    Abstract:

    Accumulating research shows that prenatal exposure to Maternal Stress increases the risk for behavioral and mental health problems later in life. This review systematically analyzes the available human studies to identify harmful Stressors, vulnerable periods during pregnancy, specificities in the outcome and biological correlates of the relation between Maternal Stress and offspring outcome. Effects of Maternal Stress on offspring neurodevelopment, cognitive development, negative affectivity, difficult temperament and psychiatric disorders are shown in numerous epidemiological and case-control studies. Offspring of both sexes are susceptible to prenatal Stress but effects differ. There is not any specific vulnerable period of gestation; prenatal Stress effects vary for different gestational ages possibly depending on the developmental stage of specific brain areas and circuits, Stress system and immune system. Biological correlates in the prenatally Stressed offspring are: aberrations in neurodevelopment, neurocognitive function, cerebral processing, functional and structural brain connectivity involving amygdalae and (pre)frontal cortex, changes in hypothalamo-pituitary-adrenal (HPA)-axis and autonomous nervous system.

  • The Effects of Prenatal Maternal Stress on Early Temperament: The 2011 Queensland Flood Study.

    Abstract:

    This study examined the effects of disaster-related prenatal Maternal Stress on infant temperament and whether the sex of the infant or the timing of the Stressor in pregnancy would moderate the effects. Mothers' objective experiences of a sudden-onset flood in Queensland, Australia, their subjective emotional reactions, and cognitive appraisal of the event were assessed. At 6 months postpartum, 121 mothers reported their infant's temperament on the 5 dimensions of the Short Temperament Scale for Infants. When controlling for postnatal Maternal factors, subjective prenatal Maternal Stress and cognitive appraisal of the disaster were associated with easier aspects of infant temperament. However, several interesting interactions emerged showing negative effects of the flood. With higher levels of objective hardship in pregnancy, boys (but not girls) received more irritable temperament ratings. When the flood occurred early in pregnancy, higher levels of objective hardship predicted more arrhythmic infant temperament. Finally, mothers whose emotional response to the flood exceeded the hardship they endured reported significantly more active-reactive infants. Prenatal Maternal Stress from a natural disaster predicted more difficult temperament ratings that were moderated by infant sex, timing of the flood in gestation, and mother's emotional response to the disaster.

    Abstract:

    Fetuses are exposed to many environmental perturbations that can influence their development. These factors can be easily identifiable such as drugs, chronic diseases or prenatal Maternal Stress. Recently, it has been demonstrated that the serotonin synthetized by the placenta was crucial for fetal brain development. Moreover, many studies show the involvement of serotonin system alteration in psychiatric disease during childhood and adulthood. This review summarizes existing studies showing that prenatal Maternal Stress, which induces alteration of serotonin systems (placenta and fetal brain) during a critical window of early development, could lead to alteration of fetal development and increase risks of psychiatric diseases later in life. This phenomenon, termed fetal programming, could be moderated by the sex of the fetus. This review highlights the need to better understand the modification of the Maternal, placental and fetal serotonin systems induced by prenatal Maternal Stress in order to find early biomarkers of psychiatric disorders.

    To determine whether disaster-related prenatal Maternal Stress and Maternal illness during pregnancy predict Maternal-rated temperament status in 6-mo-old infants. The temperamental status of 121 infants (60 boys and 61 girls) exposed in utero to varying degrees of Maternal Stress and/or illness during either first (n = 40), second (n = 43), or third (n = 38) trimester of pregnancy was assessed using the Infant Characteristics Questionnaire. Higher levels of Maternal subjective diStress and illness were primarily independently associated with poorer temperamental status in the infants. Maternal subjective diStress explained 3.4, 3.1, and 9.8% and early pregnancy illness explained 4.3, 5.8, and 2.9% of the variance of the infants’ fussy/difficult, dullness, and needs attention temperament dimensions, respectively. This is the first study to assess whether temperament status is influenced by disaster-related prenatal Maternal Stress. Moreover, this is the first study to assess whether Maternal Stress and illness during pregnancy interact to determine infant temperament status. The findings suggest that while both factors predict temperament status at 6 mo, they do so primarily in an independent manner. These results suggest that pathways through which Maternal Stress and illness during pregnancy influence temperament status differ.

