rupture and/or changes in the ossicular chain due to fixation or erosion caused by the Chronic inflammatory process. When cholesteatoma or granulation tissue is present in the middle ear cleft, the degree of ossicular destruction is even greater. An issue that has recently gained attention is additional sensorineural hearing loss due to Chronic Otitis Media. While the conductive loss can be minimized through surgery, sensorineural hearing loss constitutes a permanent after effect, attenuated only through the use of a hearing aid. However, a few groups have reported a decrease in sensorineural function in these patients as well. This survey study performed at a referral center evaluates the occurrence of sensorineural hearing loss in ambulatory patients with this disease. We reviewed the files of patients with unilateral Chronic Otitis Media. One hundred and fifty patients met the inclusion criteria: normal otoscopy and normal hearing in the contralateral ear. Main outcome measure: bone-conduction threshold averages were calculated for frequencies of 500, 1,000, 2,000, 3,000 and 4,000 Hz, with comparison between the normal ear and the ear with Chronic Otitis Media.
