Engle, David SchottenfeldAbstract:Chronic Venous disease is a common condition presenting to physicians in Western Europe and the United States. This article provides a comprehensive review of the published literature in the English language, from 1942 to the present, and focuses on the prevalence of Chronic Venous Insufficiency and varicose veins, as well as the involved risk factors. Prevalence estimates vary widely by geographic location, with the highest reported rates in Western countries. Reports of prevalence of Chronic Venous Insufficiency vary from < 1% to 40% in females and from < 1% to 17% in males. Prevalence estimates for varicose veins are higher, <1% to 73% in females and 2% to 56% in males. The reported ranges in prevalence estimations presumably reflect differences in the population distribution of risk factors, accuracy in application of diagnostic criteria, and the quality and availability of medical diagnostic and treatment resources. Established risk factors include older age, female gender, pregnancy, family history of Venous disease, obesity, and occupations associated with orthostasis. Yet, there are several factors that are not well documented, such as diet, physical activity and exogenous hormone use, which may be important in the development of Chronic Venous disease and its clinical manifestations.

Latent dystrophic subcutaneous calcification in patients with Chronic Venous Insufficiency.

To determine the association between subcutaneous calcification and Chronic Venous Insufficiency, X-ray examinations were performed in 20 patients with Chronic Venous Insufficiency and in 20 control subjects to detect latent calcification in their lower legs. Of the 20 patients, 13 (65%) had subcutaneous calcification, and the prevalence appeared to increase with disease duration, while only 4 control subjects (20%) had minimal calcification. Two types of calcification were identified based on their radiographic features: punctate and trabecular/reticular types. Patients with trabecular/reticular calcification had longer disease duration and more severe clinical scores than patients with punctate calcification. None of the control subjects had trabecular/reticular types of calcification. The identification of the presence and progression of latent calcification in the lower legs is useful, and may be necessary for the long-term management of Chronic Venous Insufficiency, since calcification of skin tissues impedes wound healing and can be a risk factor for refractory ulcers.

Chronic Venous Insufficiency: Clinical Assessment and Patient Selection

NichollsAbstract:Chronic Venous Insufficiency is a complex condition, with widely varied clinical manifestations, etiologies, and underlying pathophysiology. An orderly workup is mandatory to assess the nature of a patient's underlying Venous disease. This begins in the office setting with a careful medical history, physical examination, and bedside diagnostic tests. These are augmented by confirmatory diagnostic testing, including duplex ultrasonography, venography, plethysmography, and ambulatory Venous pressure measurement. Based upon the results of these examinations, the patient's Venous disease can be classified according to standardized classification schemes, which in turn leads to the selection of an appropriate treatment strategy. This article outlines the steps in the clinical assessment and classification of patients with Chronic Venous Insufficiency.

Chronic Venous Insufficiency and Venous leg ulceration

Venous disease has a significant impact on quality of life and work productivity. In addition, the costs associated with the long-term care of these Chronic wounds are substantial. Although the exact pathogenic steps leading from Venous hypertension to Venous ulceration remain unclear, several hypotheses have been developed to explain the development of Venous ulceration. A better understanding of the current pathophysiology of Venous ulceration has led to the development of new approaches in its management. New types of wound dressings, topical and systemic therapeutic agents, surgical modalities, bioengineered tissue, matrix materials, and growth factors are all novel therapeutic options that may be used in addition to the "gold standard," compression therapy, for Venous ulcers. This review discusses current aspects of the epidemiology, pathophysiology, clinical presentation, diagnostic assessment, and current therapeutic options for Chronic Venous Insufficiency and Venous ulceration.
