    Abstract Conidiobolomycosis is an emerging entomophthoramycosis caused by fungi Conidiobolus spp. Animal models are essential for the study of infectious disease in various areas such as pathogenesis, diagnostic methods, treatment and prevention. There is not currently an animal model for Conidiobolomycosis. The aim of this study was to create an experimental infection protocol for Conidiobolus lamprauges in gerbils ( Meriones unguiculatus ). The study animals were randomly divided into four groups of four animals: immunosuppressed with cyclophosphamide (CPA) and infected with C. lamprauges (G1), immunocompetent and infected with C. lamprauges (G2), immunosuppressed with CPA (G3), and an immunocompetent control group (G4). Clinical signs were observed only in G1 animals, where the mortality rate reached 75% by day 7 after infection (AI) with a median survival of 2 days. C. lamprauges was detected only in G1, both by PCR and by isolation. Necropsies of the G1 animals showed lesions in the nasal cavity and lung tissue. These lesions were characterized by polymorphonuclear infiltrate cells and by the presence of hyphal structures under silver staining. This animal model will be useful for further investigation of diseases caused by C. lamprauges , particularly of those associated with immunosuppression factors in naturally occurring animal infections.

    Abstract:

    The present work describes the occurrence of two Conidiobolomycosis outbreaks in Santa Ines breedsheep, in the state of Bahia. The first outbreak (FO) occurred in a property located in Santo Estevao city,involving three animals, and the second outbreak (SO) occurred in a property in Entre Rios city, affecting one  sheep. In both outbreaks, the clinical findings were characterized by progressive weight loss, dyspnea, andbloody serous nasal discharge. In the FO was also observed unilateral craniofacial asymmetry, associatedwith exophthalmia and epiphora of the right eye. Autopsies performed on the three FO sheep showeda large mass predominantly yellowish-white with red-black areas of the uneven surface, finely granular,moist and crumbly located in the ethmoidal region, invading the nasopharyngeal, choana, sinuses, retrobulbar region, who infiltrated the cribriform plate and reached the meninges. In two sheep of the FO, themass reached the front portion of the brain. In SO sheep it was found, on the mucous membrane of thenasal vestibule, small red-black multifocal to coalescing nodules, with eroded surface and hyperemiaof the nasal turbinates with hemorrhagic catarrhal secretion, and in the most rostral portion of nasalcavity, the mucosa had a irregular, wet, bleeding and red-black surface, with blackish gray and brownishareas, displaying partial focal destruction of nasal turbinates. Microscopic examination of granulomatouslesions revealed multiple areas of coagulation necrosis containing intense reaction of Splendore-Hoeppliassociated with negative images of fungal hyphae and granulomatous inflammation. In all cases, theGMS and PAS staining showed numerous wide hyphae, with bulbous distension at one end. By PCRtechnique it was characterized Conidiobolus lamprauges by analysis of serum samples of two sheep andparaffin block lesions of one sheep. Immunohistochemical analyzes were positive for C. lamprauges inall animals. It can be proved, for the first time, the occurrence of nasopharyngeal / rhinocerebral andrhinofacial Conidiobolomycosis infection by C. lamprauges in the state of Bahia.

    Abstract Conidiobolomycosis is a fungal disease that affects the upper respiratory tract of animal mainly sheep in tropical region. There are no vaccines or efficient treatments for this pathology and identifying immunoreactive proteins could create new opportunities for preventing or diagnosing this disease. The objectives of this study were to characterize the Conidiobolus lamprauges enolase-gene (eno) and evaluate its immunogenic capacity in naturally infected sheep serum. Gene amplification of eno gene from C. lamprauges resulted in a 1305 bp amplicon and 434 residues of deduced amino acid with a predicted molecular weight of 47.2 kDa and it had a signature enolase sequence. Anti-recombinant enolase IgG showed 80% of sensibility and 81.8% of specifity, with no cross-reactivity detected in sheep with pythiosis. Therefore, this study demonstrated that recombinant ENO from C. lamprauges is immunogenic against the sera of sheep that were naturally infected with Conidiobolomycosis. The detection of specific anti-enolase antibodies in animals shows that the protein is a promising candidate antigen for the development of effective vaccines against the disease or diagnostic tests.

