    Background: Uptake of the human papillomavirus (HPV) Vaccine is low in Appalachian Ohio and areas with high cervical cancer rates. Methods: We conducted a group-randomized trial among 12 counties in Appalachian Ohio randomized to receive either an HPV Vaccine (intervention counties) or influenza Vaccine (comparison counties) multilevel intervention (MLI). Parents ( n = 337) who had a daughter aged 9 to 17 years who had not received the HPV Vaccine were recruited from commercial lists. Clinics ( N = 24) and 119 providers from these clinics were also recruited. The primary outcome was medical record–confirmed receipt of the first shot of the HPV Vaccine 3 months after receiving the intervention among daughters of parents enrolled in the study. Secondary outcomes included receipt of the first HPV Vaccine shot by 6 months and changes in provider knowledge. Results: According to medical records, 10 (7.7%) daughters of intervention participants received the first shot of the HPV Vaccine within 3 months of being sent the intervention materials compared with 4 (3.2%) daughters of comparison group participants ( P = 0.061). By 6 months, 17 (13.1%) daughters of intervention participants received the first HPV Vaccine shot compared with eight (6.5%) daughters of comparison group participants ( P = 0.002). Provider knowledge about HPV increased ( P < 0.001, from baseline to after education). Conclusions: The MLI increased uptake of the HPV Vaccine among girls aged 9 to 17 years; however, uptake was low. Impact: To improve HPV Vaccine uptake, attention to additional levels of influence (e.g., policy, community) and more elements within levels (e.g., reminders, automated prompts) may be needed.     To assess HPV Vaccine acceptability, focus groups of women (18–26 years), parents, community leaders, and healthcare providers were conducted throughout Ohio Appalachia. Themes that emerged among the 23 focus groups (n=114) about the HPV Vaccine were: barriers (general health and Vaccine specific), lack of knowledge (cervical cancer and HPV), cultural attitudes, and suggestions for educational materials and programs. Important Appalachian attitudes included strong family ties, privacy, conservative views, and lack of trust of outsiders to the region. There are differences in HPV Vaccine acceptability among different types of community members highlighting the need for a range of HPV Vaccine educational materials/programs to be developed that are inclusive of the Appalachian culture.

    Background: Increasing knowledge about human papillomavirus (HPV) and HPV Vaccine is a potentially important way to increase vaccination rates, yet few education interventions have addressed these topics. We report the results of an education intervention targeting three key groups who have contact with adolescent females. Methods: We conducted HPV education intervention sessions during 2008 and 2009 in Guilford County, North Carolina. Parents ( n = 376), healthcare staff ( n = 118), and school staff ( n = 456) attended the one-time sessions and completed self-administered surveys. Analyses used mixed regression models to examine the intervention's effects on participants' self-rated HPV knowledge, objectively assessed HPV and HPV Vaccine knowledge, and beliefs about HPV Vaccine. Results: Participants had relatively low levels of objectively assessed HPV and HPV Vaccine knowledge prior to the intervention. The education intervention increased self-rated HPV knowledge among all three key groups (all P < 0.001), and objectively assessed knowledge about many aspects of HPV and HPV Vaccine among healthcare and school staff members (all P < 0.05). Following the intervention, more than 90% of school staff members believed HPV and HPV Vaccine education is worthwhile for school personnel and that middle schools are an appropriate venue for this education. Most parents (97%) and school staff members (85%) indicated they would be supportive of school-based vaccination clinics. Conclusions: Our education intervention greatly increased HPV and HPV Vaccine knowledge among groups influential to the HPV vaccination behaviors of adolescent females. Impact: Education interventions represent a simple yet potentially effective strategy for increasing HPV vaccination and garnering stronger support for school-based vaccination clinics.     In 2009, the United States approved quadrivalent HPV Vaccine for males 9-26 years old, but data on Vaccine uptake are lacking. We determined HPV Vaccine uptake among adolescent males, as well as stage of adoption and Vaccine acceptability to parents and their sons. A national sample of parents of adolescent males ages 11-17 years (n=547) and their sons (n=421) completed online surveys during August and September 2010. Analyses used multivariate linear regression. Few sons (2%) had received any doses of HPV Vaccine, and most parents and sons were unaware the Vaccine can be given to males. Parents with unvaccinated sons were moderately willing to get their sons free HPV Vaccine (mean=3.37, SD=1.21, possible range 1-5). Parents were more willing to get their sons vaccinated if they perceived higher levels of HPV Vaccine effectiveness (β=0.20) or if they anticipated higher regret about their sons not getting vaccinated and later developing an HPV infection (β=0.32).     ABSTRACTBackground: We sought to estimate the national prevalence of HPV Vaccine refusal and delay in a nationally-representative sample of parents of adolescents. We also compared parents who refused versus delayed HPV Vaccine in terms of their vaccination beliefs and clinical communication preferences. Methods: In 2014 to 2015, we conducted an online survey of 1,484 US parents who reported on an 11- to 17-year-old child in their household. We used weighted multinomial logistic regression to assess correlates of HPV Vaccine refusal and delay. Results: Overall, 28% of parents reported that they had ever “refused or decided not to get” HPV Vaccine for their child, and an additional 8% of parents reported that they had “delayed or put off getting” HPV Vaccine. Compared to no refusal/delay, refusal was associated with lower confidence in adolescent vaccination (relative risk ratio [RRR] = 0.66, 95% confidence interval [CI], 0.48–0.91), lower perceived HPV Vaccine effectiveness (RRR = 0.68, 95% CI, 0.50–0.91)...

