compensatory class iii Malocclusion treatment associated with mandibular canine extractions

However, if there are no significant soft tissue implications and the patient does not want to undergo orthognatic surgery, other treatment options may be considered. The current case report describes a compensatory alternative for Class III Malocclusion treatment, by means of mandibular canine extractions. This treatment alternative provided facial profile and occlusal improvement, which remains stable seven years posttreatment.

Methods The sample consisted of dental casts from 165 subjects that were divided into 2 groups: 115 normal occlusion subjects (mean age, 14.3 years) and 50 complete Class II Division 2 subjects (mean age, 13.9 years). Dental wear was assessed by using a modified version of the tooth wear index. The 2 groups were compared with the Mann-Whitney test for the frequency and severity of wear on each surface of each group of teeth. The level of statistical significance was set at 5%. Results The normal occlusion group statistically had greater tooth wear on the incisal surfaces of the maxillary lateral incisors and the incisal surfaces of the maxillary canines than did the Class II Division 2 Malocclusion group. The Malocclusion group showed statistically greater tooth wear on the labial surfaces of the mandibular lateral incisors, the occlusal surfaces of the maxillary premolars and first molars, the occlusal surfaces of the mandibular premolars, the palatal surfaces of the maxillary second premolars, and the buccal surfaces of the mandibular premolars and first molars than did the normal occlusion group. Conclusions Subjects with normal occlusion and those with complete Class II Division 2 Malocclusions have different tooth-wear patterns. Tooth wear on the Malocclusion subjects should not be considered pathologic but, rather, the consequence of different interocclusal arrangements.

The aim of this systematic review was to evaluate the true effects of Class II elastics in Class II Malocclusion treatment. Methods A search was performed on PubMed, Scopus, Web of Science, Embase, Medline, and Cochrane databases, complemented by a hand search. Study eligibility criteria were the application of Class II elastics in Class II Malocclusion treatment and the presentation of dental or skeletal outcomes of treatment. All age groups were included. Results The search indentified 417 articles, of which 11 fulfilled the inclusion criteria. Four studied the isolated effects of Class II elastics, and 7 were comparisons between a single use of elastics and another method for Class II Malocclusion correction. Because of the differences in treatment modalities in these articles, a meta-analysis was not possible. Conclusions Based on the current literature, we can state that Class II elastics are effective in correcting Class II Malocclusions, and their effects are primarily dentoalveolar. Therefore, they are similar to the effects of fixed functional appliances in the long term, placing these 2 methods close to each other when evaluating treatment effectiveness. Little attention has been given to the effects of Class II elastics on the soft tissues in Class II Malocclusion treatment.

Methods A sample of 59 records from patients with complete Class II Malocclusion was used. This sample was divided into 2 groups with the following characteristics: group 1, comprising 29 patients treated without extractions, and group 2, comprising 30 patients treated with extraction of 2 maxillary premolars. Dental cast measurements were obtained before and after treatment and at a minimum of 2.4 years after treatment. The pretreatment, posttreatment, and postretention occlusal statuses were evaluated with the peer assesment rating index. The occlusal indexes at the postretention stage and the posttreatment changes and percentages of posttreatment changes were compared with t tests. Results The nonextraction and the 2 maxillary premolar extraction treatment protocols of complete Class II Malocclusions had no statistically significant differences in occlusal stability. Conclusions Finishing Class II Malocclusion treatment with the molars in a Class II relationship has similar occlusal stability as finishing with the molars in a Class I relationship.

