Single-port thoracoscopic rib resection: a case report

We present the technique, which may expand the indications of single-port thoracoscopic procedures.

Tumors were prepared by injecting human HT-1080 fibrosarcoma cells into the subcutaneous tissue of 6–9 week old male athymic nude mice (BALB/c nu/nu). When the tumors reached the desired size, the nude mice were sacrificed, and the tumors were harvested en bloc. Then the tumors were used in either contamination group or tumor inoculation group. In the contamination group, 34 tumors were cut by Gigli Saw (Group G) and 35 tumors were cut by No. 11 scalpel (Group S). In the tumor inoculation group, a tumor weight of 100 mg was inoculated into 32 non-tumor bearing nude mice. Then, the nude mice were randomized into either of 5 subgroups: group B–N: no local agent was used, group B–S: lavage by saline, group B–P: lavage by povidone iodine 10%, group B–H: lavage by hydrogen peroxide, group B–C: lavage by cisplatin and distilled water. For lavage, the fluid was kept in the wound for 5 minutes apart from the cisplatin which was kept in the wound for 2.5 minutes and then the wound was filled with distilled water for the next 2.5 minutes. Tumor growth occurred in S–N, S–S, S–H, S–P and S–C groups in 33.3%, 67.7%, 16.7%, 16.7%, and 0% respectively. Tumor growth occurred in G–N, G–S, G–H, G–P, and G–C groups in 50%, 67.7%, 33.3%, 33.3%, and 0%, respectively. The total growth incidence after tumor contamination in the no-agent, saline, hydrogen peroxide, povidone iodine and cisplatin groups were 41.7%, 58.3%, 25%, 25% and 0%, respectively. Growth occurred in 100% of B–N and B–S groups, 83% of B–H and B–P groups and 33.3% of the B–C group. Cisplatin used for 2.5 minutes followed by distilled water for 2.5 minutes is effective for protection against local tumor recurrence due to tumor contamination. It also has a reasonable effect against tumor blocks.

Tumor growth potential after tumoral and instrumental contamination: an in-vivo comparative study of T-Saw, Gigli Saw, and scalpel

The purpose of this study was to compare the potential for tumor growth after intralesional cutting by T-Saw, Gigli Saw, and scalpel. Tumors, prepared by the subcutaneous injection of human HT 1080 fibrosarcoma cells in nude mice, were harvested and cut with a T-Saw, Gigli Saw, or scalpel. A 3-cm wound was created in the skin on the back in another group of nude mice. The cut surface of the tumor was rubbed for 10 s against the subcutaneous tissue in this second group of nude mice. In the same manner, the instrument used for tumor cutting was rubbed against the subcutaneous tissue in the 3-cm wound in the back of a third group of nude mice for 10 s. Other instruments used for tumor cutting were immediately washed in culture medium, and the total number of tumor cells was counted. Tumor blocks and single-cell suspensions prepared from tumor tissues of the same weight were inoculated and injected into a fourth group of nude mice. The incidence of tumor growth after the rubbing of the subcutaneous tissue with the tumor surface cut with a T-Saw, Gigli Saw, and scalpel was 16.7%, 50.0%, and 33.3%, respectively. The corresponding figures after the rubbing of the subcutaneous tissue with a T-Saw, Gigli Saw, and scalpel was 16.7%, 33.3%, and 33.3%, respectively. The mean numbers of tumor cells attached to the T-Saw, Gigli Saw, and scalpel were 1.88 × 10^5, 5.02 × 10^5, and 4.92 × 10^5, respectively. Finally, the incidence of tumor growth after the inoculation of tumor blocks and single-cell suspensions was 100% and 77.8%, respectively. These findings show that tumor recurrence is less likely after intralesional tumor cutting with a T-Saw than after such cutting with a Gigli Saw or scalpel.

Comparison of four different techniques for performing an osteotomy: a biomechanical, radiological and histological study on rabbits tibias

Üsümez, Ergun Bozdag, Cengiz SenAbstract:This animal study compares different methods of performing an osteotomy, including using an Erbium-doped Yttrium Aluminum Garnet laser, histologically, radiologically and biomechanically. A total of 24 New Zealand rabbits were divided into four groups (Group I: multihole-drilling; Group II: Gigli Saw; Group III: electrical Saw blade and Group IV: laser). A proximal transverse diaphyseal osteotomy was performed on the right tibias of the rabbits after the application of a circular external fixator. The rabbits were killed six weeks after the procedure, the operated tibias were resected and radiographs taken. The specimens were tested biomechanically using three-point bending forces, and four tibias from each group were examined histologically. Outcome parameters were the biomechanical stability of the tibias as assessed by the failure to load and radiographic and histological examination of the osteotomy site. The osteotomies healed in all specimens both radiographically and histologically. The differences in the mean radiographic (p = 0.568) and histological (p = 0.71) scores, and in the mean failure loads (p = 0.180) were not statistically significant between the groups. Different methods of performing an osteotomy give similar quality of union. The laser osteotomy, which is not widely used in orthopaedics is an alternative to the current methods.

