innervated reverse island flap based on the end dorsal branch of the Digital Artery surgical technique
Journal of Hand Surgery (European Volume), 2005Co-Authors: Yanfeng LiAbstract:Fingertip or pulp loss of the fingers is observed frequently in unskilled workers. To reconstruct a sensate fingertip or pulp we designed the innervated reverse island flap based on the end dorsal branch of the Digital Artery, which was harvested from the dorsum of the middle phalanx. The sensation of the fingertip or pulp was re-established through coaptation of the proper branches of the Digital dorsal nerves to the Digital nerves. Three fingertip or pulp defects were reconstructed with this technique. All patients achieved satisfactory functional and cosmetic results. The mean follow-up time was 7.7 months. The average size of the flaps was 1.6 × 1.8 cm. The average static 2-point discrimination and moving 2-point discrimination of the flaps were 4.6 mm and 3.0 mm, respectively. The technique we applied seems to be an excellent option for 1-stage reconstruction of fingertip or pulp defects.

An idiopathic true aneurysm of the radial Digital Artery of the small finger is described, with its clinical presentation and corresponding arteriogram. Microsurgical repair with an interposition vein graft was performed to re-establish the vascular anatomy. To our knowledge this is the first such reconstruction reported.

Repairing the Soft Tissue Defect of Fingertip with A Pedical Reverse Island Flaps of Dorsal Digital Artery

The microsu rgical dissection of vessels at dorsal metacarpal Artery and dorsal Digital arte ry were studied. The dorsal metacarpal Artery anastomosed with the palmar Artery at the level of the first phalanx neck,the dorsal Digital Artery was the last part of the dorsal metacarpal Artery. There were a lot of vascular collaterals b etween dorsal Digital Artery and palmar Artery. The reverse island flaps with th e pedicel of dorsal Digital Artery was designed according to the distribution of vessels. From June 2002 to August 2004, 16 cases of soft tissue defect of the f ingertip were repaired with this kind of flap. All 16 flaps survived well withi n 2 to 16 months follow-up. The subcutaneous dorsal Digital Artery can be desig ned as pedicel of reverse island flaps, it is practicable and valuable in repair of the soft tissue defect of the fingertip.

Conclusions: The cutaneous dorsal Digital Artery can be designed as pedicel of reverse island flaps, which is practicable and valuable in repairing of the soft tissue loss of the fingertip.
