    Context.—The newest generation Hematology Analyzer, Sysmex XE-5000 (Sysmex Corporation, Kobe, Japan) is equipped with an improved body fluid analysis mode. Objective.—To evaluate the applicability of the XE-5000 Analyzer to white blood cell (WBC) analysis in cerebrospinal fluid (CSF). Design.—A total of 425 routinely collected, consecutive CSF samples were included in the study. For a comparison of total WBC counts, the results of routine chamber counts were grouped into categories of 0 to 5 (n  =  330), >5 to 10 (n  =  36), >10 to 50 (n  =  39), >50 to 200 (n  =  15), and >200 (n  =  5) WBC/µL. Microscopic differential counts were performed using cytospins from 276 samples. Results were grouped according to the percent content of polymorphonuclear (PMN) cells, 0% to 25% (n  =  263), >25% to 50% (n  =  7), >50% to 75% (n  =  3), and >75% to 100% (n  =  3) of WBC.     Abstract Context.—The new Hematology Analyzer Sysmex XE-2100 (TOA Medical Electronics, Kobe, Japan) has a novel, combined, white blood cell differential technology and a special reagent system to enumerate nucleated red blood cells. Design.—Performance evaluation of both technologies of the Sysmex XE-2100 according to the H20-A protocol of the National Committee for Clinical and Laboratory Standards and comparison of the results with those for the Hematology Analyzer Sysmex NE-8000 (TOA Medical Electronics). Specimens.—Five hundred forty-four blood samples randomly chosen from various inpatient and outpatient departments of the Vienna University hospital. Conclusions.-From the present evaluation and our former experience with other types of Sysmex Analyzers, we conclude that the new white blood cell differential technology of the XE-2100 represents a further development toward more efficient flagging of abnormal white blood cells.

    Context.—The Coulter DxH 800 Hematology Analyzer can determine leukocyte numerical parameters (total leukocyte counts and differentials). It also measures intrinsic biophysical properties of these cells in their near-native state. These morphologic measurements are known as cell population data (CPD). Objective.—To study, for the first time, the biological variations of morphologic parameters or CPD and reinvestigate numerical parameters using the newest Coulter Hematology Analyzer. Design.—Forty adult volunteers (21 women, 19 men) were included. All participants maintained their normal lifestyles. Blood samples were drawn in duplicate by a single experienced phlebotomist and analyzed within 2 hours using a single Analyzer.     Abstract:

    We studied the clinical usefulness of the VCS parameters (mean channels of cell volume, conductivity, and light scatter) in reactive neutrophils for predicting acute bacterial infection, which are obtained by the Coulter LH 750 Hematology Analyzer (Beckman Coulter, Fullerton, CA) during automated differential counts. Peripheral blood samples from 69 patients with positive blood cultures for bacteria and 35 control subjects were studied. We observed a significant increase in the mean channel of neutrophil volume (MNV) from septic patients compared with control subjects (156 ± 13.5 vs 143 ± 4.8; P < .001). The mean channel of neutrophil light scatter was decreased significantly in patients (140 ± 10.1 vs 146 ± 7.3; P = .002). An elevation of the MNV was associated with a higher WBC count and percentage of neutrophils and was present even in patients who did not have leukocytosis or neutrophilia. With a cutoff of 150 for the MNV, a specificity of 91% and sensitivity of 70% were achieved. As a quantitative, objective, and more sensitive parameter, we believe that the MNV has a potential to be an additional indicator for acute bacterial infection.

The correct and timely diagnosis of severe acute infectious processes, such as septicemia, is extremely important for proper patient management. However, given the overlapping clinical signs and symptoms with those of other acute critical medical conditions, achieving a high level of certainty while diagnosing septicemia can be a difficult challenge in which the laboratory has a crucial role providing invaluable information.
