    Departament responsable de la tesi: Departament de Cirurgia.La infección por el VIH ha sido una de las epidemias que han impactado más en los últimos años en la salud pública.Una de las líneas de nuestro grupo de trabajo es la relación entre el VIH y el Papilomavirus (VPH), puesto que se ha visto que este tipo de pacientes son altamente susceptibles de sufrir una coinfección. De especial interés, son los condilomas, una forma no maligna del VPH pero que aún tiene muchos aspectos desconocidos.Nos planteamos 4 hipótesis que han sido validadas de forma consecutiva mediante diferentes publicaciones científicas con alto factor de impacto.-HIPÓTESIS 1: Los condilomas en el canal anal en personas infectadas por el VIH, especialmente entre los hombres que practican sexo con hombre, es una patología prevalente.-HIPÓTESIS 2: Las personas infectadas por el VIH con un diagnóstico de condiloma en el canal anal, presentan una coinfección por el VPH en el canal anal por más de un genotipo.-HIPÓTESIS 3: La presencia de condiloma/s en el canal anal se asocia a una alta prevalencia de lesiones escamosas intraepiteliales del canal anal en personas infectadas por el VIH. Por tanto, la coexistencia de dos patologías relacionadas con el VPH el canal anal: condiloma/s y lesiones escamosas intraepiteliales, es prevalente en personas infectadas por el VIH.-HIPÓTESIS 4: Los diferentes tratamientos usados en el manejo de los condiloma/s del canal anal en la práctica clínica presentan una alta tasa de recidiva entre las en personas infectadas por el VIH .Los artículos presentados en este trabajo dan respuesta a estas hipótesis:ARTÍCULO 1:"Condylomata, cytological abnormalities and human papillomavirus infection in the anal canal in HIV-infected men". En este artículo se evidencia que la recurrencia del condiloma después de cualquier tratamiento es común, así como el riesgo de desarrollar un cáncer anal; por lo que se trata de una población que merece un especial control.ARTÍCULO 3: "Distribution of human papillomavirus genotypes in anal cytological and histological specimens from HIV infected men who have sex with men and men who have sex with women". Este artículo puso en evidencia que es necesario un programa de cribado para los hombres VIH + independientemente de su orientación sexual. ARTÍCULO 4: "Long-term effectiveness of infrared coagulation for anal intraepithelial neoplasia 2 and 3 in HIV- infected males and females". En este artículo se evaluó la eficacia de los diferentes tratamientos actuales para el tratamiento de las lesiones provocadas por el VPH en los hombres VIH+.Los resultados de este trabajo dejan entrever un poco más de luz en un tema en el que aún quedan muchas incógnitas por conocer, como es la evolución a largo plazo de la infección por el VPH en el canal anal en hombres VIH+.In the past years, HIV-infection has been one of the epidemics that had a greater impact in our society. One of our research lines is the relation between HIV and human papilloma virus (HPV), since it has been proved that these patients are at risk of coinfection. Condylomas are of special interest, a non-malignant form of HPV infection that still has many unknown aspects. We initially set out 4 hypothesis that have been validated consecutively through different scientific publications with a high impact factor. - FIRST HYPOTHESIS: Condyloma at anal canal are hihgly prevalent in HIV-infected people, specially men who have sex with other men. - SECOND HYPOTHESIS: People infected with HIV and a diagnosis of Condyloma in the anal canal presents a coinfection for more than one HPV genotype. - THIRD HYPOTHESIS: The presence of Condyloma in the anal canal is associated with high prevalence of squamous intraepithelial lesions of the anal canal in HIV-infected people. Therefore, the existence of two pathologies related to HPV in the anal canal: Condyloma and squamous intraepithelial lesions, is prevalent in HIV-infected people. - FORTH HYPOTHESIS: The different treatments that are used in the management of anal canal Condylomas in the clinical practice present a high rate of recurrence among HIV infected people. The following articles provide evidences about our hypothesis: ARTICLE 1:"Condylomata, cytological abnormalities and human papillomavirus infection in the anal canal in HIV-infected men". This article concludes that HIV infected men with Condylomas have a higher risk of presenting anal dysplasia and they are infected with multiple gentoypes of HPV. ARTICLE 2: "Effectiveness of physically ablative and pharmacological treatments for anal Condyloma in HIV- infected men". This article shows that recurrence after any treatment of Condyloma is a common event. Thus, this population deserves a closer follow-up. ARTICLE 3: "Distribution of human papillomavirus genotypes in anal cytological and histological specimens from HIV infected men who have sex with men and men who have sex with women". This article pointed out the need of screening programs for HIV infected men regardless their sexual orientation. ARTICLE 4: "Long-term effectiveness of infrared coagulation for anal intraepithelial neoplasia 2 and 3 in HIV- infected males and females". This article evaluated the efficacy of the different treatments for anal lesions caused by HPV in HIV infected men. The results of this research line bring lights on this topic, in which there are still many questions to be answered, such as the long-term evolution of HPV infection in the anal canal in HIV infected men

    Objectives: Condyloma acuminatum are the most common sexually transmitted diseases worldwide, and they are closely associated with human papillomavirus (HPV) infection. Urethral meatus is one of the places that warts occur. Many treatments for uretheral warts have limitations. In this study, we performed 5-aminolevulinic acid-photodynamic therapy (ALA-PDT) on patients and investigated the effectiveness of reducing HPV viral loads. Materials and Methods: In our study, 21 patients diagnosed with urethral Condyloma acuminatum were included. After 4 h treatment of ALA, patients received PDT. Each patient received HPV test before every PDT cycle. The frequency of PDT was dependent on viral load changes. Results: All patients achieved complete clinical remission after the last session of ALA-PDT. There were significant differences in HPV viral loads between pretherapy and after one or three rounds of PDT treatment. Conclusions: ALA-PDT is a safe and effective method for treatment of Condyloma acuminatum in urethra meatus. Dynamic monitoring of HPV viral loads can more objectively demonstrate the effectiveness and guide the treatment of PDT.