    Abstract Objectives While the provision of cervical cancer prevention services in the United States has expanded to new settings beyond clinics that give Pap smears, prevention efforts are being hampered by relatively low human papillomavirus (HPV) Vaccine coverage. Pharmacies are an underused setting to deliver HPV Vaccine. To better understand this opportunity, we sought to classify pharmacists' authority to administer HPV Vaccine in each US state. Methods For each US state and the District of Columbia (for simplicity, we refer to these 51 regions as states), we interviewed a member of the state's pharmacy association, member of the state board of pharmacy, or a faculty member at a school or college of pharmacy. Results Most states (80%) allowed pharmacists to provide HPV Vaccine to adult women ages 19 and older, and 61% of states allowed provision to girls age 12. The mechanism for pharmacists to immunize was highly variable across states. For example, a 12year-old girl seeking HPV Vaccine could receive it from a pharmacist in 31% of states under a protocol between a specific physician and pharmacist, in 24% with an HPV Vaccine prescription, and in 6% without prior physician approval. Pharmacists' authority was broadest on the west coast and limited on the east coast. Pharmacist authority to provide HPV, Tdap, and meningitis Vaccines was very similar, but it was highly dependent on patient age. Conclusions US states' laws governing pharmacists' ability to offer HPV Vaccine varied widely. One consequence is that newly expanded cervical prevention efforts underuse pharmacists.

Results: According to medical records, 10 (7.7%) daughters of intervention participants received the first shot of the HPV Vaccine within 3 months of being sent the intervention materials compared with 4 (3.2%) daughters of comparison group participants ( P = 0.061). By 6 months, 17 (13.1%) daughters of intervention participants received the first HPV Vaccine shot compared with eight (6.5%) daughters of comparison group participants ( P = 0.002). Provider knowledge about HPV increased ( P < 0.001, from baseline to after education). Conclusions: The MLI increased uptake of the HPV Vaccine among girls aged 9 to 17 years; however, uptake was low. Impact: To improve HPV Vaccine uptake, attention to additional levels of influence (e.g., policy, community) and more elements within levels (e.g., reminders, automated prompts) may be needed.     To assess HPV Vaccine acceptability, focus groups of women (18–26 years), parents, community leaders, and healthcare providers were conducted throughout Ohio Appalachia. Themes that emerged among the 23 focus groups (n=114) about the HPV Vaccine were: barriers (general health and Vaccine specific), lack of knowledge (cervical cancer and HPV), cultural attitudes, and suggestions for educational materials and programs. Important Appalachian attitudes included strong family ties, privacy, conservative views, and lack of trust of outsiders to the region. There are differences in HPV Vaccine acceptability among different types of community members highlighting the need for a range of HPV Vaccine educational materials/programs to be developed that are inclusive of the Appalachian culture.

    Background: The initiation and timely completion of the human papillomavirus (HPV) Vaccine in young women is critical. We compared the initiation and completion of the HPV Vaccine among women in 2 community-based networks with electronic health records: 1 with a prompt and reminder system (prompted cohort) and 1 without (unprompted cohort). Methods: Female patients aged 9 to 26 years seen between March 1, 2007, and January 25, 2010, were used as the retrospective cohort. Patient demographics and vaccination dates were extracted from the electronic health records. Results: Patients eligible for the Vaccine included 6019 from the prompted cohort and 9096 from the unprompted cohort. Mean age at initiation was 17.3 years in the prompted cohort and 18.1 years in the unprompted cohort. Significantly more (P Conclusion: More patients aged 9 to 26 years initiated and achieved timely completion of the HPV Vaccine series in clinics using an electronic health record system with prompts compared with clinics without prompts.

    Abstract:

    We evaluated the characteristics associated with uptake of HPV Vaccine by 19-26 year old women seen in primary care university-based clinics. Of the 11,545 women analyzed only 18% had initiated the 3-dose Vaccine series. Series completion among the sample overall was only 10% in the 30 month study period. Decreased series initiation was associated with older age, public insurance, white race and non-family medicine specialty. Decreased series completion was associated with public insurance and African American race. Utilization disparities by race and insurance worsened over time suggesting that the highest risk populations of women were not getting vaccinated.
