    Abstract:

    Objective To study the X-ray feature of congenital Duodenum stenosis complicated by small intestine malrotation and evaluate the value of X-ray examination to congenital Duodenum Obstruction.Methods The X-ray feature of congenital Duodenum stenosis complicated by small intestine malrotation which confirmed by surgery were analyzed retrospectively and summarized,abdominal plain film,radiography with iodine solution of upper digestive tract and barium enema were performed in all patients.Results On the plain film,double bubble or tri-bubble were found in all cases.Dilatation of stomach,too much leftover in stomach,Duodenum dilataltion above stensosis and reversed peristalsis were noticed in opacification of upper digestive tract with meglumine diatrizoate.Lune prolapse were noticed in cases of membranous stenosis.Little meglumine diatrizoate reach to other small intestine and colon,and spend much time.The filament of meglumine diatrizoate were noticed slowly go through the stenosis bowel.Conclusions Congenital Duodenum stenosis is the cause of congential Duodenum Obstruction,which complicated with small intestine malrotation or other malformations.Radiography and contrast examination can offer important value to clinical diagnosis.

    【Objective】To summarize in clinical manifestation,operative method and peri-operative management of Congenital Duodenum Obstruction.【Methods】54 cases of Congenital Duodenum Obstruction who's age from 1 hour to 10 months were analyzed retrospectively.14 cases in Membrane Atresia of Duodenum,3 cases in other type of Duodenum Obstruction,3 cases in abnormal zone around of Duodenum,22 cases in Intestinal Malrotation,12 cases in Annular Pancreas.31 cases were cured by the Ladd's operation,13 cases by anterior wall of Duodenum diamond anastomosis,14 cases by membrane resection,2 cases by Duodenum-Duodenum anastomosis,3 cases by adhesion lysis.【Results】1 case in Intestinal Malrotation complicated with Volvulus and extensive intestinal necrosis automatic discharged after operation,1 case abandoned treatment for extensive intestinal adhesion and without operation,52 cases were cured and left hospital.【Conclusion】Congenital Duodenum Obstruction is common diseases in neonatal.Main causes were intestinal malrotation,membrane atresia of Duodenum and annular pancreas.The key of improving curing rate are early diagnosis,reasonable selection of operative methods,avoiding missed diagnosis and strengthening peri-operative treatment.

    ABSTRACT The macroscopic, biochemical, and cytological characteristics of peritoneal fluid of equines submitted to intestinal Obstruction using an experimental model were evaluated. Twenty-four animals were distributed in four groups: instrumented control (GI), Duodenum Obstruction (GII), ileum Obstruction (GIII), and large colon Obstruction (GIV). Peritoneal fluid samples were collected before the surgery (T0), during the Obstruction (T60i-T180i), and after unblocking procedures (T60ri-T7 o ). During obstructive period, significant alterations were not observed in biochemical and cytological examination of peritoneal fluid of all animals. After unblocking procedure, animals from GII and GIII presented intense inflammatory response characterized by higher global and differential leukocytes counts, as well as in fibrinogen and total protein concentrations; lactate and inorganic phosphorus concentrations in peritoneal fluid were also increased. The laboratory alterations were not associated with clinical signs, indicative of the abdominal inflammatory process presence. Results showed that analyses of peritoneal fluids can be used as support in the evolution of healing process. However, they can not be isolated used for a diagnosis and prognosis of equine postoperative complications. Keywords: equine, peritoneal fluid, colic, peritonitis

    : Intestinal malrotation is a congenital abnormality and is rarely seen in the adulthood. Most adult cases would be classified to the non-rotation type with Ladd's band and Ladd procedure is the treatment of choice. A 66 year-old woman admitted to our hospital due to Duodenum Obstruction. Several tests revealed that she had intestinal malrotation previously undiagnosed. Operative findings showed the fusion of Duodenum with jejunum by the incomplete Treitz ligament. There was no Ladd's band and the right colon was unfixed. Dissection of the fusion completely released her symptom and she discharged without any complication. This is the first report of untypically intestinal malrotation in the adulthood without Ladd's band.