    Animal studies of prenatal Maternal Stress permit random assignment of pregnant animals to Stress and no-Stress groups, and allow total control of the type, severity, and timing of the Stressor in utero. Human studies have obvious constraints that make the use of experimental methods nearly impossible. Studying pregnant women who experience natural disasters during pregnancy, however, approximates the random assignment to groups enjoyed by animal studies, and can characterize the timing of the Stressor in utero with great precision. In this chapter, we briefly describe our three ongoing prospective longitudinal studies of children exposed to prenatal Maternal Stress from natural disasters. We present results from Project Ice Storm in detail, showing effects of prenatal Maternal Stress on cognitive and neurodevelopment. We contrast these results with preliminary findings from the Iowa Flood Study and introduce the QF2011 Queensland Flood Project. In the “Discussion” section, we present conclusions to date and discuss the relative effects of the severity of Maternal objective disaster exposure and Maternal subjective diStress levels, the moderating effects of fetal sex and the timing of the Stressor in utero, and the longevity of the effects. Finally, we discuss some possible mechanisms that may mediate the effects of prenatal Maternal Stress on the neurodevelopment of children.

    There exists considerable research on the effects of prenatal Maternal Stress on offspring. Animal studies, using random assignment to experimental and control groups, demonstrate the noxious effects of prenatal Maternal Stress on physical, behavioural and cognitive development. The generalizability of these results to humans is problematic given that cognitive attributions moderate reactions to Stressors. In humans, researchers have relied upon Maternal anxiety or exposure to life events as proxies for the Stressors used with animals. Yet, the associations between Maternal anxiety or potentially non-independent life events and problems in infants are confounded by genetic transmission of temperament from mother to child. We summarize the literature on prenatal Maternal Stress and infant cognitive development, leading to the conclusion that the human literature lacks the ability to separate the effects of the objective exposure to a Stressor and the mother's subjective reaction. We then describe our prospective Project Ice Storm in which we are following 150 children who were exposed in utero to a natural disaster. We demonstrate significant effects of the objective severity of exposure on cognitive and language development at age two years with important moderating effects of the timing during pregnancy. The implications of our findings are discussed.

    Telomeres play an important role in maintaining chromosomal integrity. With each cell division, telomeres are shortened and leukocyte telomere length (LTL) has therefore been considered a marker for biological age. LTL is associated with various lifetime Stressors and health-related outcomes. Transgenerational effects have been implicated in newborns, with Maternal Stress, depression, and anxiety predicting shorter telomere length at birth, possibly reflecting the intrauterine growth environment. Previous studies, with relatively small sample sizes, have reported an effect of Maternal Stress, BMI, and depression during pregnancy on the LTL of newborns. Here, we attempted to replicate previous findings on prenatal Stress and newborn LTL in a sample of 1405 infants using a qPCR-based method. In addition, previous research has been expanded by studying the relationship between Maternal sleep quality and LTL. Maternal prenatal Stress, anxiety, depression, BMI, and self-reported sleep quality were evaluated with self-reported questionnaires. Despite sufficient power to detect similar or even considerably smaller effects than those previously reported in the literature, we were unable to replicate the previous correlation between Maternal Stress, anxiety, depression, or sleep with LTL. We discuss several possible reasons for the discrepancies between our findings and those previously described.

    Preterm birth defined as birth prior to 37 weeks of gestation is caused by different risk factors and implies an increased risk for disease and early death for the child. The aim of the study was to investigate the effect of Maternal Stress during pregnancy on the risk of preterm birth. A case–control study that included 340 women; 168 women who gave birth preterm and 172 women who gave birth at term. Data were manually extracted from standardized medical records. If the medical record contained a psychiatric diagnosis or a self-reported Stressor e.g., depression or anxiety the woman was considered to have been exposed to Stress during pregnancy. Adjusted odds ratio (AOR) was used to calculate the attributable risk (AR) of Maternal Stress during pregnancy on preterm birth, both for the women exposed to Stress during pregnancy (AR1 = (AOR-1)/AOR) and for the whole study population (AR2 = AR1*case fraction).
