Cataract surgery to lower Intraocular Pressure

Cataract surgery is frequently performed in patients with glaucoma. In this study, a review of literature with search terms of cataract, glaucoma and Intraocular Pressure is followed by evaluation and synthesis of data to determine the effect of cataract surgery on Intraocular Pressure. Cataract surgery seems to lower Intraocular Pressure on a sustained basis, especially in patients with higher preoperative Intraocular Pressure. The mechanism of action of these finds remains speculative.

Religious fasting and Intraocular Pressure.

METHODS: The authors measured diurnal Intraocular Pressure values during religious fasting and 1 month later in 38 healthy male volunteers with a mean age of 22.4 +/- 2.7 years. Body weight and urine specific gravity were determined to assess the extent of dehydration caused by fasting. RESULTS: Although each patient had weight loss (0.1-1.4 kg) representing a slight dehydration caused by fasting, Intraocular Pressure values were not statistically different between fasting and nonfasting periods (P > 0.05). CONCLUSION: Fasting did not alter diurnal Intraocular Pressure values in healthy people.

The contralateral reduction of Intraocular Pressure by timolol.

Spaide, G DunhamAbstract:Administration of timolol to one eye is associated with a decrease in Intraocular Pressure in both eyes. To further investigate this effect the contralateral decrease in Intraocular Pressure was measured in a three arm crossover study using a 0.1 mg dose of timolol given topically to an eye, a 0.1 mg dose of timolol given lingually, and a placebo given topically. Two hours after topical timolol administration the mean Intraocular Pressure reduction in the fellow eye was 3.1 mm Hg compared with baseline (p = 0.0007). Two hours after lingual timolol administration the mean Intraocular Pressure reduction was 3.9 mm Hg compared with baseline (p = 0.0004). Two hours after topical administration of placebo the mean Intraocular Pressure reduction in the fellow eye was only 0.33 mm Hg (p = 0.6). These findings suggest the contralateral reduction in Intraocular Pressure from timolol is caused by systemic absorption. The significant Intraocular Pressure reduction obtained from lingual timolol raises the possibility that this route of drug administration may be useful in selected patients who cannot use eye drops.

BAICALEIN LOWERS Intraocular Pressure IN GERBILS

CivanAbstract:Purpose: Currently, no clinical treatments are available to prevent the glaucomatous optic neuropathy except for lowering Intraocular Pressure. Baicalein is a common traditional Chinese medicine that has no reported toxicity. We have previously demonstrated that baicalein inhibits chloride secretion and net fluid transport across the excised ciliary epithelium, potentially reducing aqueous humor formation and Intraocular Pressure. The aim of this study is to determine the changes in Intraocular Pressure by baicalein in living animals. Methods: Gerbils (sand rats) were used in the experiments. Baicalein was administered intraperitoneally to the animals in the treatment group once every two days for two weeks, while saline solution was used in the control animals. Measurements of the Intraocular Pressure were conducted under anesthesia with a rebound tonometer (TonoLab) prior to drug administration. Results: Intra-peritoneal injection of baicalein (100mg/kg) significantly lowered IOP in gerbils. The maximum hypotensive effect was achieved two days after the first injection and then maintained at almost the same level throughout the experimental period. After two weeks, baicalein-treated animals had a lower Intraocular Pressure of -3.92 ± 0.89 mmHg (N = 4, p < 0.05) as compared to the saline-treated controls, accounting for a 23-26% reduction of Intraocular Pressure from the baseline measurements. At a lower concentration of baicalein (4mg/kg), a hypotensive effect of 15% was also observed (N = 5). Conclusions: Baicalein appears to function as an ocular hypotensive agent. Its site and mode of action remain to be determined.