    Conidiobolomycosis is an emerging disease caused by fungi of the cosmopolitan genus Conidiobolus . Particular strains of Conidiobolus coronatus, Conidiobolus incongruus and Conidiobolus lamprauges , mainly from tropical or sub-tropical origin, cause the mycosis in humans and animals, domestic or wild. Lesions are usually granulomatous and necrotic in character, presenting two clinical forms: rhinofacial and nasopharyngeal. This review includes the main features of the disease in sheep, with an emphasis on the epidemiology, clinical aspects, and diagnosis of infections caused by Conidiobolus spp. in Brazil. In this country, the disease is endemic in the Northeast and Midwest, affecting predominantly woolless sheep breeds and occasioning death in the majority of the studied cases. The species responsible for infections of sheep are C. coronatus and C. lamprauges and the predominant clinical presentation is nasopharyngeal. These fungal infections are very important, since they compromise the health status of the sheep flock and cause serious economic losses to the sheep industry. Thus, research is needed to investigate faster tools for diagnosis and effective methods for the control and treatment of Conidiobolomycosis.

    Conidiobolomycosis is a granulomatous disease caused by the fungus Conidiobolus spp. in humans and animals. Traditional technique for diagnosis of the disease is isolation of the agent associated with the presence of typical clinical signs and pathological conditions. The aim of this study was to describe the development of a specific polymerase chain reaction (PCR) test for Conidiobolus lamprauges to detect the fungus in clinical samples. Samples from suspected animals were collected and submitted to isolation, histopathological analysis and amplification by PCR. DNA from tissues was subjected to PCR with fungi universal primers 18S rDNA gene, and specific primers were designed based on the same gene in C. lamprauges that generated products of about 540 bp and 222 bp respectively. The culture was positive in 26.6% of clinical samples. The PCR technique for C. lamprauges showed amplification of DNA from fresh tissues (80%) and paraffin sections (44.4%). In conclusion, the PCR technique described here demonstrated a high sensitivity and specificity for detection of fungal DNA in tissue samples, providing a tool for the rapid diagnosis of C. lamprauges.

    : We have determined the in vitro activity of antifungal, antibacterial, and antiprotozoal drugs alone and in combination against seven Conidiobolus lamprauges clinical isolates. The assays were based on the M38-A2 protocol and the checkerboard microdilution method. The lowest inhibitory concentrations were observed for amphotericin B, miconazole (MCZ), terbinafine, and miltefosine (MTF) (MIC range 0.25-1; 2-8; 0.25-2; 2-16 μg/ml, respectively). The main synergism observed was through the combination of azithromycin (AZI)+MTF and dapsone (DAP)+MTF (100%), AZI+DAP (85.7%), AZI+MCZ (57.1%) as well as MCZ plus CTX and DAP (42.9%). The in vitro activities suggest that the combination of MTF and AZI or DAP are promising candidate therapies for Conidiobolomycosis.

    : Data regarding the susceptibility of Conidiobolus lamprauges is limited and there is no consensus about the optimal treatment for infections caused by Conidiobolus spp. In this context, the objective of this study was to evaluate the in vitro susceptibility of six C. lamprauges strains isolated from sheep Conidiobolomycosis to amphotericin B, ketoconazole, fluconazole, itraconazole, posaconazole, voriconazole, anidulafungin, caspofungin, micafungin, flucytosine, and terbinafine using the CLSI M38-A2 microdilution technique. Terbinafine was the most active (MIC range <0.06-0.5 μg/mL). Resistance or reduced susceptibility was observed for amphotericin B and azole and echinocandin antifungals. Additional studies are necessary to determine the therapeutic potential of terbinafine as monotherapy or in combination therapy with other antifungals.