Effects of calcitonin and alendronate on distraction osteogenesis

Forty-five mature female New Zealand type rabbits were used. Rabbits were randomized into three groups, each consisting of 15 animals. Under optimal operating conditions a pre-reconstructed circular external fixator was applied to the right tibia of rabbits, and osteotomy was performed with a Gigli Saw just below the tibial tuberosity. During the ten days of distraction, a placebo was administered for group I (control group), group II received 10 U of calcitonin, and group III was treated with alendronate (0.5 mg/kg/day) gavages. Rabbits were examined histologically (at the third and eighth weeks, according to Huddlestone et al.), radiologically (at the third, sixth, and eighth weeks, according to Lane and Sandhu), and mechanically (at the eighth week, as torsional loading). During the first three weeks, the groups did not differ much with regard to radiological parameters. However, in the calcitonin and alendronate groups improved histological scores were detected compared with the control group. Radiological images obtained at the end of six weeks demonstrated improved consolidation in the calcitonin and alendronate groups compared with the control group. At the end of eight weeks postoperatively, radiological and histological parameters did not differ among the groups, while torsional failure load was found to be much improved in the calcitonin group ( p =0.006). During the distraction osteogenesis period, calcitonin and alendronate affected the quality of regenerate favourably. This effect is sustained until the end of the consolidation period. However, this finding needs to be supported by experimental and human studies. Etude expérimentale des effets de la calcitonine et de l’alendronate sur l’ostéogénèse en distraction. Quarante-cinq lapins adultes femelles de Nouvelle Zélandeont ont été le support de l’expérimentation. Les lapins ont été randomisés en trois groupes de 15 animaux dans des conditions opératoires satisfaisantes. Il a été mis en place un fixateur externe sur les tibias droits de chaque lapin. L’ostéotomie a été réalisée avec la scie de Gigli juste en dessous de la tubérosité tibiale. Pendant les dix premiers jours d’allongement, un placebo a été administré au premier groupe (groupe contrôle), le groupe 2 a reçu 10 unités de calcitonine, le groupe 3 a été traité par l’alendronate (0,5 mg/kg/jour). Les lapins ont été sacrifiés et examinés histologiquement à la troisième et la huitième semaines (Ref. Huddlestone et col.), radiologiquement à la troisième, sixième et huitième semaines (Ref. Lane et Sandhu) et mécaniquement à la huitième semaine avec tests de torsion. Dans les trois premières semaines, on ne retrouve pas de différences radiologiques entre les trois groupes, cependant l’on constate une amélioration histologique sur les groupes calcitonine et alendronate comparés ou groupe contrôle. Les images radiologiques obtenues à la fin de la sixième semaine nous montrent une meilleure consolidation pour les groupes calcitonine et alendronate. A la fin de la huitième semaine, post-opératoire il n'y avaient pas de différences radiologiques et histologiques dans les différents groupes, les tests de torsion sont simplement améliorés dans le groupe calcitonine ( p =0,006). En conclusion, durant la période d’ostéogénèse en distraction, la calcitonine et l’alendronate ont des effets positifs sur la qualité du régénérat, ces effets se maintiennent jusqu’à la période de consolidation. Cependant, ces effets positifs doivent être confirmés par des études chez l’homme.

The reliability of percutaneous osteotomy with the Gigli Saw technique in the proximal tibia. A cadaveric study

Methods: Forty-two attendants comprising of residents or consultants without any prior experience were assigned to 14 cadaveric models to perform percutaneous osteotomy. The technique was first described by an experienced specialist and then illustrated on a cadaver specimen. Thereafter, the attendants performed osteotomy using a Gigli Saw without any help by the instructors. Finally, exploration of the deep and superficial branches of the fibular nerve, posterior tibial nerve, posterior and anterior artery and vein was made with regard to any injury caused during osteotomy. Results: There were ten incisional injuries in three specimens due to erroneous determination of the osteotomy level or incision. A mean of 3.5 cm (range 2.5 to 4.0 cm) incision length was observed in four specimens. No procedure-associated injuries to major nerves and vessels were documented except for those to the cutaneous branch of the superficial peroneal nerve in two specimens, and to the extensor hallucis longus muscle in one specimen. There was no incidence of incomplete osteotomy. Conclusion: Despite the lack of any prior experience with the osteotomy method with the Gigli Saw on the part of the attendants, the full success in performing osteotomy confirms the reliability of the technique. Moreover, if this method is to be used by experienced surgeons, excellent results can be achieved without any injuries to cutaneous nerves and surrounding muscles.
