    Background: The purpose of this study was to evaluate the feasibility of temporal Contrast Sensitivity Testing using full-field flicker stimulation in patients after penetrating keratoplasty (PK) and to assess whether this method is influenced by postoperative corneal topographic changes. Methods: Forty-five patients (age 46.5±14.2, median 47 years) who had undergone PK and 194 age-matched controls were included in this study. The postoperative interval was 11.8±10.2 months (median 9 months). Patients with pre-existing glaucoma or any postoperative intraocular pressure elevation were excluded. The indications for PK were keratoconus in 54% of cases, Fuchs' dystrophy in 38% and stromal dystrophies in 8%. Temporal Contrast Sensitivity was determined with sinusoidal flickering light (37.1 Hz) of constant mean photopic luminance (10 cd/m^2) presented in a full-field bowl with an increasing threshold strategy. Results: Mean temporal Contrast Sensitivity did not differ between patients after PK (1.49±0.13, range 1.26–1.78, confidence interval 1.45–1.53) and controls (1.55±0.17, range 1.16–1.98, confidence interval 1.47–1.51). No significant correlation between temporal Contrast Sensitivity and visual acuity could be found in patients after PK or in normals ( r

    Purpose: Determination of lens opacity is important in aging glaucoma patients, because sensory Testing may be influenced by increasing lens opacity. Purpose of this study was to measure lens opacity in normals and glaucoma patients using two different techniques, to compare both methods, and to correlate the results with Contrast Sensitivity Testing. Methods: 94 glaucomatous eyes, 47 ocular hypertensive eyes, and 79 control eyes were studied using the following devices: opacity lens meter (OLM, objective back scatter method), stray light meter (SLM, subjective forward scatter using flicker light), and temporal Contrast Sensitivity (full-field flicker test). SLM results were separately analyzed at the lower and upper flicker disappearance. All subjects were younger than 66 years and had no significant cataracts.     : The purpose of this study was to test the usefulness of the scanning laser ophthalmoscope (SLO) as a stimulator for localized Contrast-Sensitivity (CS) measurements under visual control in normals and glaucomatous patients. The helium-neon laser (630 nm) of a Rodenstock SLO was used to generate a localized square-wave stripe pattern and a fixation mark while an infrared laser (780 nm) visualized the eye's fundus. Thus, measurements with unstable fixation could be discarded. The spatio-temporal CS was determined with a localized alternating pattern as follows: 5 Hz, 2.88 cycles/degree; stimulus diameter 2 degrees; test location central, temporal upper, and temporal lower retinal area. The subjects included 47 controls, 23 ocular hypertensive patients (intraocular pressure > 21 mmHg, normal optic disc, no visual field defect), 27 "preperimetric" glaucoma patients (intraocular pressure > 21 mmHg, early glaucomatous optic disc damage, no visual field defect), and 41 "perimetric" glaucoma patients with glaucomatous defects of the optic disc and perimetric losses. Nonparametric tests (Wilcoxon, Spearman) were used for statistical analyses. Among the three locations tested, CS values recorded for the temporal lower retinal areas showed the strongest reduction in glaucomas ("preperimetric" P = 0.002, "perimetric" P < 0.0001) and a significant correlation with the perimetric mean defect of the stimulated area (r = 0.65, P < 0.001). In all, 95% of glaucomatous eyes with extensive visual field losses were recognized as pathologic at a specificity of 80%. Spatiotemporal CS of temporal retinal areas is sensitive to glaucoma. The present study confirms the feasibility of using the SLO system for localized CS measurements, in normals and patients.
