Similarly, AADs can affect ICD rhythm discrimination algorithms that depend on atrio-ventricular ratios. There is concern whether these drugs remain effective during stress situations.

This article documents the termination of a pacemaker Endless Loop tachycardia by a critically timed atrial extrasystole. Although predictable electrophysiologically, this mode of termination has not been previously reported. The observation is conceptually important because it provides the final link in establishing the similarity of Endless Loop tachycardia and spontaneous reentrant AV nodal or junctional tachycardias in terms of initiation and termination by single atrial or ventricular extrasystoles.

This report describes the induction of a far-field Endless Loop tachycardia by hyperkalemia in a patient with severe heart disease. Ventricular activation was so prolonged that the atrial channel sensed the far-field ventricular deflection within the atrial electrogram beyond the 300 ms postventricular atrial refractory period.

Pacing and Clinical Electrophysiology, 1997Co-Authors: Serge S BaroldAbstract:: During dual chamber pacing, repetitive ventriculoatrial (VA) synchrony secondary to retrograde conduction can occur whenever otherwise effectual atrial stimuli fall continually within the atrial myocardial refractory period generated by preceding unsensed retrograde atrial depolarization. The latter falls within the pacemaker atrial refractory period and cannot initiate Endless Loop tachycardia, another form of repetitive VA synchrony secondary to a reentrant process through the pacemaker and the heart. This report describes the usefulness of Holter recordings in the diagnosis of repetitive nonreentrant VA synchrony and documents initiation and termination of this process by a ventricular extrasystole.

Pacemaker Repetitive Nonreentrant Ventriculoatrial Synchronous Rhythm. A Review

LevineAbstract:Ventriculoatrial (VA) synchrony during dual chamber pacing can occur in any patient who has the ability to sustain repeated retrograde conduction. If the retrograde P wave is sensed, the result will be an Endless Loop tachycardia or repetitive reentrant VA synchrony. VA synchrony can also occur when a dual chamber pacemaker does not sense a retrograde P wave within the postventricular atrial refractory period. In this situation if the normally suprathreshold atrial stimulus at the end of the atrial escape interval is continually delivered when the atrial myocardium is physiologically refractory, the result will be a repetitive nonreentrant VA synchronous rhythm. Repetitive nonreentrant VA synchrony may produce unfavorable hemodynamic consequences and the pacemaker syndrome. It represents an example of functional atrial undersensing combined with functional loss of atrial capture. Management requires modification of the programmed settings of the pacemaker and utilization of certain algorithms designed for other functions but nevertheless effective in this situation.

Cross-ventricular Endless Loop tachycardia during biventricular pacing.

ByrdAbstract:This report describes the development of an unusual pacemaker tachycardia in a patient with a conventional dual chamber pacemaker used for biventricular pacing in the VVIR mode. The atrial channel was connected to the left ventricle and the ventricular channel to the right ventricle. The tachycardia was sustained by sensing of the T wave by the "atrial channel" which triggered pacing by the "ventricular channel. " This "cross-ventricular" Endless Loop tachycardia is a reentrant pacemaker tachycardia like classic Endless Loop tachycardia and can be prevented by appropriate programming of the pacemaker.

Der Endless-Loop-Rekorder im klinischen Alltag –

SeidlAbstract:Die Synkope ist ein häufiges Problem des klinischen Alltags, das trotz ausführlicher nicht-invasiver und invasiver Diagnostik bei 13–48% der Patienten ungeklärt bleibt. Seit einigen Jahren ist ein implantierbarer Loop-Rekorder (ILR, Reveal®, Medtronic Inc.) verfügbar, mit dem ein Langzeit-EKG-Monitoring über mindestens 14 Monate möglich ist. Das Aggregat wird subkutan implantiert und über zwei Sensing-Elektroden wird ein kontinuierliches Ein-Kanal-EKG registriert. Im Rahmen des Reveal®-Registers wurde der ILR bei 149 Patienten mit rezidivierenden Synkopen und/oder Präsynkopen implantiert, bei denen die vorherige nicht-invasive und invasive Synkopendiagnostik keinen pathologischen Befund ergeben hatte. Die mittlere Nachbeobachtungsdauer betrug 8,6±6,9 Monate. Während dieser Zeit trat bei 59Patienten (40%) ein Rezidiv auf, die übrigen 90 Patienten (60%) blieben asymptomatisch. Von 3 Patienten wurde der ILR nicht aktiviert, bei allen 56 Patienten, die den ILR aktivierten, konnte aufgrund einer stabilen Signalqualität eine Symptom-Rhythmus-Korrelation (95%) hergestellt werden. Bei 34 Patienten (59%) wurden während einer Synkope oder Präsynkope keine pathologischen Arrhythmien festgestellt, so dass eine arrhythmogene Ursache sicher ausgeschlossen werden konnte. Von den 21 Patienten (36%) mit dokumentierter Arrhythmie lag bei 11 Patienten eine bradykarde Herzrhythmusstörung (Sinusbradykardie, SA-Block, AV-Block II°/III°) vor, bei 3 Patienten wurde eine gehäufte ventrikuläre Extrasystolie dokumentiert. Bei 5 Patienten wurden supraventrikuläre Tachykardien dokumentiert, bei 2 Patienten eine nicht-anhaltende Kammertachykardie. Ein junger Patient zeigte in der EKG-Aufzeichnung typische Muskelpotentiale im Sinne eines Krampfanfalls. Eine Indikation zur Implantation eines ILR liegt derzeit bei rezidivierenden unklaren Synkopen vor. In der Zukunft wird der ILR möglicherweise auch in der Risikostratifikation eingesetzt werden. Patients with recurrent syncope of unknown origin after an extensive noninvasive and invasive testing pose a diagnostic and therapeutic dilemma. A few years ago, a new diagnostic tool for evaluation of unexplained syncope was introduced. The implantable Loop recorder (ILR, Reveal®, Medtronic Inc.) continously monitors a single lead electrogram using two sensing electrodes on the device shell. In the German Reveal® Registry, the device was implanted in 149 patients with a history of recurrent syncope. During a mean follow-up of 8.6±6.9 months, 59 patients (40%) experienced syncope and/or presyncope. In the remaining 90 patients (60%), no symptoms occurred. 3 patients did not activate the ILR, in all 56 patients (95%) with activation of the ILR, Loop recording definitively determined, whether an arrhythmia was the cause of symptoms or not: Diagnosis included bradycardia in 11 patients, frequent premature ventricular beats in 3 patients. 5 patients showed a supraventricular tachycardia during symptoms and a non-sustained ventricular tachycardia was revealed in 2 patients. One young patient showed typical muscle potentials during syncope, so that epilepsia was diagnosed. Syncope was nonarrhythmic in 34 patients (59%). An implantable Loop recorder is useful for establishing diagnosis if symptoms are recurrent but too infrequent for conventional monitori ng techniques. In the future, there might be a an additional diagnostic field for the ILR in risk stratification of asymptomatic patients e.g. patients with genetic disorders causing arrhythmias like the Brugada-syndrome.

