    Abstract:

    Purpose: Platelet-rich plasma (PRP) is derived from centrifuging whole blood to obtain a high platelet concentration containing numerous growth factors. Despite its widespread use, there is still a lack of high-level evidence regarding randomized clinical trials assessing the efficacy of PRP in treating Ligament injuries. Although there is research showing an improvement in the early stages of healing in the animal model of acute medial collateral Ligament (MCL) injury of the knee, there is no strong evidence to support the efficacy of PRP injections for treating MCL Lesions in humans. Case Report: In this report, we present a case of an elite football player, treated with multiple PRP local injections followed by rehabilitation, for a high grade MCL Lesion of the knee. He was able to resume training at day 18, painfree, with full range of motion and the ability to complete a functional test based on all sport specific movements. He played matches at 25 days with no residual symptoms or functional deficit. There were no further complaints or recurrences at the 16 months follow up Conclusions: On the basis of this report, we can assume that the treatment of high grade acute MCL Lesions of the knee with PRP is a promising therapeutic option to be further explored with good quality Randomized Controlled Trials (RCTs).

    Abstract:

    Objective To explore the relationship between tibial slope and anterior cruciate Ligament(ACL)Lesion. Methods Thirty-eight patients diagnosed as ACL Lesion were enrolled as Lesion group,and 38 patients with patellofemoral pain were enrolled as control group.The patients in two groups were matched on the basis of age and sex.All patients underwent X-ray and MRI.The slope angle of tibial plateau was measured by MRI.The results in two groups were compared. Results In Lesion group,the slope angle of lateral tibial plateau(8.10°±2.17°)was greater than in control group(4.84°±2.21°,P0.001).The slope angle of medial tibial plateau(5.99°±2.10°)was lower than in control group(6.96°±2.24°),but there was no statistically significant difference(P=0.055). Conclusion The tibial slopes of medial condyle and lateral condyle may have the opposing effects on the ACL Lesion.The greater tibial slope of the lateral tibial plateau may be a risk factor for the injury of anterior cruciate Ligament.

    This study examines the effect of the tibial slope on the anterior cruciate Ligament Lesion (separately on the lateral and medial tibial condyle). In the examined group, the lateral tibial plateau was significantly greater than in the control group (P < 0.001), and the medial tibial plateau had lower tibial slope values than the control group; however, the difference was not statistically significant (P = 0.066). In both groups, the difference between the slopes on the lateral and medial plateaus was statistically significant (P < 0.001). In relation to ACL intact patients, population with ACL rupture have greater tibial slope of the lateral condyle. The greater tibial slope of the lateral tibial plateau may be the factor that leads to the injury of the anterior cruciate Ligament. Compared to the medial plateau, the population with ACL rupture have a greater tibial slope on the lateral plateau, while the population of the intact ACL have greater tibial slope on the medial plateau. The tibial slope of the medial and lateral condyle should be compared separately because the values obtained from the two sets of data were different, revealing apparently opposing effects on the ACL Lesion.

    Abstract:

    Study design. Using helical computed tomography with three-dimensional and multiplanar reconstruction, ossification of the posterior longitudinal Ligament in the cervical and thoracic region was observed. Preoperative simulation also was performed, and the availability of these methods was evaluated. Objective. To use preoperative evaluation and simulation with helical computed tomography to enhance the accuracy of excision of ossification of the posterior longitudinal Ligament Lesion. Summary of Background Data. Ossification of the posterior longitudinal Ligament Lesion is sometimes so complicated that preoperative morphologic evaluation and excision of the Lesion are difficult when using only conventional imaging techniques. Methods. Seven cases of cervical and two cases of thoracic ossification of the posterior longitudinal Ligament were scanned using helical computed tomography at 2-mm or 5-mm slice thickness. Three-dimensional and multiplanar reconstruction were performed at 0.7-mm or 2-mm intervals in the bone window. Surgical simulation of the anterior approach for cervical Lesion and posterior approaches for thoracic Lesion was performed. Results. Preoperative direct observation of the ossification of the posterior longitudinal Ligament Lesion was possible, and the complicated structures could be understood more easily than with other conventional methods. When surgical simulation was performed in the workstation, the ossification of the posterior longitudinal Ligament Lesion was removed sufficiently on arbitrarily reconstructed view in the spinal canal. When the viewpoint then was changed to the approaching side, the location and dimension of the removed area were determined. In all cases, surgical approach and excision of the ossification of the posterior longitudinal Ligament Lesion were performed more easily and more precisely than in the surgery with no three-dimensional images. Conclusion. Helical computed tomography with three-dimensional, multiplanar reconstruction would be a valuable tool for evaluation and surgical simulation of ossification of the posterior longitudinal Ligament Lesion by enhancing the accuracy of the surgical procedure.
