    Recently, many neuroendocrine mediators have been reported to be related to the formation and enlargement of the neoplasm. From our electron microscopic analysis of the gastric MALT Lymphoma, many unmyelinated nerve fiber-like structures have been detected especially in the early stage of the tumor formation. Thus, the present study was undertaken to clarify the localization of choline acetyltransferase (ChAc), substance P, its receptor NK-1R, CGRP and RAMP-1, in the MALT Lymphoma tissues. The effect of eradication of the bacilli as well as spantide II, a substance P antagonist, was also investigated. Materials and Methods The gastric, hepatic and pulmonary MALT Lymphoma was formed by the per oral administration of homogenized gastric mucosa from Helicobacter heilmannii-infected mice. Conclusion: Long-term infection with Helicobacter heilmannii in C57BL/6 mouse induced lowand high-grade hepatic and pulmonary MALT Lymphoma as well as gastric Lymphoma. Eradication of the bacteria brought about shrinkage of the hepatic and pulmonary lesions as well as gastric MALT Lymphoma.

    Abstract:

    Helicobacter heilmannii (Hh) has been clinically reported to have some relation to gastric low grade MALT Lymphoma. Recently, we have formed the gastric MALT Lymphoma in C57BL/6 mice. Materials and methods: C57BL/6 mice infected with Hh from cynomolgus monkey for more than 6 months were used. The macroscopic, immunohistochemical and electron microscopic observation was performed Results: MALT Lymphoma was formed in almost 100 % of the infected mice. Increased VEGF-A and Flt-3 immunore- activity was recognized. Conclusion: Hh was shown to be related to the formation of MALT Lymphoma and VEGF is suggested to play a role in this Lymphoma.

    BACKGROUND: The hepatic and pulmonary MALT Lymphoma (mucosa-associated lymphoid tissue Lymphoma) is clinically occasionally observed but its pathogenesis is unknown and thought to be important to establish the treatment strategy. OBJECTIVES: The present study was designed to clarify the characteristics of these Lymphomas and the effect of Helicobacter eradication regimen and substance P antagonist. METHODS: After the long term infection of Helicobacter suis to the C57BL/6 mice stomach, the whole organ was surveyed pathologically. Histochemical characteristics of the lesion and the localization of bacteria was observed. In addition, the effect of the administration of antibiotics and a proton pump inhibitor or the substance P antagonist was investigated. RESULTS: We have detected the hepatic and pulmonary MALT Lymphoma after the long term infection. In situ hybridization study revealed the positive reaction of Helicobacter suis in the hepatic and pulmonary MALT Lymphoma. After the administration of antibiotics and a proton pump inhibitor, the bacterial number has significantly decreased and the tumor size in the fundus, liver and lung markedly reduced. Substance P immunoreactivity was clearly shown in the Lymphoma cells in the liver and lung, and the spantide II administration induced the marked decrease in the size of tumors. CONCLUSION: By our experiments using the long term infection of Helicobacter suis to the C57BL/6 mice, we have detected the liver and pulmonary MALT Lymphoma. In situ hybridization study suggested the direct interaction of this bacterium to the etiology of these lesions. Substane P within the Lymphoma cells was suggested to work on the maintenance of the extragastric MALT Lymphoma.

    microRNAs (miRNAs) are small non-coding RNAs that can function as endogenous silencers of target genes and play critical roles in human malignancies. To investigate the molecular pathogenesis of gastric mucosa-associated lymphoid tissue (MALT) Lymphoma, the miRNA expression profile was analyzed. miRNA microarray analysis with tissue specimens from gastric MALT Lymphomas and surrounding non-tumor mucosae revealed that a hematopoietic-specific miRNA miR-142 and an oncogenic miRNA miR-155 were overexpressed in MALT Lymphoma lesions. The expression levels of miR-142-5p and miR-155 were significantly increased in MALT Lymphomas which do not respond to Helicobacter pylori (H. pylori) eradication. The expression levels of miR-142-5p and miR-155 were associated with the clinical courses of gastric MALT Lymphoma cases. Overexpression of miR-142-5p and miR-155 was also observed in Helicobacter heilmannii-infected C57BL/6 mice, an animal model of gastric MALT Lymphoma. In addition, miR-142-5p and miR-155 suppress the proapoptotic gene TP53INP1 as their target. The results of this study indicate that overexpression of miR-142-5p and miR-155 plays a critical role in the pathogenesis of gastric MALT Lymphoma. These miRNAs might have potential application as therapeutic targets and novel biomarkers for gastric MALT Lymphoma.

    Helicobacter pylori infection is a pre-MALT Lymphoma condition. H pylori eradication leads to complete remission in 80% of low grade stage E1 Lymphomas, with a yearly recurrence rate of approximately 5%. The possibility for complete remission in high grade Lymphomas needs to be investigated in prospective studies. In addition, the significance of persistent B cell monoclonality (stable disease? danger of relapse? regression of monoclonality?) needs to be investigated in follow up studies.

    Abstract Background & Aims: Cure of Helicobacter pylori infection may lead to complete remission of associated low-grade mucosa-associated lymphoid tissue (MALT) Lymphoma in stage EI. This study investigated whether Helicobacter heilmannii infection–associated primary gastric MALT Lymphoma will regress after cure of the infection. Methods: H. heilmannii –induced gastritis was diagnosed histologically, by a new specific immunoglobulin G enzyme-linked immunosorbent assay, and with 16S ribosomal RNA amplification and sequencing in 5 consecutive patients with primary gastric MALT Lymphoma clinical stage EI. Patients received 40 mg omeprazole and 750 mg amoxicillin 3 times per day for 14 days. Polymerase chain reaction (PCR) was used to detect rearrangement of immunoglobulin heavy-chain genes before treatment and during follow-up. Results: Five patients (3 men, 2 women; mean age, 65 years; range, 42–79 years) were studied. H. pylori was not detected by culture, histology, serology, or PCR. Treatment resulted in the cure of H. heilmannii infection in each case and complete histological and endoscopic remission of the tumors. Three of 5 patients showed monoclonal B cells before treatment, 2 of whom remained PCR positive. Within a median follow-up period of 24 months, no relapse of the Lymphoma or reinfection with H. heilmannii occurred. Conclusions: These data suggest that gastric MALT Lymphoma may arise in patients with H. heilmannii infection. Cure of this infection may lead to complete remission of the MALT Lymphoma.     Abstract BACKGROUND & AIMS: Recent studies suggest that eradication of Helicobacter pylori may result in complete regression of low-grade Lymphoma of the gastric mucosa-associated lymphoid tissue (MALT). Which patients benefit from this treatment is unknown. The aim of this study was to prospectively study whether staging by echoendoscopy predicts the outcome of treatment of MALT Lymphoma by eradication of H. pylori. METHODS: Twenty-two patients positive for H. pylori with low-grade gastric B-cell MALT Lymphoma were examined by echoendoscopy at the time of diagnosis. Treatment for H. pylori consisted of a 2-week course of oral omeprazole and amoxicillin. Patients were followed up by endoscopy and biopsy. RESULTS: H. pylori was eradicated in all patients. Complete regression of MALT Lymphoma was observed in 12 patients (54%), partial regression in 5 patients (23%), and no response in 5 patients (23%) during follow-up. Twelve of 14 patients with Lymphoma restricted to the mucosa or submucosa (stage E-I1) at echoendoscopy, but none of the 10 patients with higher stage (P CONCLUSIONS: Staging of gastric low-grade MALT Lymphomas by endoscopic ultrasonography allows prediction of the response to therapy by eradication of H. pylori.