SengesAbstract:Die Synkope stellt im klinischen Alltag ein haufiges Problem dar, das trotz aufwendiger invasiver und nichtinvasiver Diagnostik bei 13–48% der Patienten ungeklart bleibt. Das Langzeit-EKG ist bei 90% dieser Patienten unauffallig. Neure Entwicklungen in der Loop-Rekorder-Technologie ermoglichen mittels eines implantierbaren Loop-Rekorders ein Langzeit-EKG-Monitoring uber 14 Monate. Uber zwei Sensing-Elektroden auf dem Gehause des subkutan implantierbaren Ereignisrekorders (61 mm × 19 mm × 8 mm, 17 g) wird ein kontinuierliches Ein-Kanal-EKG registriert. Der Ereignisrekorder wurde bei 20 Patienten mit rezidivierenden Synkopen implantiert. Wahrend der Nachbeobachtungszeit von 12±6 Monaten hatten 11 Patienten (55%) eine Synkope (8Patienten) oder Prasynkope (3 Patienten). Die ubrigen 9 Patienten hatten noch kein erneutes Synkopenrezidiv. Mittels des Ereignisrekorders konnte bei allen 11 Patienten festgestellt werden, ob es sich um eine arrhythmogene oder nichtarrhythmogene Synkope handelte. Bei einem Patienten wurde eine Bradykardie und bei 2 Patienten eine tachykarde Rhythmusstorung diagnostiziert, bei einer weiteren Patientin wurden zu verschiedenen Zeitpunkten zwei unterschiedliche Arrhythmien aufgezeichnet: zum einen ein ventrikularer Bigeminus bei bradykardem Grundrhythmus und zum anderen Vorhofflattern. Bei 2 Patienten zeigte sich eine neurokardiogene Synkope. Bei 5 Patienten wurde eine arrhythmogene Synkope mit Sicherheit ausgeschlossen. Der implantierbare Loop-Rekorder ist bei der diagnostischen Abklarung rezidiverender unklarer Synkopen von Nutzen. Der Hauptvorteil gegenuber dem konventionellen Langzeit-EKG oder dem externen Loop-Rekorder liegt im kontinuierlichen EKG-Monitoring uber Monate bis zum Auftreten einer Synkope. Auch wenn diese selten und unvorhersehbar auftritt, kann eine EKG-Aufzeichnung wahrend des Ereignisses erfolgen.

Holter monitoring is non-diagnostic in 90% of cases. Recent developments in Loop recorder technology permit longterm ECG monitoring in patients with recurrent unexplained syncope. The implantable Loop recorder monitors a single lead electrogram continuously using 2 sensing electrodes on the device shell. The device was implanted in 20 patients (11 male, 9 female) with the history of recurrent syncope. During a mean follow-up of 12±6 months after device implantation, 11 patients (55%) experienced syncope (8 pts) or presyncope (3 pts). In the remaining 9 patients, no syncope occurred. In all 11 patients with syncope or presyncope during follow-up, Loop recording definitively determined whether an arrhythmia was the cause of symptoms or not. Diagnosis included bradycardia in one patient, tachycardia in two patients, in one patient two rhythm disturbances were revealed: frequent ventricular premature beats with bigemini and atrial flutter. Two patients had a neurocardiogenic syncope. Syncope was nonarrhythmic in 5 patients.
