    Abstract:

    Abstract The paper presents investigations of the electromagnetic characteristics of circular waveguides made of uniaxial anisotropic chiral medium; the outer surface of the guide being coated with a PEMC (perfect electromagnetic conductor) medium. The emphasis is given on the Energy Flux Density patterns of such guides with varieties of anisotropic chiral materials. Dispersion relation of the guide is developed, and followed by the evaluation of sustained modes, which determine the Energy Flux Density patterns corresponding to different low-order hybrid modes. The Flux Density characteristics provide the evidence of existing backward waves in uniaxial anisotropic mediums, contributing more to the phenomenon of slow light in chiral metamaterials.

    Abstract:

    The paper reports on the wave propagation through twisted clad optical fibers. Analytical treatment of the boundary-value problem has been presented to evaluate the dispersion relation of the fiber followed by the Flux densities. Considering the EH11 and EH12 modes, the propagation constants are obtained to deduce the Energy Flux Density corresponding to these modes. It has been theoretically investigated that the increase of pitch causes the Flux Density to increase in the fiber core - the feature that would be of great use to device attenuators in communication systems.

    Abstract:

    A detailed analytical investigation of the propagation of electromagnetic waves in optical fibers made of uniaxial anisotropic chiral medium is presented. The study has been performed considering varying radial parameter of the guide. Also, two different types of anisotropic mediums as well as fiber dimensions are taken into account. The dispersion relation of the guide is deduced which provides the allowed values of propagation constants. This is followed by the evolution of the expression for the Flux Density in the fiber. A study of the Flux Density pattern reveals the existence of both the forward as well as the backward waves in the fiber structure under consideration.

    Abstract:

    The paper deals with the propagation of electromagnetic waves through twisted clad dielectric optical flbers. The structure of these flbers is analogous to travelling wave tubes used in microwave devices, and the usefulness would be in the areas of optical sensing. This is because the twists in flber would be afiected due to the imposed stress and/or strain, leaving thereby the possibility to alter the propagation characteristics. A rigorous analytical investigation has been carried out with the emphasis on the Energy ∞ux Density patterns due to the difierent propagating modes in the flber. The dispersion relations of the system are deduced and the Energy ∞ux densities are evaluated under difierent pitch angles of twist. The efiect due to conducting helix pitch on the electromagnetic wave propagation is emphasized.

    Background This study aims to investigate the effects of extracorporeal shock-wave therapy (ESWT) and intra-articular steroid injection (IASI) on pain, depression, quality of life and pressure pain threshold (PPT) in patients with costochondritis. Methods A total of 67 patients diagnosed with costochondritis were included. Patients were divided into 2 groups. Group 1 (n=34) received high-Energy Flux Density (H-ESWT) (> 0.28 mJ/mm2) for a total of 7 sessions at 3-day intervals. Group 2 (n=33) received IASI twice at 2-week intervals. At baseline and one month after treatment, Visual Analog Scale (VAS), Short Form-36 (SF-36), Pittsburgh Sleep Quality Index (PSQI), Beck Depression Inventory (BDI) scores and PPT values were compared. Results There was a statistically significant decrease in VAS scores after treatment compared with baseline scores in both groups. Methoden Insgesamt wurden 67 Patienten mit der Diagnose einer Costochondritis eingeschlossen. Die Patienten wurden in 2 Gruppen eingeteilt. Gruppe 1 (n=34) erhielt fur insgesamt 7 Sitzungen in 3-tagigen Intervallen eine hochenergetische Flussdichte (H-ESWT) (> 0,28 mJ/mm²). Gruppe 2 (n=33) erhielt 2-mal im Abstand von 2 Wochen IASI. Zu Studienbeginn und einen Monat nach der Behandlung wurden die Werte fur Visual Analog Scale (VAS), Short Form-36 (SF-36), Pittsburgh Sleep Quality Index (PSQI), Beck Depression Inventory (BDI) und PPT-Werte verglichen. Ergebnisse Es gab eine statistisch signifikante Abnahme der VAS-Werte nach der Behandlung im Vergleich zu den Ausgangswerten in beiden Gruppen. Schlussfolgerungen Unsere Daten deuten darauf hin, dass sowohl die H-ESWT als auch die IASI-Behandlung bei Patienten mit Costochondritis wirksam sind. Die H-ESWT hat dabei eine starkere Wirkung auf Schmerzen und PPT-Scores.

    Abstract:

    Hypertrophic scar is characterized by excessive deposits of collagen during skin wound healing, which could become a challenge to clinicians. This study assessed the effects of the extracorporeal shock wave therapy (ESWT) on hyper-trophic scar formation and the underlying gene regulation. A rabbit ear hypertrophic scar model was generated and randomly divided into three groups: L-ESWT group to receive L-ESWT (Energy Flux Density of 0.1 mJ/mm2), H-ESWT (Energy Flux Density of 0.2 mJ/mm2) and sham ESWT group (S-ESWT). Hypertrophic scar tissues were then collected and stained with hematoxylin and eosin (H&E) and Masson's trichrome staining, respectively, to assess scar elevation index (SEI), fibroblast Density and collagen fiber arrangement. Expression of cell proliferation marker proliferating cell nuclear antigen (PCNA) and α-smooth muscle actin (α-SMA) were assessed using RT-PCR and immunohistochemistry in hypertrophic scar tissues. H&E staining sections showed significant reduction of SEI and fibroblast Density in both ESWT treatment groups compared to S-ESWT, but there was no dramatic difference between L-ESWT and H-ESWT groups. Masson's trichrome staining showed that collagen fibers were more slender and broader and oriented in parallel to skin surface after administration of ESWT compared to control tissues. At the gene level, PCNA-positive fibroblasts and α-SMA-positive myofibroblasts were significantly decreased after L-ESWT or H-ESWT compared to the controls. Furthermore, there was no significant difference in expression of PCNA mRNA between L-ESWT or H-ESWT and S-ESWT, whereas expression of α-SMA mRNA significantly decreased in L-ESWT compared to that of H-ESWT and S-ESWT (P=0.002 and P=0.030, respectively). In conclusion, L-ESWT could be effective on suppression of hypertrophic scar formation by inhibition of scar elevation index and fibroblast Density as well as α-SMA expression in hypertrophic scar tissues of the rabbit model.