    Conidiobolus lamprauges is a pathogen zygomycetes fungi of humans and animals, responsible for Conidiobolomycosis, which is characterized by a severe granulomatous chronic rhinosinusitis. The ability to adapt and grow at high temperatures is suggested as an attribute of virulence in fungi that infect animals and humans, however regarding C. lamprauges little information is available about this aspect. This paper aims to identify differential expression genes in C. lamprauges grown at 30°C and 37°C through the technique of Representational Difference Analysis (RDA). After the analysis and sequencing of a set of 120cDNAs, it was identified enolase, a glycolytic enzyme, differentially expressed at 37°C. This gene performs functions related to pathogenicity during host-pathogen interaction process in several pathogenic microorganisms. showing a potential involvement in host-pathogen relationship, and virulence in C. lamprauges.

    Conidiobolus lamprauges é um fungo zigomiceto patógeno de humanos e animais, causador da conidiobolomicose, caracterizada por uma rinossinusite crônica granulomatosa severa. A capacidade de se adaptar e crescer a altas temperaturas são sugeridos como um atributo de virulência em fungos que infectam animais e humanos, no entanto, em C. lamprauges, pouca informação é disponível sobre esse aspecto. O objetivo deste trabalho foi identificar genes com expressão diferencial em C. lamprauges cultivado a 30° e 37°C através da técnica de Análise de Diferença Representacional (RDA). Após análise e sequenciamento de um conjunto de 120cDNAs, identificou-se uma enzima glicolítica denominada enolase, diferencialmente expressa a 37°C.     La entomoftoromicosis es un tipo de micosis subcutanea en el cual se incluyen la basidiobolomicosis y la conidiobolomicosis; esta ultima es causada principalmente por Conidiobolus coronatus , un hongo saprobio que habita el suelo en paises tropicales y genera una afeccion rinofacial que compromete los tejidos blandos de la cara, los senos paranasales y la orofaringe, y tiene la capacidad de deformar el rostro en personas sin inmunodeficiencia aparente. La infeccion tiene un curso cronico con tendencia a la formacion de granulomas que se pueden observar en el estudio histopatologico. Se presenta el caso de un agricultor de 28 anos con un cuadro clinico de tumefaccion rinofacial, obstruccion nasal y escurrimiento posterior, de seis meses de evolucion, a quien se le diagnostico conidiobolomicosis a partir de un cultivo de tejido despues de multiples biopsias de tejidos del area facial. El paciente recibio tratamiento antifungico con anfotericina B y posteriomente con itraconazol, con lo cual su mejoria fue notable, por lo que no requirio tratamiento quirurgico. El tratamiento con itraconazol se extendio por un ano, al cabo del cual no habia evidencia de recaida. Dada la poca frecuencia de esta enfermedad, no hay una estrategia de tratamiento establecida; sin embargo, el uso de azoles, como el itraconazol, se senala cada vez mas en los diferentes reportes de caso, haya habido tratamiento quirurgico adyuvante o no. En este reporte de caso se describe, ademas, la respuesta clinica y terapeutica a largo plazo de esta micosis infrecuente en Colombia.

    Abstract:

    Entomophtoramycosis is a type of subcutaneous mycosis which includes both basidiobolomycosis and Conidiobolomycosis; the latter is caused by Conidiobolus coronatus, a saprophytic fungus which lives in tropical soils. This mycosis characteristically affects the paranasal sinuses and oropharynx, with the potential to deform the face in patients without apparent immunodeficiency. It has a chronic course of infection with a tendency to form granulomas visible using histology. We present the case of a 28 year-old male agricultural worker, with a clinical profile of 6 months' evolution of rhinofacial tumefaction, nasal obstruction and post-nasal drip who was diagnosed with Conidiobolomycosis by means of tissue culture after multiple biopsies of the facial area. The patient received antifungal treatment with amphotericin B and subsequently with itraconazol, resulting in a dramatic improvement without the need for surgical treatment; itraconazol was administered for one year and there was no evidence of relapse at the end of this period. Due to the low frequency of this disease there is no established treatment strategy; however, the use of azoles such as itraconazol with or without adjuvant surgical treatment is increasingly seen in case reports. The present report adds to the clinical experience in Colombia of this rare mycosis and also describes the long-term clinical and therapeutic response.

