    Abstract:

    Abstract Synovial chondromatosis (SC) is a reactive joint disorder characterized by the development of metaplastic cartilaginous nodules floating within the joint space. We report a case of SC of the temporoMandibular joint (TMJ) which had difficulty in diagnosis in a 40-year-old woman. The patient had TMJ pain with Mandibular lateral deviation for two years. CT and MRI revealed well-defined and round-shaped bone resorption at the right Mandibular Fossa and an ovoid-shaped expansion of the superior articular cavity, which showed heterogeneous high signal intensity on T2-weighted MRI. Following a clinical diagnosis of benign TMJ tumor, the lesion was surgically removed. Histopathologically, it was diagnosed as SC in Milgram’s stage 1, mainly involving the upper joint space. This is the first case report of SC in an early stage in which no cartilaginous nodules were generated but instead the Mandibular Fossa of the temporal bone was resorbed.

    Abstract:

    Bone changes in the Mandibular head and Mandibular Fossa in 33 patients with TMJ disorders were studied with axial computed tomography in relation to clinical findings to clarify possible factors leading to bone changes in this phenomenon. Bone changes of the Mandibular head were observed in 45 (68%) of the 66 TMJs. The Mandibular head was juxtaposed to the Mandibular Fossa in 13 (29%) of the 45 joints in centric occlusion and in 29 joints (64%) in the anterior position on CT, whereas the Mandibular head with no pathological bone change was juxtaposed to the Mandibular Fossa in none of the 21 joints in centric occlusion and in only 1 joint (5%) in the anterior position. In the two groups of patients with and without juxtaposition of the Mandibular head and Mandibular Fossa with bone changes, the incidence of the initial symptoms such as pain. crepitus, and difficulty in opening the mouth was increased compared with the symptoms at presentation. However, the former group had severer symptoms than the latter group. These findings suggest that bony degeneration of the TMJ is accelerated by juxtaposition of the head and Fossa.

    Objective During therapy of distoclusion entailing a rigid, fixed orthodontic appliance, the Mandibular Fossa and condyle are ideally remodeled, while dentoalveolar effects occur through adaptive mechanisms. Adaptive processes, especially in the Fossa region, have not been adequately investigated. Our magnetic resonance imaging (MRI) investigation aimed to assess the effects of therapy with a functional Mandibular advancer (FMA) on Mandibular Fossa morphology. Patients and methods We monitored via MRI the therapeutic course of 25 patients at three time points. Visual findings and metric assessments were carried out in the sagittal plane. Three-dimensional (3D) reconstructions of the joint structure of two exemplary patients were also made. Results Visual examinations of the MRI slices at the three time points revealed no changes in Fossa shape in any of the 50 temporoMandibular joints. Lateral comparisons showed that the morphology of the Fossae of all 25 patients was identical. Metric analysis demonstrated no significant alterations in width, depth, or in their ratio, not even laterally. Nine measurements of the distances between the porion, Mandibular Fossa, and articular eminence revealed no significant changes in total or on the left and right sides, or intralaterally. Conclusion The visual findings and metric analyses of parasagittal MRI slices did not indicate any morphological changes in the Mandibular Fossa or articular eminence in patients with distoclusion treated via a rigid, fixed orthodontic appliance. However, special reworking of the MRI data facilitated reconstruction of the surfaces of joint structures in 3D. This new method makes it possible to depict more accurately and noninvasively the adaptive mechanisms not ascertainable via metric methods and to assess them as 3D structures. Zielsetzung Bei der Behandlung von mandibulären Retrognathien mit starren, festsitzenden Apparaturen erfolgen neben dentoalveolären Effekten im Idealfall auch Remodellationen der Fossa Mandibularis und der Kondylen als adaptive Mechanismen. Insbesondere die möglichen Adaptationsvorgänge im Bereich der Fossa wurden bislang jedoch nicht umfangreich untersucht. Ziel der vorliegenden magnetresonanztomographischen Studie war es deshalb, quantitativ und qualitativ zu überprüfen, welche Auswirkungen eine Behandlung mit einem Functional Mandibular Advancer (FMA) bei der Korrektur der mandibulären Retrognathie auf die Morphologie der Fossa Mandibularis hat. Methoden Die Behandlungsverläufe von 25 Patienten wurden zu 3 definierten Zeitpunkten magnetresonanztomographisch kontrolliert. Visuelle Befundung und metrische Auswertungen erfolgten in der Sagittalebene. Bei 2 exemplarischen Beispielen wurden 3‑dimensionale Rekonstruktionen der Gelenkstrukturen vorgenommen. Schlussfolgerungen Bei der Behandlung mit einer starren, festsitzenden, funktionskieferorthopädischen Apparatur zur Korrektur von mandibulären Retrognathien geben weder die visuelle Befundung noch verschiedene metrische Analysen an parasagittalen MRT-Schichten Hinweise auf morphologische Veränderungen der Fossa Mandibularis und des Tuberculum articulare. Eine spezielle Aufbereitung des MRT-Datenmaterials ermöglicht 3‑dimensionale Oberflächenrekonstruktionen der Gelenkstrukturen. Dadurch könnten in Zukunft adaptive Mechanismen besser dargestellt und noninvasiv am 3‑D-Objekt bewertet werden.

The samples were classified according to age a nd dentalstate (dentate, partially dentate and totally edentulous) and a multivariate analysis was employed. Subjects average age was 41 .1 (20-85).There were more partially dentate individuals (59.16%) than dentate (30%) or edentulous ones (10.83%). Average Fossa depth was 6.9mm (6.6-7.1 mm 95%CI) which significantly decreased with increased age and dental health deterioration (p<0.01). A period of fiveyears edentulism increased the likelihood of Mandibular Fossa flattening (<6.9 mm) by 20.6% and 7.2% in dentate ones. Our findingsfound an association between dental state and age as determinant factors in reducing Mandibular Fossa depth.KEY WORDS: TemporoMandibular joint; Mandibular Fossa; Dental condition; Articular tubercule; Age groups.
