Labial frenectomy with Nd:YAG laser and conventional surgery: a comparative study

Occasionally, they assume inadequate size or location and may lead to functional and esthetic limitations. The aim of the present study was to compare pre-, trans-, and postsurgical clinical parameters of labial frenectomies performed with conventional surgery and neodymium-doped yttrium aluminum garnet (Nd:YAG) laser. Forty individuals were assessed as a convenience sample and were divided into two groups according to the treatment: group 1 (G1), conventional surgery ( n = 22), and G2, Nd:YAG laser surgery ( n = 18). Clinical parameters such as Frenulum insertion, location, bleeding, surgical time, suturing, preoperative fear, and postoperative discomfort/functional limitations were evaluated. All surgeries were performed by the same operator, and the level of fear, pain, and discomfort related to oral functions were assessed with a visual numeric scale. Nd:YAG laser frenectomies reduces transoperative bleeding, avoiding the need of suturing, and promotes a significant reduction of surgical time in comparison with conventional surgery. Therefore, further studies are necessary to provide a complete understanding and standardization of the technique as well as the expected clinical results.

Posterior lingual Frenulum in infants: occurrence and maneuver for visual inspection

Methods: an experimental study including 1,715 healthy infants, in which prematurity, perinatal complications, craniofacial anomalies neurological disorders, and visible genetic syndromes were the exclusion criteria. A clinical examination was performed by means of a maneuver that consisted in rising the lateral margins of the tongue to visualize the anatomical characteristics of the lingual Frenulum. In some of the infants, a special maneuver was performed to assist visualization of posterior lingual Frenulum, since its visualization was not possible. The maneuver consisted in two simultaneous actions: elevating and pushing the tongue back. Results: 558 infants (32.54%), out of the 1,715 had posterior Frenulum, which required the special maneuver that consisted in both elevating and pushing the tongue back, simultaneously. Conclusion: the occurrence of posterior lingual Frenulum was high and the special maneuver consisted in elevating and pushing the tongue back proved to be efficient to visualize the posterior lingual Frenulum.

Materials and Methods: Between March and July, 2011, 50 children between 6 months and 4 years of age were assessed by a Speech Language Pathologist in a Public Health Care Service in Brotas - Sao Paulo, Brazil. The lingual Frenulum evaluation protocol proposed by Marchesan (2010, 2012) was administered to the 50 children - 26 male and 24 female. Of the 50, 8 children had lingual Frenulum alterations. The 8 children with lingual Frenulum alteration underwent lingual frenectomy. The materials for analyses were taken from 7-micrometer-thick tissues sections. Staining was employed using haematoxylin paired with eosin (H&E), Masson's trichrome and picrosirius red. Results: The assessment of the lingual Frenulum indicated that 8 children had lingual Frenulum alteration. Surgery was performed and the material collected was submitted to analyses. The histological analyses demonstrated that the lingual Frenulum of children with ankyloglossia or with short Frenulum had bundles of striated skeletal muscle fibers, high frequency of type I collagen in the deep layers and compact bundles of elastic fibers positioned near the epithelial lining. Bundles of muscle fibers were not observed in anterior Frenulum and short Frenulum with anterior fixation. Type I collagen and bundles of elastic fibers sparse and far from the epithelial lining were observed in those types of Frenulum. Conclusion: The analysis of the histological sections demonstrated the characteristics of altered human lingual Frenulum.

METHODS: Participants were 53 subjects who had never undergone speech therapy or lingual Frenulum surgery. A specific lingual Frenulum protocol with scores was used by speech-language pathologists when there was evidence of Frenulum alteration. Ten subjects had abnormal Frenulum and were referred to an otolaryngologist for frenectomy. After surgery, the subjects were re-evaluated using the same protocol. Photos and videos were taken for comparison. RESULTS: Thirty days after surgery, the subjects had the shape of the tip of the tongue and its movements improved. Lip closure and speech were also improved. CONCLUSION: Frenectomy is efficient to improve tongue posture, tongue mobility, oral functions, and oral communication.

Lingual Frenulum protocol.

From a specific lingual Frenulum evaluation used until 2004, a new protocol was designed. Ten speech language pathologists experienced in orofacial myology used the new protocol with different groups of subjects. 1235 subjects were evaluated during 3 years. From the experience of these ten speech language pathologists, the protocol was re-structured, and a scoring system was added. Absence of alteration (normal tongue and Frenulum) was scored zero. The alterations observed were scored in ascending order. Four additional speech language pathologists experienced in orofacial myology were trained by the researcher to administer the final version of the protocol. The protocol was administered in 2008 and 2009 to 239 subjects: 160 children between 7 years and 2 months old and 11 years and 7 months old; and to 79 adults from 16 years and 8 months or older. From the results of administration of the protocol, a new lingual Frenulum protocol with scores was designed. According to the scores, the Frenulum can be considered altered or normal. When the sum of general tests is equal or higher than 3, the Frenulum may be altered. The interference of the lingual Frenulum in the oral functions may be considered when the sum of the functional tests is equal or higher than 25. This new lingual Frenulum protocol with scores was designed and has been an efficient tool to diagnose an altered lingual Frenulum.

After obtaining the data and statistical analysis, the protocol was re-designed and administered to 100 infants. The aim of this study is to present an efficient and effective lingual Frenulum protocol with scores for infants. From the experimental protocol model, a new protocol was designed. One speech-language pathologist, and specialist in orofacial myology, administered the new protocol to 100 full-term infants. All steps of the protocol were recorded and photographed. The data collected was sent to two specialists in the area, who evaluated the cases based on the recordings and photographs. The data from the three evaluations were compared. A two-part protocol was designed to evaluate the lingual Frenulum in infants. The first part consists of clinical history with specific questions about family history and breastfeeding. The second part consists of clinical examination: anatomo-functional, non-nutritive and nutritive sucking evaluations. A new lingual Frenulum protocol with scores for infants was designed, and has proved to be an effective tool for health professionals to assess and diagnose anatomical alterations of the lingual Frenulum, and its possible interference with breastfeeding.

Surface Electromyographic Analysis of the Suprahyoid Muscles in Infants Based on Lingual Frenulum Attachment during Breastfeeding.

Here, we aim to analyze the suprahyoid muscle electrical activity of infants via surface electromyography, based on lingual Frenulum attachment to the sublingual aspect of the tongue and floor of the mouth during breastfeeding. In the present study, we have studied full-term infants of both genders, aged between 1 and 4 months old. The mean muscle activities were recorded in microvolts and converted into percent values of the reference value. Associations between the root mean square and independent variables were tested by one-way analysis of variance and Student’s t-test, with a significance level of 5% and test power of 95%, respectively. We evaluated 235 infants. Lower mean muscle electrical activity was observed with the lingual Frenulum attached to apex/lower alveolar ridge, followed by attachment to the middle third/lower alveolar ridge, and between the middle third and apex/lower alveolar ridge. Greater suprahyoid muscle activity was observed with lingual Frenulum attachment to the middle third of the tongue/sublingual caruncles, showing a coordination between swallowing, sucking, and breathing. Surface electromyography is effective in diagnosing lingual Frenulum alterations, the attachment points of which raises doubt concerning the restriction of tongue mobility. Thus, it is possible to identify oral motor dysfunctions.
