  • Detection of Disseminated Cancer Cells in Linitis Plastica-type Gastric Carcinoma

    Background: Dissemination of cancer cells in the abdominal cavity may lead to peritoneal carcinomatosis, a common event in the Linitis Plastica type of gastric carcinoma. Reverse transcriptase polymerase chain reaction (RT-PCR) is a sensitive technique for detecting these cells. Methods: Peritoneal washings were obtained during laparotomy from 47 consecutive Linitis Plastica patients who preoperatively were considered candidates for curative resection. Together with conventional cytological examination using Papanicolaou staining, real-time RT-PCR was performed to quantitate prospectively carcinoembryonic antigen (CEA) mRNA in these washings. Samples above a cutoff value for CEA mRNA were considered positive for molecular detection of disseminated cancer cells. Results: Conventional cytological examination was positive in 43% (20/47). Positivity of CEA mRNA was much higher at 83% (39/47) and peritoneal carcinomatosis actually was observed in 77% (36/47), either at laparotomy or during postoperative follow-up. Although only two of eight patients who were negative for CEA mRNA suffered from peritoneal carcinomatosis, three more patients died of recurrences through other metastatic pathways. Multivariate analysis revealed that curability of the operation (knowledge of the CEA mRNA values excluded) and Tcategories were the only significant independent prognostic factors. Conclusions: RT-PCR of the peritoneal washes had little value as a prognostic factor, but identified over 80% of the patients planned for curative surgery to have disseminated cancer cells in the peritoneal cavity. Almost the same proportion of patients actually suffered from peritoneal carcinomatosis. These facts indicate why treatment with surgery alone rarely cures patients with Linitis Plastica.

    The term “Linitis Plastica” or leather-bottle stomach was introduced by William Brinton in 1858. This Greek eponym literally translates as “linen cloth” and refers to the characteristic fibrous proliferation noted on histology. Although largely accepted as a morphologic description of diffuse infiltrative gastric adenocarcinoma, the term Linitis Plastica is also used in the medical literature to describe the lesions caused by lye ingestion and diffuse gastric metastasis. Although less than 30 % of individuals with gastric cancer have signet-ring cell histology, it accounts for the majority of patients who present with Linitis Plastica [1]. Our case highlights the diagnostic difficulties associated with Linitis Plastica. In addition, it describes the rare association of diffuse signet-ring cell gastric adenocarcinoma and pernicious anemia and attempts to define a pathophysiologic correlation between the two.

    Abstract:

    Background Linitis Plastica (scirrhous gastric carcinoma) has a poor prognosis due to delay in diagnosis. Pathological feature of this tumor is diffuse fibrosis of the gastric wall. Detection of small fibrotic lesion in the gastric wall would contribute to early diagnosis of Linitis Plastica, since the primary lesion usually lacks remarkable protrusion or ulceration. Methods We performed a multislice CT evaluation of 20 patients with gastric carcinoma with 8 data acquisition system (Aquilion, Toshiba Medical Systems Corporation, Japan). Results Out of 20 primary lesions, 3 were diagnosed as relatively small size (less than 10 cm in maximum diameter) Linitis Plastica. We have successfully demonstrated an en face virtual endoscopic image of the primary ulcers of Linitis Plastica by arterial-phase, and reactive fibrosis (a desmoplastic response) of the gastric wall by delayed-phase multiplanar reformation (MPR) images perpendicular to the en face image. This preoperative information was useful to evaluate extent of tumor invasion. Conclusions The multislice CT evaluation of Linitis Plastica using delayed-phase images was potentially useful in the detection of relatively small fibrotic lesion and in determining the optimal mode of resection of the stomach.

    Secondary rectal Linitis Plastica is a very rare malignancy with poor prognosis. Diagnosis is difficult because of nonspecific clinical and endoscopic findings and negative biopsies in most cases owing to the fact that the mucosa is frequently unaffected. We herein describe a 68-year-old man who presented with a six-month history of tenesmus and constipation. Endoscopy revealed a narrow distal rectum with an indurated, cobblestone appearance of mucosa. Multiple biopsies and fine-needle aspiration were negative for malignancy. Abdominal MRI and transrectal ultrasonography showed findings compatible with rectal Linitis Plastica. He underwent rectal extirpation with total cystectomy and lymph nodes dissection. Histology demonstrated secondary rectal Linitis Plastica due to a poorly differentiated urinary bladder carcinoma. We emphasize the endoscopic and endosonographic features and the difficulty to establish a preoperative diagnosis of secondary rectal Linitis Plastica.
