    Despite strong beliefs that property titling and registration will enhance credit access, empirical evidence in support of such effects remains scant. The gradual roll-out of Computerization of land registry systems across Andhra Pradesh's 387 sub-registry offices allows us to combine quarterly administrative data on credit disbursed by all commercial banks for an eleven-year period (1997-2007) aggregated to the sub-registry office level with the date of shifting registration from manual to digital. Computerization had no credit effect in rural areas but led to increased credit-supply in urban ones. A marked increase of registered urban mortgages due to Computerization supports the robustness of the result. At the same time, estimated impacts from reduction of the stamp duty are much larger, suggesting that, without further changes in the property rights system, impacts of Computerization will remain marginal.

    Background.This collaborative study, conducted by members of the Cancer Information Service Research Consortium Team for Evaluation and Audit Methods and the Network Analysis Advisory Board, is part of a larger project that evaluates the impact of communication structure on innovation within the contractual network of the Cancer Information Service (CIS). This study examines four different technological innovations with respect to the characteristics of relative advantage, compatibility, observability, complexity, trialability, adaptability, riskiness, disadvantage, computer knowledge, and acceptance. Methods.Data were gathered from self-report questionnaires completed in May 1995 by organizational members (n= 82) within the National Cancer Institute's CIS, a geographically dispersed federal government health information program. Results.Paired comparisonttests found that organizational members rate contrasting dimensions of an innovation differentially, depending on the nature of the specific technology. For example, significantly lower levels of riskiness were reported for Computerization for communication (e.g., e-mail) than for Computerization for telephone service or outreach. In addition, Computerization for office management had significantly lower levels of riskiness than Computerization for telephone service or outreach. With respect to complexity, Computerization for communication had significantly lower ratings than did outreach. In terms of observability and trialability, Computerization for communication had significantly lower ratings than for telephone service. With respect to relative advantage, Computerization for office management had significantly lower ratings than all other areas of Computerization. In terms of computer knowledge, ratings were significantly higher for communication than for all other areas of Computerization. No significant differences were found between contrasting innovations for adaptability or acceptance. Conclusions.Results suggest that organizational members rate contrasting dimensions of an innovation differentially, depending on the nature of the specific innovation.     Abstract:

    Objective: Given the slow adoption of medical informatics in Hong Kong and Asia, we sought to understand the contributory barriers and potential incentives associated with information technology implementation. Design and Measurements: A representative sample of 949 doctors (response rate = 77.0%) was asked through a postal survey to rank a list of nine barriers associated with clinical Computerization according to self-perceived importance. They ranked seven incentives or catalysts that may influence Computerization. We generated mean rank scores and used multidimensional preference analysis to explore key explanatory dimensions of these variables. A hierarchical cluster analysis was performed to identify homogenous subgroups of respondents. We further determined the relationships between the sets of barriers and incentives/catalysts collectively using canonical correlation. Results: Time costs, lack of technical support and large capital investments were the biggest barriers to Computerization, whereas improved office efficiency and better-quality care were ranked highest as potential incentives to computerize. Cost vs. noncost, physician-related vs. patient-related, and monetary vs. nonmonetary factors were the key dimensions explaining the barrier variables. Similarly, within-practice vs external and “push” vs “pull” factors accounted for the incentive variables. Four clusters were identified for barriers and three for incentives/catalysts. Canonical correlation revealed that respondents who were concerned with the costs of Computerization also perceived financial incentives and government regulation to be important incentives/catalysts toward Computerization. Those who found the potential interference with communication important also believed that the promise of improved care from Computerization to be a significant incentive. Conclusion: This study provided evidence regarding common barriers associated with clinical Computerization. Our findings also identified possible incentive strategies that may be employed to accelerate uptake of computer systems.

    Abstract:

    Abstract Objective: To identify the prevailing attitudes among physicians to use of computers in the clinical setting and specifically those attitudes that may be associated with the adoption of computers in practice. Design: A self-completed, 20-question postal questionnaire sent to 4850 randomly selected physicians. The questionnaire focused on details of the physicians' practice; actual Computerization of or intention to computerize clinical and administrative functions; attitudes towards Computerization; self-perceived computer ability and knowledge; and demographic information. The attitude statements were grouped under four themes according to a factor analysis. Results: The survey was completed by 897 physicians. Only physicians in ‘individual' practices were included in the analysis. This group was further dichotomized into clinical users (those with one or more clinical functions computerized) and non-clinical users. Non-clinical users were older and had fewer specialist qualifications. Although there was strong support for the attitude statements among both groups with regard to the benefit of Computerization to patient care, there was much less support for electronic medical records. Non-clinical users were concerned about the potentially negative impact of Computerization on the clinical encounter and the perceived high cost of Computerization. Discussion: The attitudes among current clinical users and non-users were substantially different. The most important disincentives to Computerization were the potential for interference with the patient–physician encounter and the cost of computerizing multiple practice locations. Turning these disincentives into opportunities for change remains the challenge.