The secondary objective was to compare the dentoskeletal asymmetries in these 2 types with a group of normal-occlusion subjects by using submentovertex and posteroanterior radiographs. Methods: The experimental group included 44 untreated Class II subdivision Malocclusion subjects with a mean age of 15.3 years. The control group included 30 subjects with normal occlusions with a mean age of 22.4 years. All had full complements of permanent teeth up to the first molars and had not received orthodontic treatment. Type 1 Class II subdivision Malocclusion is coincidence of the maxillary dental midline with the facial midline and deviation of the mandibular midline. Type 2 has the opposite characteristics. The frontal photographs were evaluated subjectively by 2 examiners. In the submentovertex and posteroanterior radiographs, symmetry was assessed by measuring the relative difference in the spatial positions of dentoskeletal landmarks between the right and left sides. Independent t tests were used to compare the dentoskeletal asymmetries of types 1 and 2 with the normal-occlusion group. Results and Conclusions: The results showed that 61.36% had type 1, 18.18% had type 2 Class II subdivision Malocclusion, and 20.45% had mixed characteristics. The predominant asymmetric dentoalveolar characteristics of both types of Class II subdivision Malocclusions were evident when individually compared with a normal-occlusion control group. There was a tendency for the type 1 subjects to have greater mandibular asymmetry than type 2, as compared with the control group.

Material and Methods: A cross-sectional study was conducted in a sample of 276 adolescents (159, 52 and 65 with Angle Class I, II and III Malocclusions, respectively) from Trujillo, Peru. Participants were asked to complete the Rosenberg Self-Esteem Scale (RSES) and were also clinically examined, so as to have Angle Malocclusion classification determined. Analysis of covariance (ANCOVA) was used to compare RSES scores among adolescents with Class I, II and III Malocclusions, with participants' demographic factors being controlled. Results: Mean RSES scores for adolescents with Class I, II and III Malocclusions were 20.47 ± 3.96, 21.96 ± 3.27 and 21.26 ± 4.81, respectively. The ANCOVA test showed that adolescents with Class II Malocclusion had a significantly higher RSES score than those with Class I Malocclusion, but there were no differences between other Malocclusion groups. Supplemental analysis suggested that only those with Class II, Division 2 Malocclusion might have greater self-esteem when compared to adolescents with Class I Malocclusion. Conclusion: This study shows that, in general, self-esteem did not vary according to adolescents' Malocclusion in the sample studied. Surprisingly, only adolescents with Class II Malocclusion, particularly Class II, Division 2, reported better self-esteem than those with Class I Malocclusion. A more detailed analysis assessing the impact of anterior occlusal features should be conducted.

Results The normal occlusion group statistically had greater tooth wear on the incisal surfaces of the maxillary lateral incisors and the incisal surfaces of the maxillary canines than did the Class II Division 2 Malocclusion group. The Malocclusion group showed statistically greater tooth wear on the labial surfaces of the mandibular lateral incisors, the occlusal surfaces of the maxillary premolars and first molars, the occlusal surfaces of the mandibular premolars, the palatal surfaces of the maxillary second premolars, and the buccal surfaces of the mandibular premolars and first molars than did the normal occlusion group. Conclusions Subjects with normal occlusion and those with complete Class II Division 2 Malocclusions have different tooth-wear patterns. Tooth wear on the Malocclusion subjects should not be considered pathologic but, rather, the consequence of different interocclusal arrangements.

Methods The sample consisted of dental casts obtained from 310 subjects, divided into 3 groups: group 1, 110 subjects with normal occlusion (mean age, 13.51 years); group 2, 100 complete Class II Division 1 patients (mean age, 13.44 years); and group 3, 100 half-cusp Class II Division 1 patients (mean age, 13.17 years). Dental wear was assessed by using a modified version of the tooth-wear index. The 3 groups were compared by means of the Kruskal-Wallis and Dunn tests, considering the frequency and the severity of wear on each surface of each group of teeth. The level of statistical significance was set at 5%. Results The normal occlusion group had statistically greater tooth wear on the palatal surfaces of the maxillary central incisors and the incisal surfaces of the maxillary canines than the corresponding surfaces in both Class II Malocclusion groups. The complete and half-cusp Class II Division 1 Malocclusion groups had statistically greater tooth wear on the occlusal surfaces of the maxillary second premolar and first molar, the occlusal surfaces of the mandibular premolars, and the buccal surfaces of the mandibular posterior teeth compared with the normal occlusion group. The half-cusp Class II Division 1 Malocclusion group had significantly greater tooth wear on the incisal surfaces of the mandibular incisors compared with the complete Class II Division 1 Malocclusion group.