    Abstract Background Anal canal Condyloma acuminata are common, sexually transmitted lesions, most often caused by the human papillomavirus. The relatively high recurrence rate of anal canal Condyloma acuminata can be attributed to the unsuccessful elimination of viruses in areas of subclinical and latent infection. This study aimed to observe and evaluate the effectiveness of 5-aminolevulinic acid-photodynamic therapy combined with monitoring of human papillomavirus load changes in patients with anal canal Condyloma acuminata. Methods A total of 19 patients with anal genital warts were recruited for this study. Firstly, visible warts around the anus were removed. Next, an anoscope examination was performed. Human papillomavirus detection, using real-time polymerase chain reaction assays, was performed before every cycle of treatment. Absorbent cotton rolls soaked with a concentration of 20% 5-aminolevulinic acid were inserted into the anus for 3 h. Finally, photodynamic therapy was applied to the lesions. Each patient required multiple PDT sessions to achieve complete response. Results All patients achieved complete clinical remission one week after the last session of treatment, and human papillomavirus loads decreased significantly. Six months follow-up after completion of therapy, none of the patients had recurrence. Conclusions 5-aminolevulinic acid-photodynamic therapy is an effective and safe approach for anal canal Condyloma acuminata. Dynamic human papillomavirus viral quantitative monitoring can aid in the evaluation of therapeutic effects and lead to better treatment outcomes.

    Abstract Background The effect of 5-aminolevulinic acid photodynamic therapy on keratinocyte proliferation and apoptosis in Condyloma acuminatum tissues was evaluated. Methods An immunohistochemical method and TdT-mediated dUTP nick end labeling were performed to detect the positive expression of the keratinocyte proliferation-related gene Ki-67 and apoptosis, respectively, in Condyloma acuminatum tissues. Results Of 52 cases, 44 showed positive expression of Ki-67 in Condyloma acuminatum keratinocytes before the treatment, with a positive expression rate of 84.62% (44/52), an expression strength of mostly ++ − +++, and a Ki-67 proliferation index of 80.26 ± 5.07%. After treatment, 22 cases showed positive expression of Ki-67 in Condyloma acuminatum keratinocytes, with a positive expression rate of 42.31% (22/52), an expression strength of mostly − − ++, and a proliferation index of 42.67 ± 3.06%. The differences in the positive expression rate, expression strength, and proliferation index in the before- and after-treatment groups were statistically significant (χ2 = 20.070, P  Conclusion 5-Aminolevulinic acid photodynamic therapy can inhibit the proliferation and promote the apoptosis of keratinocytes, and represents an effective mechanism for treating Condyloma acuminatum.

    BACKGROUND AND OBJECTIVES To determine the feasibility of photodynamic therapy (PDT) of vulvar and vaginal Condyloma and intraepithelial neoplasia (VIN, VAIN) and to compare PDT results with conventional treatments. STUDY DESIGN/MATERIALS AND METHODS Thirty-eight patients with vulvar or vaginal intraepithelial neoplasia (VIN) grade II/III (n = 22) or Condyloma (n = 16) had 10% 5-aminolevulinic acid (ALA)-gel applied topically. After 2-4 hours, 80-125 J/cm(2) laser light at a wavelength of 635 nm was applied. PDT was compared to conventional treatments for Condyloma (CO(2) laser evaporation) and for VIN III (laser evaporation, surgical excision). RESULTS The complete clearance rate for Condyloma treated by PDT was 66% and the rate for IN was 57% (as determined by biopsy). Of the neoplasia patients, none with hyperkeratotic VIN (n = 4) responded, and only one of four with increased pigmentation cleared. No scarring occurred, and postoperative discomfort lasted 4.9 +/- 3.4 days. Reduced disease-free survival (DFS) was associated with multifocal VIN (P = 0.02, OR 2.17, 95% CI 1.15-4.08), but DFS did not vary with treatment mode. CONCLUSIONS Although PDT is not equally efficacious for all subgroups, PDT for Condyloma and intraepithelial neoplasia appears to be as effective as conventional treatments, but with shorter healing time and excellent cosmetic results.

    Abstract OBJECTIVE: Our purpose was to determine the feasibility of selective photosensitization of vulvar Condylomas by use of topical application of 5-aminolevulinic acid. STUDY DESIGN: In vivo fluorescence was assessed and biopsy specimens of Condylomas were taken for fluorescence microscopy in 24 patients at different times after application of 2.5% 5-aminolevulinic acid ointment or 20% 5-aminolevulinic acid cream. RESULTS: Both in vivo fluorescence imaging and fluorescence microscopy showed selective fluorescence of Condylomas of the labia minora and vestibule only within short time intervals, because fluorescence of poorly keratinized normal epithelium was induced by both 5-aminolevulinic acid formulations. In non-hair-bearing skin, lesional fluorescence remained highly selective. Fluorescence microscopy showed that 90 minutes after drug application peak selectivity in epithelial lesional fluorescence was significantly higher with 2.5% 5-aminolevulinic acid ointment (4.5 ± 0.9) than it was with 20% cream (2.1 ± 0.2). CONCLUSIONS: Selective fluorescence of vulvar Condyloma acuminatum can be induced by nonselective topical 5-aminolevulinic acid application. Studies evaluating selective photodynamic destruction of Condylomas are justified. (AM J OBSTET GYNECOL 1996;174:951-7.)
