    Abstract:

    OBJECTIVE: To evaluate the efficacy of specific immunotherapy (SIT) with standardized Dermatophagoides pteronyssinus extract for allergic rhinitis (AR)accompanied with asthma. METHOD: One hundred and fifty-five patients (40 AR with asthma, AR & asthma) in accordance with the inclusion criteria of SIT, were allocated to receive standardized Dermatophagoides pteronyssinus extract (SIT group, n = 89) or medical treatment (control group, n = 66). AR with or without asthma was observed separately. Symptom and medicine scores, quality of life were recorded and analyzed before and after 1 year treatment. Side effects were registered. Subjective evaluation of symptoms was made by the patients. RESULT: Rhinitis and asthma symptom scores, medicine scores and quality of life were greatly improved in SIT group of AR & asthma after 1 year, which were not significant changed in control group except for medicine scores. The subjective evaluation of symptoms was also significantly improved in SIT group. In patients of AR without asthma, the symptom scores, medicine scores and quality of life were both improved. The SIT group improved greater than that of control group. CONCLUSION: The clinical efficacy of patients with AR & asthma was not good with simple medical treatment, while great clinical efficacy could be acquired with SIT.

    Abstract The cDNA encoding an allergen from the dust mite Dermatophagoides pteronyssinus has been cloned and sequenced. The allergen (Der p 10) is a tropomyosin that shared more than 65% identical residues with other invertebrate tropomyosins. The final recovery of recombinant Der p 10 from the culture media after a single purification step was as much as 26 mg/l. The recombinant allergen is reactive to shrimp antitropomyosin IgG antibodies and has a 5.6% frequency of IgE reactivity in sera from mite-allergic patients.

    Abstract:

    : Two of the WHO First International Standards (IS) of allergenic extracts, the corresponding to Dermatophagoides pteronyssinus (House dust mite) and phleum pratense (Thymothy grass), have been used to assess the quality of our in-house reference preparations (IHR) and to calibrate their relative potency. The qualitative comparison was achieved by SDS-PAGE, Agarose-IEF and their respective enzime immunoblottings (Western blot and immunoprinting). The band patterns obtained were fairly similar for both Phleum pratense extracts. With the exception of two basic IgE binding proteins not detected in the IEF-blotting of the International Standard. However, several differences were noted between IS and IHR Dermatophagoides pteronyssinus extracts, indicating so the higher raw material heterogeneity. The difference was specially remarkable at the level of SDS-PAGE Immunoblotting since while our IHR extract showed similar ratio of Der p I to Der p II major allergens, the IS exhibited a large preponderance of Der p I, with Der p II allergen scarcely detected by our chromogenic method. Calibration of potency was done by means of RAST inhibition, therefore allowing the expression of our extract concentrations in International Units. The higher degree of homogeneity between Phleum pratense extracts was corroborated by the comparison of slopes from the RAST-inhibition plots.

    BACKGROUND: We undertook this study to test the possible correlation between serum concentration of total and specific IgE antibodies and asthma severity in asthmatic children sensitized to the house dust mite Dermatophagoides pteronyssinus. METHODS: The study included 157 asthmatic children aged 5-15 years (8 +/- 3 years). Clinical diagnosis was based on personal and family history, physical examination, pulmonary function testing and skin tests. Asthma severity was determined according to GINA guidelines. In vitro tests included serum concentration of total and specific IgE. RESULTS: All asthmatic children had elevated serum concentration of total IgE. The children with elevated serum concentration of total IgE also showed an increased serum concentration of specific IgE. Asthma of higher higher severity was present in patients with total IgE concentration >288.0 kIU/L (AUC = 0.736) and specific IgE to Dermatophagoides pteronyssinus >44.1 kIUA/L (AUC = 0.843). Intermittent asthma was present in 76.9% of children with specific IgE concentration 44.1 kIUA/L would have a more severe form of asthma. CONCLUSIONS: Asthmatic children with higher asthma severity have a higher serum concentration of both total IgE (>288.0 kIU/L) and specific IgE to Dermatophagoides pteronyssinus (>44.1 kIUA/L), respectively.
