    Abstract:

    Objective To investigate the relationship between the serum homocysteine (Hcy) and type 2 diabetic peripheral neuropathy (DPN) in the elderly.Methods The level of homocysteine in serum was measured in 78 patients with type 2 diabetes mellitus and 20 normal control subjects in the elderly.The variations of Hcy level were compared among diabetic patients with or without DNP and normal control subjects.Patients with DNP were treated by Cobamamide.Results The level of serum Hcy was significantly higher in the DPN patients than in patients without DPN and the control group (P 0.05).The serum Hcy level and motor or sensory nerve conduction velocity of the DPN patients who received Cobamamide treatment were obviously improved (all P 0.05).Conclusions Hyperhomocysteinemia is an independent risk factor for diabetic neuropathy in humans.Efficiently decreasing the levels of hyper-homocysteinemia could improve the electrobiological activity.

    Objective To observe the short term effect of combined buflomedil injection and colmmamide on type 2 diabetic patients with erectile dysfunction(ED).Methods 65 diabetic patients withED under dier ang insulin control were giyen buflomedil injection 100mg.D-land Cobamamideinjection 500 ug.d-1 for 3 wk.The internatlanai index of erectile functioil 5(HEF-5) was used for self evalnation and the urine albumin dliminate rate(UAER),hepatic function' renal function,low density cholesterlo,total cholesterlo and triglyceride were determined before and after the treatment.respectively ? Results Improvements jn mean scores Of IIEF-5 and each item of IIEF-5 were significantly better in pa tients received buflomedil and Cobamamide compa ring with those Of pretreatment.The UAER decreased significantly,While the hepatic function,renal function,low density cholesterol,total cholesterol and triglyceride showed no sign ificant change and no adverse reaction were found during the study.Conclusion Combined therapy of buflomedil and cohamamide is rather effective for diabetic patients with ED.

  • A case of a novel CACNA1G mutation from a Chinese family with SCA42: A case report and literature review.

    Abstract:

    RATIONALE Spinocerebellar ataxia (SCA), a genetically inherited heterogeneous disorder, is characterized by gait ataxia, dysarthria, parkinsonism, choreic movements, dystonia, epilepsy, cognitive and psychiatric symptoms. Spinocerebellar ataxia-42 (SCA42), caused by heterozygous mutation in the calcium channel 1G (CACNA1G) gene, is a rare SCA subtype and the transmission pattern is autosomal dominant inheritance. PATIENT CONCERNS We presented a novel mutation (c.4721T>A; p.Met1574Lys) in 3 patients from a Chinese family using whole-exome sequencing. All patients exhibited cerebellar ataxia and the clinical manifestations were similar to those that were previously reported in the French and Japanese families. In addition, cerebral magnetic resonance imaging (MRI) showed cerebellar atrophy, and the hot cross bun sign of brainstem was found in the proband and her sister. DIAGNOSES The clinical features and MRI findings indicated the diagnosis of SCA. Taken together, the symptoms, MRI findings, as well as whole-exome sequencing made the diagnosis of SCA42 most likely candidate. INTERVENTIONS AND OUTCOMES The patient was treated with Cobamamide (1.5 mg once daily) for nerve nutrition and further physical therapy. At the 4-month follow-up visit, the patient's condition did not improve obviously. LESSONS Recently, a missense mutation in CACNA1G gene (c.5144G4A; p.Arg1715His) was identified in French and Japanese families with SCA42. However, there has been no report of SCA42 or its mutant loci in Chinese patients. Our finding showed a novel mutation in CACNA1G gene and provided important insights into the pathogenesis of SCA42.

    Abstract:

    A rapid, sensitive and specific method to quantify cyproheptadine in human plasma using amitriptyline as the internal standard (IS) is described. The analyte and the IS were extracted from plasma by liquid-liquid extraction using a diethyl-ether/dichloromethane (70/30; v/v) solvent. After removing and drying the organic phase, the extracts were reconstituted with a fixed volume of acetonitrile/water (50/50 v/v)+0.1% of acetic acid. The extracts were analyzed by high performance liquid chromatography coupled to electrospray tandem mass spectrometry (LC-MS/MS). Chromatography was performed isocratically using an Alltech Prevail C18 5 mu m analytical column, (150mm x 4.6mm I.D.). The method had a chromatographic run time of 4min and a linear calibration curve ranging from 0.05 to 10 ng/mL (r(2)>0.99). The limit of quantification was 0.05 ng/mL. This HPLC/MS/MS procedure was used to assess the bioequivalence of cyproheptadine in two cyproheptadine + Cobamamide (4mg+1mg) tablet formulations (Cobactin (R) [cyproheptadine + Cobamamide] test formulation supplied from Zambon Laboratorios Farmaceuticos Ltda. and Cobavital (R) from Solvay Farma (standard reference formulation)). A single 4 mg + 1mg [cyproheptadine + Cobamamide] dose of each formulation was administered to healthy volunteers. The study was conducted using an open, randomized, two-period crossover design with a 1-week washout interval. Since the 90% CI for Cmax and AUCs ratios were all within the 80-125% bioequivalence limit proposed by the US Food and Drug Administration, it was concluded that the cyproheptadine test formulation (Cobactin (R)) is bioequivalent to the Cobavital (R) formulation for both the rate and the extent of absorption of cyproheptadine.