    Abstract:

    Entomophtoramycosis is a kind of subcutaneous mycosis, include both basidiobolomycosis and Conidiobolomycosis; last one is caused by Conidiobolus coronatus a saprophytic fungus that inhabits of the soil of the tropical countries. Typically this mycosis involves rhinofacial, soft tissues of the face, sinuses and oropharynx, with a potential capacity of deforming. Usually it’s happening in immunocompetent patients and it has a chronic course and in the histology granulomas are shaped. We report the case of a 28-year-old farmer with a clinical picture of 6 months of evolution of rhinofacial swelling, nasal obstruction and subsequent runoff; Diagnosis of conodiobolomycoses was made through tissue culture after multiple biopsies of the facial area. He received antifungal treatment with amphotericin B and after were changed by itraconazole with dramatic improvement without requiring surgery, treatment was extended for one year and at the end there was no evidence of relapse. There is no clear strategy of treatment because of the low frequency of this disease, however the use of azoles such as itraconazole is increasing with or without adjuvant surgical treatment. This case report is attached to the clinical experience in Colombia about this rare mycosis and describes the clinical and therapeutic response to long time.

    : We have determined the in vitro activity of antifungal, antibacterial, and antiprotozoal drugs alone and in combination against seven Conidiobolus lamprauges clinical isolates. The assays were based on the M38-A2 protocol and the checkerboard microdilution method. The lowest inhibitory concentrations were observed for amphotericin B, miconazole (MCZ), terbinafine, and miltefosine (MTF) (MIC range 0.25-1; 2-8; 0.25-2; 2-16 μg/ml, respectively). The main synergism observed was through the combination of azithromycin (AZI)+MTF and dapsone (DAP)+MTF (100%), AZI+DAP (85.7%), AZI+MCZ (57.1%) as well as MCZ plus CTX and DAP (42.9%). The in vitro activities suggest that the combination of MTF and AZI or DAP are promising candidate therapies for Conidiobolomycosis.

    Conidiobolomycosis is an emerging disease caused by fungi of the cosmopolitan genus Conidiobolus . Particular strains of Conidiobolus coronatus, Conidiobolus incongruus and Conidiobolus lamprauges , mainly from tropical or sub-tropical origin, cause the mycosis in humans and animals, domestic or wild. Lesions are usually granulomatous and necrotic in character, presenting two clinical forms: rhinofacial and nasopharyngeal. This review includes the main features of the disease in sheep, with an emphasis on the epidemiology, clinical aspects, and diagnosis of infections caused by Conidiobolus spp. in Brazil. In this country, the disease is endemic in the Northeast and Midwest, affecting predominantly woolless sheep breeds and occasioning death in the majority of the studied cases. The species responsible for infections of sheep are C. coronatus and C. lamprauges and the predominant clinical presentation is nasopharyngeal. These fungal infections are very important, since they compromise the health status of the sheep flock and cause serious economic losses to the sheep industry. Thus, research is needed to investigate faster tools for diagnosis and effective methods for the control and treatment of Conidiobolomycosis.

    : Data regarding the susceptibility of Conidiobolus lamprauges is limited and there is no consensus about the optimal treatment for infections caused by Conidiobolus spp. In this context, the objective of this study was to evaluate the in vitro susceptibility of six C. lamprauges strains isolated from sheep Conidiobolomycosis to amphotericin B, ketoconazole, fluconazole, itraconazole, posaconazole, voriconazole, anidulafungin, caspofungin, micafungin, flucytosine, and terbinafine using the CLSI M38-A2 microdilution technique. Terbinafine was the most active (MIC range <0.06-0.5 μg/mL). Resistance or reduced susceptibility was observed for amphotericin B and azole and echinocandin antifungals. Additional studies are necessary to determine the therapeutic potential of terbinafine as monotherapy or in combination therapy with other antifungals.
