The clinician should be aware of the configuration of the pulp system. Maxillary First Premolars usually have two canals. The incidence of three canals in these teeth is quite rare. This case report presents the diagnosis and clinical management of a maxillary First Premolar with two distinct canals in the apical third of buccal root (type IV), drawing particular attention to tactile examination of all the canal walls.

At times, there are apparent variations in the teeth. These discrepancies should be thoroughly surveyed and piled up to date. One such rare complexity of canal morphology was discussed in the present case. A 48-year-old female patient referred to the endodontic clinic for full-mouth rehabilitation. On intraoral hard-tissue examination, decay was noted in multiple teeth which were confirmed by an orthopantomogram and endodontic treatment plan was suggested. The patient was prepared for the access opening of right mandibular First Premolar (tooth #44) before which endodontic treatment of First and second quadrants was completed. The cone-beam computed tomography (CBCT) images revealed that tooth # 44 had two separate roots and four distinct root canals (mesiobuccal, mesiolingual, distobuccal, and distolingual). Root canal treatment was completed in two visits. Postoperative CBCT was advised to confirm the obturation of all the canals.

Maxillary canine and First Premolar transposition (Mx.C.P1 transposition) is most commonly observed. Data from Native Americans samples are lacking. The purpose of this study was: 1) to document the occurrence of Mx.C.P1 transposition, and 2) to generate information on the prevalence of this transposition type among world populations. Eleven cases (M = 5, F = 4, ? = 2) of Mx.C.P1 transposition were identified in skeletal samples from Pecos Pueblo (n = 500) and Sambrito Village (n = 10), both in northern New Mexico. Nine cases were unilateral, 6 affecting the right side and 3 affecting the left side. The remaining 2 cases were bilateral. The prevalence rate of Mx.C.P1 transposition in African and Native American samples, including Pecos Pueblo (1.8%), is higher than in samples from Scotland, Sweden, Saudi Arabia, and India. Mx.C.P1 transposition frequencies vary between geographically distinct populations, although sample composition may be a factor contributing to sample variation. This study investigated the location of the lingual canal orifice and the curvature of the lingual canal by using micro–computed tomography. Methods One hundred fifteen mandibular First Premolars were scanned by micro–computed tomography, reconstructed 3-dimensionally by using Mimics 10.01 software, and displayed in parallel projection mode. Twenty-six teeth with Vertucci type V canal were selected for further study. The lingual canal orifice was located by measurements made in both lingual and proximal views. The angle alpha (α) between the start of the lingual canal and the main canal and the angle beta (β) of the curvature of lingual canal were also measured. Results In proximal view, 69% of lingual canals were located in the middle third of the tooth and the remainder in the apical third. In lingual view, 73% were located in the middle third of the root and the remainder in the coronal third. Mean angle α and angle β were 33.54° and 26.66°, respectively, in proximal view and 8.31° and 11.31°, respectively, in lingual view ( P Conclusions Detailed information on the lingual canal is essential for successful endodontic treatment in patients with mandibular First Premolar. The view used for imaging influences the information obtained.

SUMMARY: The complex morphology of geminated teeth renders their endodontic and periodontal management difficult. Root canal and periodontal treatments were performed on a geminated mandibular First Premolar with three canals. Clinical examination showed two separated crowns with united roots. Radiographically, two distinct pulp chambers with two joined and a third independent canal were seen. Conventional root canal treatment resulted in complete healing of the apical lesion. However, the occurrence of a vertical fracture led to the extraction of the mesial segment. At the follow-up visit, the distal segment was clinically healthy and continued to satisfy functional demands.
