Fetal heart rate tracing patterns associated with Congenital Hypothyroidism.

FHR patterns of 59 women whose babies were diagnosed with Congenital Hypothyroidism were retrospectively compared with tracings of 78 of their siblings. Tracings were interpreted during the first stage of labor. Multivariable analysis was used to control for confounders. Neonates with Congenital Hypothyroidism had significantly higher rates of reduced variability in FHR tracing patterns as compared with those without Congenital Hypothyroidism (49.2% vs 3.8%; odds ratio, 24.1; 95% confidence interval, 6.8-85.3; P < .001). No significant differences were noted between the groups regarding decelerations or baseline abnormalities. The significant association between Congenital Hypothyroidism and reduced variability persisted after controlling for confounders such as treatment with pethidine, MgSO4, and gestational age (odds ratio, 14.1; 95% confidence interval, 1.1-190.7; P = .046). Congenital Hypothyroidism is significantly and independently associated with reduced variability in FHR tracing patterns.

Prevalence of permanent primary Congenital Hypothyroidism in Taiwan.

Chao, Li-yang Chen, Sung-jan Lin, Ju-yu ChenAbstract:The purpose of this study was to find the prevalence of permanent primary Congenital Hypothyroidism in Taiwan. From January 1988 to December 1990, there were 991,132 live births in Taiwan. Of these, 329,891 neonates were screened for primary Congenital Hypothyroidism. Fifty-seven cases of permanent primary Congenital Hypothyroidism were confirmed. Hence, the prevalence of permanent primary Congenital Hypothyroidism in that period in Taiwan was 1 in every 5,788 live births. There was a female preponderance with a female to male ratio of 1.7. Of 54 infants who had a thyroid scan, 36 were found to have an ectopic thyroid gland, while 11 were found to have dyshormonogenesis. Among the 57 infants, one case with a delayed rise in the serum thyrotropin level was missed on initial screening. When compared with other studies, these data suggest that the prevalence of permanent primary Congenital Hypothyroidism varies in different ethnic groups throughout the world.

Congenital Hypothyroidism in Taiwan: experience before mass screening.
Journal of the Formosan Medical Association = Taiwan yi zhi, 1992Co-Authors: Wen-yu TsaiAbstract:The clinical data of 91 patients with Congenital Hypothyroidism, not detected by neonatal screening, were reviewed. Our results disclosed that Congenital Hypothyroidism in Taiwan is more common in girls than boys, with a female to male ratio of 1.8:1. The majority of our patients were from a first pregnancy. Only 4.8% of our patients had a history of prolonged gestation, and the median value of birth weight in these patients was 3,300 g. Both of these data are comparable to those of the normal population. Short stature, constipation, dry skin and periorbital edema were the most common symptoms and signs. An ectopic thyroid gland was the most common type of Congenital Hypothyroidism in our series. The conditions of most of our patients were detected after the age of three months; poor mental prognosis was expected. However, it is hoped that neonatal screening programs for Congenital Hypothyroidism in Taiwan can improve such situations.
